Students using these services must submit this Offsite Testing Request Form to schedule exams. Location: Date: Time: Signed: Process OFFSITE TESTING REQUEST FORM Step 1: Student completes Section 1 of the Offsite Testing Request Form, and emails form to course instructor. Step 2: Course instructor completes Section 2 of the form, and forwards it to Michael Cunningham in the UMSL Testing Center at offsitetesting@umsl.edu. Step 3: Michael Cunningham forwards the form to the off-campus test site. The off-campus test site confirms the date and time of the student’s test. Step 4: Michael Cunningham emails student with test date and time. Instructor will be copied on this email. Student goes to test site to complete the exam. Step 5: Test Site signs form when student completes test, then scans and emails form to offsitetesting@umsl.edu. If hard copies of the test are required, the test site will gather completed materials, and scan and emails them to the instructor and offsitetesting@umsl.edu. Students must allow five business days’ notice to schedule their exams through UMSL’s Testing Center. The Testing Center recommends that students make appointments for their exams early each term when due dates are given in a course syllabus. Cancellations must be made at least two hours in advance. Missed test appointments may result in additional costs for the student. Students are responsible to ensure parking details and exact testing center location. Online Testing Fees Proctoring fees are included in online course fees. UMSL students are not expected to pay additional fees at approved test sites.
