Check the table above to ensure the course/s you have applied for accept a personal statement. Read through Preparing your personal statement to help you compose your responses to the Assessment Criteria on this form. Once you have completed each section of the Assessment Criteria convert the document to a PDF#. If you are attaching supporting documents to your personal statement, convert them to PDFs and then combine them with this Personal Statement Form PDF to form one single PDF document. The combined PDF document must be smaller than 3 megabytes (MB). There should be no more than 5 A4 pages of supporting documents combined with the Personal Statement form. Select upload and follow the prompts to submit your personal statement to the relevant course or course area. 2. Submit one Personal Statement for each eligible course application. 3. The Single PDF containing Personal Statement form and supporting documents must be smaller than 3MB. Important# If you do not have access to convert the Word document to PDF you may email it to UAC's PDF converter at conv2pdf@uac.edu.au.
