Education:
As a child entering Robert E. Lee Public Elementary School Forrest had trouble
socializing with children his own age and making friends. Forrest had trouble academically and
staying focuses while learning. It soon became apparent Forrest had disabilities to his teacher
and the rest of administration after he entered the first grade. They waited to recommended that
Forrest go to a school for the mentally impaired until after completion of the school year. The
mother at first refused the recordation based on the fact she did not want to accept the fact that
her son was mentally impaired. But she changed her mind and enrolled her son in the
neighboring school for mentally impaired.
Forrest went on to complete grade school and some of high school in the school for the
mentally impaired. While there he was given the Wechsler Intelligence Scale for Children
(WISC) test for his IQ. He scored an IQ of 70. He was found to be idiot savant, which would
follow him though out his whole life.
Forrest was recruited to play football for a local Public high school. He was recruited for
his statues size and running capabilities which were perfect components for an all-star football
player. But his lack of coordination and memory skills made it hard for him while playing.
Forrest had a different class schedule than most students. He has six classes in total, three classes
were homeroom classes where he can do whatever he would like and three classes where he had
a one on one aid teaching him. Which the teacher would teach the basics of high school classes.
At the end of his High School year Bear Bryant recruited him to play on the college football
team.
Forrest went to Alabama State University, but failed all of his classes except for Intermediate
Light, an advanced physics class. In his Intermediate Light class, he received a letter grade of A.
Which surprised almost everyone who saw his grades. Unfortunately he did not have the grades
to keep his scholarship for college and was kicked out. After being kicked out Forrest was
drafted into the United States Army.

Occupation:
Forrest first real job was being a member of the 9th Infantry Division in Vietnam.
Forrest’s commanding officer said that “Gump was one of the best Goddamn soldiers this
Army’s ever seen.” Forrest would follow orders without question and do whatever he could do to
help out his fellow soldiers, even going back through the jungle while being shot at to try an save
other soldiers. While in the army Forrest was mostly in charge of carrying equipment though the
jungle. Forrest received the Congressional Medal of Honor for his outstanding service while in
the army. While severing in Vietnam Forrest friend Bubba gave him the idea of opening a
shrimp business before he died. While in the army he played on the US national ping pong
torment against China. Forrest went on to play in a band called The cracked eggs with longtime
friend Jenny Curran. He played the harmonica, the bands popularity grew and he went on tour
for a short time before deciding to quite the band. He was a wrestler for a short time performing
under the name The Dunce. Forrest then opens a shrimp business which became a multi-million
dollar business. He was approached to run for senate which he decided to do. His slogan was “I
got to pee”, unfortunately because of past events Forrest had to drop out of the race.
Institutional Record:
Forrest was arrested when his longtime friend Jenny Curran persuaded him to throw away
his medal of honor. Which was a federal crime, the medal hit one of the men on stage which
resulted in it also being counted as assault. The judge observing the case sentenced Forrest to
undergo psychiatric evaluation in a mental hospital.
Forrest was also arrested for stealing a dress with actress Raquel Welch. But once the
police realize it was the real actress not an impersonator they released everyone.
Mental Institutions:
The doctors at the hospital found Forrest to be a high functioning idiot. They did not see
Forrest as a threat to himself or others. But should receive therapeutic treatment and be
monitored for further analysis concerning his lack of attention span and proper socialization.
Forrest was initialized at Alabama State Psychiatric Center for 30 days, under the condition’s he
comes back in for checkups and follow up tests.

Medical History:
Physical Medical History:
When Mrs. Gump was pregnant with Forrest she did not have proper prenatal care which
resulted in Forrest’s retardation. Forrest was a fairly healthy child , he never had any critical
illness. While fighting in the Vietnam War Forrest was shot in the buttocks. He was then sent to
a special hospital for wounded soldiers in Vietnam, where he recovered for the next two months.
They aren’t any other reports of Forrest being hospitalized.
Mental Medical History:
When Forrest entered grade school he was given the Wechsler Intelligence Scale for
Children (WISC) test. He scored very low with an IQ of 70 placing him in the 3rd percentile.
Since this is a valid form of collecting information it inferred that Forrest is indeed mental
handicapped.

Formal test and one on one interviews resulted in finding Forrest had trouble with:
1. Intellectual skills
2. Behavioral/ Emotional skills
3. Social skills
The formal testing showed presence of mental retardation, thus Forrest was pronounced
medically and legally to have mental retardation. Because of Forrest’s lack of attention skills it
was important to test for ADD.

Forrest picked almost all fours on the questionnaire. Based on the results of the questionnaire, it
can be theorized that Forrest is suffering from ADD. The side effects of his condition based on
this information are the following:
-Poor testing skills
- Difficulties in memory
- Extreme distractibility; wandering attention makes it hard to stay on track
-Poor listening skills
- Changes topic of discussion often
- Difficulty paying attention or focusing, such as when reading or listening to others
For further review and analysis, we gave Forrest an MRI scan of his brain to further are
knowledge on his A.D.D. before a final diagnosis was made.

