Instructions for “Job Ready” Application This packet contains the application for “Job Ready” Certification. Please complete the following items: 1. “Job Ready” Application for Certification, signed by your sponsor-teacher and parent/guardian 2. Rubric, completed and signed by you and a sponsor-teacher 3. Personal Statement: Why should the School District of South Milwaukee certify you as “Job Ready”? If you have received the “Job Ready” certification previously, you do not have to complete another personal statement. Please indicate this on your application by checking the appropriate box on the personal statement page. 4. Verification of Community Service, at least two hours within the last twelve months Upon completion, please submit all documentation to the Career Center located in the Guidance Office. Sophomore, junior or senior standing No unexcused absences in the current school year No more than four unexcused tardies in the current school year No office referrals for inappropriate behavior in the current school year Cumulative grade point is at least 2.0 At least two hours of community service within the past twelve months Faculty Statement: I have discussed the attached rubric and personal statement with the student, and I attest that he/she meets or exceeds standards in all areas (at least a 2 in every category). Date: Upon approval, the student will receive an official School District of South Milwaukee “Job Ready” certificate. This endorsement is valid for one year and indicates that the student has the qualities that will make him/her a desirable employee. The District is not responsible for securing employment for “Job Ready” students, but encourages employers to hire students with this endorsement.
