I. INTRODUCTION
The Local Civil Registry Office has the thrust of recording the vital events of every person in the City
of Muntinlupa, which has something to do with the individual’s entrance into or departure from life, as well
as the changes in civil status that may occur during his lifetime. Such recording is continuous, permanent
and compulsory, which, the Office of the Local Civil Registry faithfully performs.
The year 2014, has been an abundant year. Ten thousand eight hundred five (10 805) individuals
were given an identity and has embraced this nation’s citizenship by registering their Birth in this Office.
Whereas, three thousand one hundred forty nine (3 149) of Death were also registered. Numerous
couples made their union legally, through marriage, that reached to two thousand four hundred fifty (2
450) registry. About three hundred twenty nine children were legitimated. On the other hand, one
hundred seven registered children, whose parents were not married, filed an Affidavit to Use the Surname
of the Father (AUSF). While, seventy-nine foreign nationals applied for Legal Capacity to marry. As well as,
thirty-four civil document owners filed a Supplemental Report. However, only three filed for a Pre-nuptial
agreement. Court Decrees, such as, Adoption, Annulment, Correction of Entries, Change of Name,
Declaration of Presumptive Death and Recognition of Foreign Decree totaled to fifty nine for the year 2014.
The LCRO were able to process Three Hundred Thirty Six Petitions for Correction of Clerical Error,
Change of First Name, Day and Month of Birth, and Change of Gender under Republic Act 9048 as amended
by R.A. 10172, collectively. This was made possible, due to the Honorable Mayor Jaime R. Fresnedi’s
appointment of Mrs. Wilhelmina N. Delfin, as the new City Civil Registrar. Subsequently, in June 29, 2014,
she was affirmed by the City Council of Muntinlupa.
CCR Wilhelmina N. Delfin was a graduate of Bachelor of Science in Commerce at Lacson College.
She earned her Masteral Degree in Business Administration at the Laguna Northwestern College. And she
also completed the Advance Management Program for Public Executives, offered by the City Government
of Muntinlupa, as conducted by the University of the Philippines. CCR Delfin has been in government
service, specifically in the LCR for forty three years (43), the time when Civil Registration was still under the
Treasury Office. She rose from the ranks, and holds the position of Assistant Civil Registrar since 1999,
about the same time; the Local Civil Registry Office of Muntinlupa was created.
Innovations and modifications of the LCRO system were instituted, with the desire to deliver
premium services, which our clients truly deserve. And so, to ensure that no Muntinlupenos will be
deprived of his identity and nationality, the then Officer-in-charge, Mr. Ariel Dolleton conceptualized and
instituted the program, “Kapanganakanko, Ipatatala ko”, in which CCR Delfin has continually implemented
on a weekly basis. LCRO focal person to Gender and Development (GAD), Ms. Amelia C. Dolleton, while in
one of its conferences, conceived the idea of educating expectant mothers and the soon to be married
couples, on how to accomplish civil document form correctly. Hence, the continuing Free Legal
Consultation and Orientation of Civil Documents were given on the seminar for expectant mothers and Premarriage counseling, that are being conducted by the City Health Office. This program is headed by the
Registration Division team, headed by Maria Edith Ador.

VII. PROJECTS AND PROGRAMS
1.
FREE MOBILE DELAYED REGISTRATION OF BIRTH
According to UNICEF, there are nearly 7.5 million Filipinos, whose births are unregistered. How
many of those are Muntinlupenos? The figure is undetermined because of the absence of data.
Nevertheless, Free Mobile Registration of Birth has been institutionalized, thus, the birth of “Kapanganakan
mo, Ipatala mo”, which later on renamed as “Kapanganakan ko, Ipatatala Ko,” to make it a more
community owned program.
Several reasons have been identified as the cause of unregistered birth. Such as, inaccessibility of
the Local Civil Registry Office; Second, is the financial incapacity to pay the fees of registration, and fare for
transportation; and third, is the incognizant of the importance of the Birth registration.
To resolve the problem of inaccessibility and the financial difficulty of our clients, the FMR’s
program coordinator Amelia Dolleton community organizer Ms. Myla Salamat identifies the financially
challenged communities to introduce the program. After organizing, the community leaders, the Offices of
Hon. Councilor Patricio Boncayao, Social Services Department and Community Affairs Development Office,
jointly, implemented the program to community level. For this year, the LCRO team were able to deliver
the FMR program to various communities at different time, as calendaredbelow.

FREE ORIENTATION SEMINAR ON CIVIL REGISTRATION
Substantial number of Petition for Correction of Clerical Error under Republic Act 9048, as
amended by R.A. 10172 were filed in LCRO. Reason varies from simple incorrect spelling of entries in civil
documents, such as Birth, Death or Marriage, or incorrect entry of Gender, place, month or date of Birth.
For this reason, Free Orientation Seminar was conceived. As the saying goes “an ounce of prevention is
better than cure.” Educating stakeholders on proper accomplishment of civil documents will prevent
committing errors, thus, could save them from time, money and the hassle of going through tedious
process. For the year 2014, various free orientation seminar was conducted:

INFORMATION DISSEMINATION CAMPAIGN
The Office never ceased to find ways on how to further avert future errors in our client’s
civil documents. CAO Amelia Dolleton, while attending the Gender and Development Strategic
Planning in Pico Sands, Batangas, coined the idea of information dissemination drive to Expectant
Mothers and soon to wed couples. With the permission of the City Health Office Head, Dr.
Magdalena Meana, understanding our goal, readily agreed and apportioned time for our program,
on the Family Planning and Pre-Marriage Counseling Seminar every Wednesday from 8:00 o’clock
in the morning until 12:00 noon. Also, we were permitted to do the orientation on CHO’s seminar
for Expectant Mothers. Maria Edith Ador, the Registration Division Head heads the orientation
team. Apart from proper accomplishment of civil forms and importance of timely registration of
12 | P a g e
civil documents, other services of the LCRO were presented, such as, Admission of Paternity,
Affidavit to Use the Name of the Father (AUSF) and Acknowledgement if parents are unmarried.
Such topics were included because it turned out that single mothers hesitated to register the birth
of their children because of unawareness, fear, and shyness because of the absence of the father.
Thus, this information dissemination campaign helped them a lot.