Though this is not a direct representation of Forrest’s brain, this graph is used to express the
similarities between brain #4 on the graph and Forrest’s brain. . Forrest’s brain is covered with
the color of red indicating he has a serious condition of increased alpha wave activity. Based on
this graph it furthers my opinion that Forrest does indeed have ADD. There is no absolute way to
diagnose Attention Deficit Disorder, but based on the test results and face to face interview of
the patient in question, I believe Forrest Gump may suffer from A.D.D.

One on One Interview with Forrest:
Q: Hello Forrest my name is DR. Purcell how are you today?
A: Good I s’pose.
Q: That’s good. Do you know why we’re having this interview?
A: Cause I’s supposed to answers some questions.
Q: That’s right. Forrest how often do you lose focus on task?
A: I got trouble remembering what I’s supposed to do. Like dis one time while I was getting a
medal for my service in da army and everyone is paying real close attention to the guy speaking,
I knew I was supposed to be paying attention but, I couldn’t stay focus or understand what he’s
trying to say. It was all just real confusing.
Q: Do you often feel confused when having to put your thought into words?
A: Yea. All da time
Q: Forrest would you ever like to go back to college, to further your education?
A: I dunno. Maybe I really liked my Intermediate Light class it was pretty fun I got an A, but I
didn’t like the other ones. I failed all of them.
Q: How was serving in the Vietnam war?
A: Ok I s’pose.
Q: What happened while you were there?
A: I done got shot in the ass. And lost my best friend Bubba
Q: I’m sorry to hear that Forrest. Do you miss your friend Bubba?
A: Yea. But I did open up a little shrimp business that we used da talk about to remind me of
him. I even done hired his daddy to work for me.

A: What are you dumb? It hurt the poor man.

One on one interview with Mrs. Gump
Q: Good morning Mrs. Gump. How are today?
A: I’m fine. But my boy don’t need to be in no institution he’s just a little slow.
Q: I understand Mrs. Gump we are only trying to help Forrest. When did you first realize that
Forrest wasn’t like the other children?
A: Ok . I guess around the time of preschool the other children wouldn’t play with him. They
would chase him all around and tease him.
Q: Did you notice that Forrest had trouble with school?
A: Yes I did. He would try so hard to understand the work he was given but he couldn’t keep
focus or understand it. So I ended up having to send him to a school for the idiots.
Q: Do you feel that Forrest is a danger to himself or others?
A: Hell no. How dare you even ask a question like that. He’s a gentle giant.
Q: I’m sorry for offending you Mrs. Gump I didn’t mean to. Do you feel there is anything else
we should know about Forrest?
A: Whatever you say. Only that he is a simple minded but a hard worker and he don’t need to be
here.

Interview with Forrest friend Jenny Curran
She declined to come stating that Forrest is one of the best and sweetest person she has ever
known. She went on to say that Forrest can do whatever he puts he’s mind to and that he doesn’t
need to be in an institution.
Unfortunately we were unable to contact any one else to be interviewed.

Analysis/Diagnosis:
Based on the tests administered and the interviews taken I can conclude two diagnosis for
Forrest Gump. The first is that Forrest is indeed mentally handicapped but high functioning. I
make this conclusion based on his ranking in the third percentile on the Wechsler IQ Test with
the IQ of 70. The second is that Forrest is suffering from a disorder called ADD. I make this
conclusion based on the high alpha waves in his brain after doing an MRI. Also from the
questioner he filed out and the one on one interview we had.

- Inadequate social skills, mainly conversing with others
- Hard time putting his thoughts into words
- Speech Impediment
- Impulsive tendencies

Recommendations:

Prescribed Medication:
Name: Adderall
Dosage: 200mg, two times daily, 6 hrs. between each dose
Purpose: Used to treat Attention Deficit Disorder A.D.D.
Action: Heightens attentiveness and awareness and lessens restlessness. Raises dopamine
and norepinephrine levels.
Side Effects: Loss of appetite, trouble sleeping, stomach pains, increased heart rate,
dizziness and/or drowsiness, possible alterations in vision
Discharge Plan:
Because Forrest has newly prescribed med’s it is important to keep an eye on his eating
habits. Forrest may lose his appetite but it is important that he gets the daily nutritionally
amounts of proteins, carbohydrates, fruits, and vegetables. Forrest should consume around 3,000
calories a day. Forrest is to be release into the care of Gemma Forrest. This shall only be
possible if, and only if, Forrest abides by all of my recommendations provided and shows he is
making progress with his medication and behavior modification. If he chooses not to do so, he
thereby is subjecting himself as a liability to society and must be taken away to be
institutionalized or hospitalized.
