Pre-enrollment Requirements Checklist
Use this checklist to help you complete the tasks required to matriculate into the IU DPT Program.
Items MUST be received on or before the dates listed in the Due Date column below.
PART I – IMMUNIZATION & FIT TEST REQUIREMENTS
A completed immunization form and all supporting immunization documentation should be submitted
together as one packet to the following TWO people:

Each student must retain copies of all immunization documentation for his/her own files to be presented at
individual clinical assignments. We suggest you take photos of all documentation on a portable electronic
device.

Read the Immunization Requirements for Matriculation information. You are
then required to have a medical professional complete, sign, and provide
documentation for the Student Immunization Checklist. If your current
physician does not have all immunizations on file, you are welcome to take the
IU Immunization Record Form along with your immunization records to a
physician’s office and request that office to transfer the immunizations onto
the IU form. These immunizations are required for all students in health
programs on the IUPUI campus.

Current immunizations must include Hepatitis B and a TB test. The TB test
must be valid for the entire school year (Aug ’15 – May ’16); therefore you
should have your TB test completed no sooner than May 1, 2015 and no later
than June 15, 2015. Schedule your test date accordingly.
You may obtain the TB test at no cost from IUPUI Campus Health located in
Coleman Hall, Room 100 at 1140 W. Michigan Street, Indianapolis, IN 46202.

N-95 Fit Test
Each student is required to take an N-95 Fit Test. The N-95 is a respiratory
isolation mask that is only effective at preventing transmission of airborne
illness if the proper size is worn. You must be fitted for this mask. The N-95 Fit
Test takes approximately 20 minutes to complete.
The N-95 mask must be valid for the entire school year (Aug ’15 – May ’16);
therefore you should have your N-95 Fit Test completed no sooner than May 1,
2015 and no later than June 15, 2015. Schedule your test date accordingly.
You may obtain the N-95 Fit Test for a small cost from IUPUI Campus Health
located in Coleman Hall, Room 100 at 1140 W. Michigan Street, Indianapolis,
IN 46202. Campus Health website: http://health.iupui.edu/. An appointment
is required, and may be made by calling (317) 274-8214.

BLS for Healthcare
Professionals CPR
Certification
Students who do not have a current BLS CPR certification card will be
expected to take a BLS for Healthcare professionals CPR course (approximately
$40). Students will submit a front and back copy of current CPR card.

suggested places to earn your CPR certification. You may obtain it anywhere
that is convenient to you.

Certification
All entering DPT students need to have proof of passing a Medical Terminology

on criminal history. Students should become familiar with rules in each state
before entering a DPT program.

Set up a computing
account
Students conduct most of their business with the university through the Web.
By activating their university account, students will be able to register, send
and receive e-mail through the university, and use Oncourse.
If you haven’t activated your university e-mail account, visit Account
Management Service. You may activate your account as soon as you are
admitted to the university. Contact the Support Center with any questions.

Students will need to create an Exchange account and forward all other IU
accounts to their Exchange Account.
You will need your 10-digit University ID number to complete this process. If
you do not know your 10-digit University ID number, you will find it on your
acceptance letter. Or you may contact Ali Cotton, IU DPT Program
Administrative Assistant, at cottona@iu.edu or 317.278.1875.
Make a note of your username and passphrase as you will use it regularly in
the future.
NOTE: Once your IU Exchange email address has been set up, The University,
including the DPT Program, will ONLY email you at this address. Please set it up
soon after you register so that you don’t miss any important information.

click Outlook Web Access.

Open Registration for Fall 2015 begins in early April 2015. The classes you are
taking are reserved solely for students in the DPT program. That is, you are
guaranteed a spot in each course, but you MUST register for them.

Submit a front & back copy of your current medical insurance card and
complete the Student Health Insurance Identification Form. All graduate and
professional students on the IUPUI campus are required to demonstrate proof
of health insurance and maintain coverage during the period of their program
enrollment.

plan offered to students at IU or to learn more about the IU Student Health

Basic HIPAA Privacy
and Security Training

(aka: IUPUI Student
Identification Card)
Submit a printed certificate of the IU HIPAA E-Training program. Online
training modules are now available in OneStart under the E Training
Tab (on the left column). Launch: E Training; Select: Catalog; Select:
University-wide; select: HIPAA Privacy and Security. Continue
to: Students: Select: Students; Select: your Location; Select: your
Program; Select: HIPAA Privacy & Security module; Enroll & Launch.
After completing the training module, go to Transcripts to print
certificate.

Once you register for classes, you will need to obtain your IUPUI Student ID,
better known as your JagTag proximity card. Your JagTag MUST be a proximity
version of the card as this card will let you swipe into the main PT classroom in
Long Hall.

go in person to get your JagTag as they will take your picture for the card. You
