JOB DESCRIPTION SECTION IDENTIFICATION Job Title: Care Services Clinical Co-ordinator Responsible to: Clinical Services Manager Hours: 37.5 hours (+ on call) Grade / Salary: Hospice Band 5 Department: Care Services Location: Hospice, Nottingham SECTION 1 – JOB SUMMARY To lead in providing a high quality clinically based palliative nursing services for patients and families with end of life care needs. In conjunction with the Clinical Services Manager, to ensure the patients referred have a level of nurse cover appropriate to their clinical needs. To reorganise and plan urgent and routine workload for the Care services, including referrals to Day Therapy Services. To provide clinical support and advice to the Care Services Co-ordinators and Day Therapy team. To liaise with a variety of Healthcare Professionals on clinical issues that may arise in the provision of Care Services. To deputise for the Clinical Services Manager when required providing leadership. To line manage the Hospice at Home Health Care Assistants and in conjunction with the Clinical Services Manager, assist with provision of clinical support and supervision to other members of the team as required. To work with the Director of Operations - Clinical services, Clinical Services Manager in the continuing development of the Care services. To work on a shift/rota basis, predominantly Monday to Friday, in conjunction with Care Services Coordinators. SECTION 5 – MAIN DUTIES AND RESPONSIBILITIES OF THE POST Communication: To facilitate sensitive and complex communication and information gathering from referrers in a variety of ways, verbal and faxed, and use the information to initiate the appropriate planning of clinically needs based patient care. To manage and safeguard information received from a variety of sources in line with information governance. To communicate sensitive and often complex information on different levels effectively with carers, patients and Healthcare Professionals. Promote effective liaison with District Nurses, Clinical Nurse Specialists, GP’s and other relevant agencies to ensure continuity of care. To log and record complaints or comments around service provision and ensure these are acted upon appropriately. Clinical Co-ordinator Job Description – Feb 2016 Manage To manage the process of receiving and responding to urgent and routine referrals to Hospice care services. Organise the urgent and routine planning of care, by prioritising the workload in accordance with the clinical needs of the patient. Clarify where appropriate clinical information regarding patients care needs from a range of sources including the referrer. Lead the allocation and reallocation of staff based on clinically identified need, skill mix and continuity of care for the patient. In conjunction with the Information and Statistics Co-ordinator, ensure staffing levels are appropriate; provide staffing rota’s, altering the provision of availability of staff as required. Participate as required in the provision of Out of Hours rota for telephone on-call services, to provide urgent and immediate advice to Nurses, patients and relatives. (The service out of hours is 5.30pm10.30pm 7 days a week in accordance with a rota system). Provide support to Hospice at Home nurses where required, this maybe in a variety of formats including emotional support. To provide line management for the Hospice at Home Healthcare Assistants, including identification of their training and developmental needs through Personal Development Plans. To report to the Clinical Services Manager matters effecting the smooth running of the service. Electronically record information in a variety of ways including databases, spread sheets and word documents. To support service improvement. Actively participate in service audit and evaluation. To participate in the appointment and induction of new staff as required. To liaise where necessary with the Clinical Services Manager on clinical matters where clinical decisions are required relating to the Nursing & Midwifery Council Code of professional conduct. To ensure staff are aware of the hospice guidance/policies and procedures and MATRON. Education Practice within the legal and ethical framework as established by the Nursing and Midwifery Council (NMC). Identify and agree own objectives in conjunction with the Clinical Services Manager for personal and professional development in line with the Nottinghamshire Hospice annual appraisal system. Maintain and develop knowledge and skills in accordance with the NMC to improve performance by undertaking training relevant to the post. Identify and lead on induction of New Starters to the Hospice at Home team. Welcome and provide information to students and visitors to give a clear understanding of the service. SECTION 6 – THE PERSON SPECIFICATION ESSENTIAL A minimum of 3 years’ Post Registration Level 1 Registered Nurse experience in a healthcare setting. Evidence of working with patients with palliative care needs. Evidence of Professional Development. Team fit and flexibility to work in a changing and developing service. Computer skills – must be proficient in the use of Word, Excel, Publisher, Outlook and Databases. Experience of Line Managing a team. DESIRABLE Community Nursing experience. Palliative care qualification or equivalent. European Computer Driving Licence qualification. SECTION 7 - WORKING CONDITIONS/ EFFORT Working at a computer for a large part of the role. Dealing with emotional families/patients/carers and staff. Communication at a variety of levels with a variety of services Ability to challenge decisions made by health and social care professionals with regard to the best interest of the patient whilst maintaining professionalism at all times. Exceptionally busy changing workload. Sitting in the same position for periods of time. Remote working from home. Based in a historic building therefore lift not available. INFECTION CONTROL Nottinghamshire Hospice has its own policy for Infection Control. All staff must keep up to date with the new policies and subsequent implementation in practice. All staff must contact their Line Manager if they are suffering from any form of infection with may put patients and other staff at risk. NO-SMOKING POLICY From 1st July 2007 the law prescribes that employers can no longer allow employees to smoke in “smoking rooms” and employers can no longer segregate smokers and non-smokers within the building. The organisation is now enforcing this policy. Breaches of confidentiality will result in disciplinary action, and may result in dismissal. Managers are also required as a condition of this Contract to represent the views of the Hospice in any dealing they may have with employees, their representatives, the media, general public or other organisations in which he/she may come into contact.
