HumanaVitality is a FREE wellness and rewards program created to help you get, and stay, on a healthier path - one small step at a time. 2 How does it work? 3 Complete the Health Assessment • The interactive assessment asks questions about your lifestyle habits. • HIPAA compliant and will not affect your premium. • Takes 10-15 minutes to complete on a computer or tablet. • If you know your medical history and key measurements have them ready to help you to complete your Health Assessment. You can still complete the assessment without them. 4 Your Vitality Age™ • Receive your Vitality Age™ and health results based on your Health Assessment responses. • Your Vitality Age tells you if your body is living younger or older than your actual age based on your lifestyle and habits. 5 Get a Vitality Check® A Vitality Check is a Biometric screening that measures: • BMI (800 points) • Blood Pressure (400 points) • Blood Glucose (400 points) • Cholesterol (400 points) Available at Walgreens Clinics , through your PCP, or at a PCSB Biometric Screening location. 6 What's new for year two? September 1, 2015 started the new HumanaVitality® program year! Your Vitality Status™ will revert to Blue until the health assessment is completed over again. Once the online Health Assessment is completed, you will have a full year (August 31, 2016) to maintain your previous Vitality Status or achieve a higher level. All of your Vitality Bucks and 10% of your points will roll over to the new program year. When 10% of your points roll over, you will be awarded bucks for those points. Example: Member ended the program year with 1,000 points and 1,000 bucks. 100 points (10%) will roll over to the new program year and 100 bucks will be awarded for those points. Total number of bucks = 1,100. 7 New Parameters for “Healthy Range Results” Healthy Range Results: If you received “Healthy Range” results for blood pressure, total cholesterol, and blood glucose in the previous HumanaVitality year, these results and points will roll over (one year only) to the new program year. This means you will not only see “Healthy Range” points on your Vitality Statement for the new program year, but you will also be awarded “Prevention” points for having those measures tested. Please Note: “Healthy Range” results for Body Mass Index (BMI) will not roll over. Example: (Program Year One) Member received “Healthy Range” results for blood pressure, Cholesterol, and BMI. In Program Year Two (September 1, 2015) the member will automatically be awarded 800 points for Blood Pressure (400 “Prevention & 400 “Healthy Range”) and 800 for Cholesterol (400 for “Prevention” & 400 “Healthy Range”). Personal Pathway™: After completing the Health Assessment, HumanaVitality will suggest goals that are personalized to your needs. (Humana Vitality > Get Healthy > My Goals) 2. Standard Activities: Potential for ALL members to earn Vitality Points in Education, Fitness, Prevention, and Healthy Living categories. 9 Standard Activities HumanaVitality members can complete over 30 activities to improve their health and earn rewards. 10 Verified Results: Fitness Fitness Devices HumanaGear Fitness Activities 11 Submitting Vitality Forms Did you know you can self- submit forms online and receive Vitality Points faster? 1. Hover over the “Earn Rewards” tab 2. Click “Humana Vitality Forms” 3. Choose the activity you would like to submit, fill out the required fields, and attach proof. Forms can also be faxed to HumanaVitality: 1-877-250-7814 12 Vitality Statement To view your Vitality Points Statement: 1. Hover over the “Earn Rewards” tab 2. Click “My Vitality Statement” 3. Scroll down after statement snapshot, here you can filter by category, then click “Go” Are you not finding your activity in the Vitality Points Statement? The date of service is the date the activity will display on your Vitality Points Statement, not the date it’s processed by/ submitted to Humana Vitality. 13 Challenges Challenges are step based Members earn 50 Vitality Points for joining a Challenge and 50 more Vitality Points for joining a Challenge team. Vitality Points for joining a Challenge and a Challenge team are awarded once a month no matter how many Challenges and Challenge teams a member may join. Note: Member defined challenges (aka community challenges) are only available on the HV App not the member web portal. Only Sponsored challenges (challenges sponsored by the employer or Humana Vitality) would be viewable on the web. 14 “Health is no longer its own reward.” Check Out the Vitality Mall for Great Rewards! Vitality Bucks® can be redeemed for the items members really want. Log on to your Humana Vitality account > Hover over “Earn Rewards” > Mall Vitality Wellness Credit Update for the 2016 Medical Plan Year Humana Vitality is included in the 2016 health plan. It is designed to foster member engagement and provide incentives for attaining key personal health goals. Employees enrolled in a PCS health plan and who have attained Vitality silver status or above by August 31, 2015, will be entitled to a Vitality Wellness Credit beginning with the first payroll in December 2015 to reduce the cost of their health insurance. During negotiations on the total compensation package, it was decided that in order to mitigate the increase in payroll costs and encourage more employee’s engagement (and ultimately a healthier workforce), Humana members can participate in the Vitality program and still earn a Wellness Premium Credit in 2016. Those employees who earn silver status or above during September 1 – June 30, 2016, will receive the Vitality Wellness Credit for 2016 effective the following month after notification is received from Humana (exp.; Employees who are awarded Silver Status during the month of November and reported to PCS December 10th and will be processes with the first payroll deduction in January).
