Minnesota CHBs have long been required to engage in a community health improvement process, beginning with a
Community Health Assessment. As part of Minnesota’s Local Public Health Assessment and Planning (LPHAP) process,
every Minnesota CHB must submit its 10 Most Important Community Health Issues (based on the Community Health
Assessment) to MDH.1 The various elements of the LPHAP cycle allow CHBs to meet state requirements and are aligned
with national public health standards from the Public Health Accreditation Board (PHAB).
The list of 10 Most Important Community Health Issues is one of five deliverables that will be submitted to MDH at the
conclusion of the 2010-2014 reporting process. In order to show a comprehensive picture of community health issues,
lists from across the state will be aggregated into regional and statewide reports which will then be shared with CHBs.
CHBs will submit this particular deliverable using the PPMRS system. Please review these instructions, and prepare your
responses prior to reporting into PPMRS.

HOW TO REPORT
Use the table on the following pages to record the most important community health issues your CHB identified
through the Community Health Assessment process; CHBs should record up to 10 issues. It is optional, although
encouraged, for CHBs to provide a brief statement about each community health issue in the text box provided.
It is acceptable to have multiple issues in one area of responsibility and to have none in other areas. For example:

CHBs can identify more than one issue in a topic area. For example:

If an issue crosses two or more topic areas, it should be noted only one time in the “other” category (not in both
topic areas) under the most appropriate area of local public health responsibility. A description of the community
health issue is required. For example “alcohol, tobacco and other drugs” crosses multiple topic areas in the healthy
communities/healthy behaviors area, so it should be place in “other” with the issue noted in the text box:
