Activities, Necessary Resources and Timeframes to achieve my goal? 1. I will speak with RNs, RPNs and PSWs regarding their roles/ responsibilities at Hillsdale Estates by March 27th, 2015. 2. I will research the scope of practice for each profession using CNO competencies (for RPNs and RNs) and PSNO (for PSWs) by March 27th, 2015. 3. I will shadow each of the 3 staff roles at Hillsdale by March 27th, 2015. 4. I will explain the roles of RNs, RPNs and PSWs with my clinical instructor to clarify my knowledge by March 27th, 2015. Evaluation How will I know that I have achieved my goal? 1. I will know that I have achieved my goal when I am able to clearly describe the roles of the 3 healthcare professionals in a long-term care setting. I will do this through writing a reflective entry on the similarities and differences I have discovered. LEARNING PLAN 2. I will observe PSWs feeding residents in a long-term care setting a variety of diets (pureed, minced) by February 9th, 2015. 2. I will discuss the benefit of different diets and issues that arise with feeding with my clinical instructor by February 23rd, 2015. 3. I will identify aspiration precautions utilizing online modules, textbook resources, and speaking with the dietician by February 23rd, 2015. 4. I will review patient charts and diet plans to ensure I am aware of feeding needs by February 23rd, 2015. 5. I will assist in feeding residents on a variety of diets in the dining area at Hillsdale by February 23rd, 2015 while being observed by my clinical instructor. Evaluation How will I know that I have achieved my goal? 1. I will know that I have achieved my goal when I am able to independently feed dependent patients without PSW guidance, considering safety issues for patients. I will do this in front of my clinical instructor to verify that proper technique and precautions are taken. LEARNING PLAN 5 2. I will write a reflective journal on feeding precautions to prevent aspiration, and clearly identify the diet types of individuals I have fed. I will also outline how I have improved in my knowledge of feeding over the course of placement. 3. I will discuss common issues that arise with feeding and different diet types with my clinical instructor by my deadline. 4. Advocate for a swallow assessment or change in diet if applicable.
