Registration Submission:
The submission of this registration form does not guarantee a roster spot since we have a limited amount of spots available and each team
can only accept 6 heavy players. We will do our best to find your son a spot on a team and will have waiting lists if needed to form more
teams. By submitting this form, you are giving permission for us to charge your credit card for the amount below. You will be notified
within 3-5 days via e-mail if his spot has been secured. No charges will be made to your credit card unless he has been placed. All spots are
available on a first-come, first served basis. Requests can be made to play with a friend and we will do our best to accommodate you.
Division Weight Description: AGE ON 9/1/2015
Widgets: 8 & 9 yr olds, up to 90 lbs. 6 players are allowed up to 100 lbs. Max. of 9 yrs. old on 9/1/2015.
Peewee: 10 & 11 yr olds, up to 110 lbs. 6 players are allowed up to 115 lbs. or older lighter of 12 year old under
95 lbs.
Lightweight: 12 & 14 yr olds, up to 130 lbs. 6 players are allowed up to 140 lbs. Max. of 14 yrs. old on 9/1/2015.
Regular players may weigh up to the weight limit in each division with NO restrictions on handling the ball. Each division is allowed a
maximum of 6 players (heavies) per team however; they are NOT allowed to handle the ball with the exception of the lightweight division.

after that date) then determine which weigh bracket applies.

1 Since the Megaplex has approximately 8 teams, it is not possible to contact each parent individually. We provide two
means of communication: 1) E-mail or text– This is the easiest way for us or your team mom to relay information to you
regarding practices, games, paperwork etc. Please check your e-mail daily for important information. 2) Website/Facebook –
We post practices, changes & game times on our Facebook page & periodically our website for easy access by parents.
2 Your child’s participation at practices and games is required. If your son participates in a spring sport, please check the
practice & game schedule before committing to football. Should your son miss practices or games due to another sport, your
sons position will be jeopardized and he will not be guaranteed any playing time. We do not want to experience changes at
the end of our season due to conflicts with other sports and jeopardize our teams’ success.
3. Due to the demands on indoor training facilities, practice is only 2 days per week for approximately 1 hour. If scheduling
allows additional time, the coach may extend or add an additional practice. All practices with tackling drills will take place on
the turf field, conditioning practices and offense/defense play run through may be on rink or court.
4. I further agree and acknowledge that all equipment and uniforms are the sole property of the Megaplex and that said
equipment and uniforms are loaned for the current season only. All loaned items must be returned, upon request, in clean and
good condition. I further agree that failure to return said equipment in whole or in part will result in financial responsibility on
my part for the full replacement cost of the equipment and uniforms not returned. An equipment deposit in the amount of
$100.00 is made payable to the Megaplex which will be returned once all equipment and uniform is checked back in.
5. Players wear gym shoes (no cleats). Players will need mouthpiece, practice jersey/pants, and girdle. MPX supplies helmet,
shoulder pads, hip/butt/thigh/knee pads, and uniform pants and game jersey.
6. In lieu of a fundraiser, the Megaplex charges an admission fee of $1.00 for game day (Children 2 yrs. and under
free).
7. I further agree and acknowledge that, when participating in the MPX activities, my child may be photographed for print,
video, or electronic imaging. I agree that the images will be used in promotional materials, news releases, the organization
website, and other published formats for the MPX teams. I acknowledge that the images will be the sole property of the
Megaplex.
8. Each team is limited to a maximum of 24 players per team. Once your son registers he is guaranteed a position on the
team therefore, all monies paid for registration are NON-REFUNDABLE.
9. Team Mom: Many want to know what type of responsibilities team moms have so we have outlined them for you.
• Relay information from the coaches or Megaplex to parents via e-mail.
• Help organize equipment and uniform hand out with coaches.
• Type official weigh-in roster and announcer roster (provided)
• Organize and distribute handouts for parents i.e. picture day order forms.
• Assist with any post-season party planning.

RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK,
AND INDEMNITY AND PARENTAL CONSENT AGREEMENT
("AGREEMENT")
IN CONSIDERATION of being permitted to participate in any way in a sport or sport related activity but not limited too activity
("Activity") I, for my child for personal representatives, assigns, heirs, and next of kin:
1. ACKNOWLEDGE, agree, and represent that I understand the nature of any sport or sport related activity but not limited
too and that my child is qualified , in good health, and is in proper physical condition to participate in such Activity. I further
agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue my child’s participation in
the Activity.
2. FULLY UNDERSTAND THAT: (a) SPORT OR SPORT RELATED ACTIVITY BUT NOT LIMITIED TOO ACTIVITIES
INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS, AND
DEATH ("RISKS"); (b) these Risks and dangers may be caused by my child’s own actions or inaction’s, the actions or
inaction’s of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE
"RELEASEES" NAMED BELOW; (c) there may be OTHER RISK AND SOCIAL AND ECONOMIC LOSSES either not known to
me or not readily foreseeable at this time; and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL
RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES I incur as a result of my child’s participation or that of the minor in
the Activity.
3. HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE G & G ENTERPRISES, INC., their respective
administrators, directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers,
and, if applicable, owner and lessors of premises on which the Activity takes place, (each considered one of the
"RELEASEES" herein) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED
OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE,
INCLUDING NEGLIGENT RESCUE OPERATIONS AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF
LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my child’s behalf, makes a claim against
any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation
expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.
I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP
SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE
OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE
GREATEST EXTENT ALLOWED BY LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE
INVALID THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
MINOR RELEASE: AND I, THE MINOR’S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF SPORT
OR SPORT RELATED ACTIVITY BUT NOT LIMITIED ACTIVITIES AND THE MINOR’S EXPERIENCE AND CAPABILITIES
AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL CONDITION TO
PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO
INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEE’S FROM ALL LIABILITY CLAIMS, DEMANDS,
LOSSES, OR DAMAGES ON THE MINOR’S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART
BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATION AND
FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR’S BEHALF MAKES A
CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF
THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR COST ANY
MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.
THE PARENT/GUARDIAN SIGNATURE ON THIS FORM ALSO PERMITS G & G ENTERPRISES, INC. TO USE STILL
PHOTOGRAPHY AND/OR VIDEO ORIGINATING FROM OUR SPORTING EVENTS OR CONTRACTED AGENTS FOR
PROMOTIONAL PURPOSES TO INCLUDE, BUT NOT LIMITED TO, TELEVISION AND VARIOUS FORMS OF VISUAL
PRINT MEDIA.
