Instructions to be read before completing a shareholder
or pricing statement

Information provided on these forms must be typed or printed electronically.

A shareholder statement must be completed by a sponsor on behalf of an applicant that
is applying for:

(a)
a primary listing of equity shares for the first time; or
(b)
a primary listing of preference shares for the first time; or
(c)
a secondary listing of equity shares for the first time.
A pricing statement must be completed by a sponsor on behalf of an applicant that:
(a)
has a primary listing and is placing equity shares of a class already listed; or
(b)
has a secondary listing and is placing equity shares of a class already listed.
In relation to the pricing statement, the issue/offer price should represent the effective
issue price to the applicant, or where applicable, the effective sale price to the vendor(s).
