Laguna Medical Center is a government owned tertiary referral hospital catering 200 beds with
advancing technologies and treatment capabilities in the province of Laguna. It provides health
services in all major specialities including Out-patient Dept., Emergency Room-Admitting Dept.,
Pediatric Dept., Medical Dept.,Surgical Dept., Pay ward Ob-gyne Dept., OR Complex, Recovery
Room, Delivery Room Dept., Laboratory and Radiology Dept., Ophthalmology, and EENT and
Dental Dept. Laguna medical Center also offers specialty areas like ICU, NICU, PICU, SICU and
other services like CT-scan imaging, 2D-echo and mammography.
Medical ward nurse
The Medical Department of Laguna Medical Center serves a variety of acute medical patients
who are experiencing Cardiac, Respiratory, Gastrointestinal and Neurologic disease and variety
of surgical procedures that requires monitoring and carduringtheir procedural phases.
Medical staff comprises of 2 nurses and 1 nursing aid per shift with 35 beds.
Duties and Responsibilities:
1. Knows and observes the Hospital policy, objectives and standards as well as those of the
Nursing Service Division in the light of the Vision-Mission of the Institution.
2. Knows and applies the Nursing Process in carrying out nursing functions.
3. Implements the medical and nursing care plans in medical adult care in coordination
with the other members of the Health Team:
a. Admission routines: vital signs; checks accuracy of records; carries out accurately
initial treatment/IV fluids, medications, etc; orientation and the plan of care.
b. Receives from and gives endorsement to other staff Nurses to ensure continuity
of care.
c. Makes patients’ rounds paying particular attention to the following:
 Assessment of the patients’ condition.
 Obtains and records initial assessment and establishes priorities
according to patient age, symptoms and psychosocial needs
 Condition of contraptions: IV Fluids, catheter, etc.
 Pertinent information for and/or from the patient and/or doctor.
 Interacting with the patients and relatives.
d. Renders direct nursing activities: assessment; taking vital signs; giving
medication; facilitates carrying out of diagnostic procedures; treatment/IV
Fluids; observation, recording and reporting of the progress of patients’
condition; hygiene and comfort measures; instructions/health teachings;
interacts with patients and relatives; etc.
e. Specific for 6AM-2PM shift:
 Attends doctor’s rounds
 Carry out doctor’s orders
 Prepares request for diagnostic procedure
 Refer patients to other doctors
 Updates Kardex
Specific for 2PM-10PM shift:
a. Follows-up results of diagnostic procedures done in the morning.
b. Follows-up of referrals to other doctors.
c. Updates Kardex and completes other legal records missed out by other shifts
Specific for 10PM-6AM shift:
a. Prepares requests for medicines good for 24 hours.
b. Prepares patients for medical procedure, diagnostic test and operation in the
morning.
c. Updates Kardex and completes other legal records missed out by other shifts.
4. Perform special procedures such as:
a. Cardio-pulmonary resuscitation
b. Suctioning per orem and thru ET
c. IV insertion
d. NGT insertion, lavage and gavage
e. Foley Catheter Insertion
f. Administration of emergency, cardiac and regulated drugs
g. Blood transfusion
h. Assist surgeon on thoracostomy procedure
i. Assist doctor on thoracentesis and paracentesis procedure
j. Assist doctor on phlebotomy procedure
5. Attends to doctors’ rounds.
6. Assists doctors in procedures (including life support) and plans with them the continuity
of care.
7. Ensures relaying of Laboratory and Diagnostic Findings to physicians appropriately and
timely.
8. Supervises and evaluates functions delegated to Nursing Attendant.
9. Prepares accurate documentation of treatment, procedures, and progress notes.
10. Carries out discharge procedure/process: settling of hospital bill, clearance, discharge
notice, take home medications and/or instructions and follow up checkups.
11. Attends meetings and other hospital activities.

• Cardiovascular: acute coronary syndrome, acute myocardial infarction, coronary artery
disease, congestive hearth failure
• Respiratory: community acquired pneumonia, bronchial asthma in acute exacerbation,
pulmonary tuberculosis
• Gastrointestinal: alcohol intoxication, chemical ingestion, upper gastrointestinal bleeding,
benign peptic ulcer disease, liver cirrhosis, diabetis mellitus type 2
• Urinary: chronic kidney disease, acute renal failure
• Neurology: cerebrovascular accident, seizure disorder, meningitis
