RESOLUTION CALLING FOR THE REVISION OF INSTITUTIONALIZATION MANUAL
AND THE ADDITION OF DEINSTITUTIONALIZATION PROCESS AND ITS
ACCOMPANYING GUIDELINES
Whereas, the University Student Government has the power that emanates from the
student body and is the sole, unified, autonomous and democratic representative
body of the students (De La Salle University Student Government Constitution,
Article III, Section 1);
Whereas, the Legislative Assembly is the highest policy making body of the Student
Government (University Student Government Constitution, Article XIV, Section
1);
Whereas, the Rules and Policies Committee shall initiate the review of the rules, policies
and procedures of the Legislative Assembly, De La Salle University Student
Government, and De La Salle University, in coordination with the concerned
offices and/or units, and present recommendations to the Legislative Assembly,
De La Salle University Student Government Executive Committee and offices
concerned (LA Rules of Internal Governance, Article VII, Section 3.1);
Whereas, the LA Representatives shall pass resolutions that are of relevance to the
studentry (De La Salle University Student Government Constitution, Article XIV,
Section 5.8)

Whereas, Institutionalization means the process of establishing something, (typically a
practice or activity) as a convention or norm in an organization or culture (Oxford
Dictionary);
Whereas, the current process of institutionalization does not state a criterion on projects
done on a yearly basis;
Whereas, the guidelines will ensure that projects that are considered for
institutionalization will be of excellent quality and that there is a genuine need for
them to be implemented termly/annually;
Whereas, there is a need to create a standardized document in order to provide for an
orderly process when institutionalizing and de-institutionalizing projects;
Whereas, this manual will also ensure the proper documentation and compliance of all
projects that are institutionalized;
Whereas, all institutionalized projects should still be considered relevant to the current
situation of the studentry;
Whereas, no guidelines and/or documents currently exist for the deinstitutionalization of
projects, the lack of which results to a confusion regarding the deinstitutionalizing
process;
Whereas, it is stated in the current Institutionalization Process Manual, ”The project,
once institutionalized, must be done throughout academic year unless the
authored resolution states otherwise.” (Section 3.1);
Whereas, the authors of this resolution believes that this revised Institutionalization
Manual and the added Deinstitutionalization Process will be a better tool to guide
the officers regarding their projects;
Wherefore, be it resolved to be known as the “Guidelines for the Institutionalization and
Deinstitutionalization of Projects”, once approved, be referred to by the USG
whenever they seek to institutionalize or deinstitutionalize projects (See Appendix
A);
Wherefore, be it further resolved that an “Institutionalized Projects Manual” be created in
order to document institutionalized projects and serve as a reference for future
USG officials;
Wherefore, be it further resolved that this manual be shown to all offices concerned and
that officials from those offices be asked to affix their signature to a “Letter of
Agreement” ensuring that they have read the specific roles and responsibilities
they are accountable to;

Wherefore, be it further resolved that the Legislative Assembly will be responsible for

Wherefore, be it finally resolved that the Institutionalized projects manual be approved
together with it’s accompanying guidelines for implementation beginning AY20132014;

ARTICLE I
INSTITUTONALIZATION PROCESS
Section 1: On The Authors
1.1.
All projects needing to be institutionalized must be authored by at least
one (1) Legislative Assembly Representative
1.2.
Author/s must seek approval/endorsement from the offices concerned
and affected by the said project
1.3.
All authors must present to, and get the approval of, the Legislative
Assembly
Section 2: On The Criteria of Institutionalizing Projects
2.1
The project must have been successfully implemented for at least 3
consecutive terms.
2.1.1. For annual projects, they must be done for at least 2 consecutive
years.
2.2
Sufficient documentation proving the effectiveness of the project
2.2.1. Pre-activity requirements
2.2.2. Post-activity documents
2.2.3. Statistics
2.2.4. Survey on the opinions of the students
2.2.5. Endorsement from needed and/or concerned offices
Section 3: On The Institutionalized Project
3.1.
The project, once institutionalized, must be done throughout academic
year unless the authored resolution states otherwise.
3.2.
All fund raisers done for the funding of the institutionalized projects will not
be considered in the cap of FRAs an office has.
3.3.
Abolishment of any resolution can be done by the approval of 2/3rds of
the Legislative Assembly unless the resolution states it otherwise.

MANUAL
About the IP Manual:
The Legislative Assembly, being the highest policy making body,
together with the Executive Branch is tasked to create projects and
institutionalize projects that will create an atmosphere that will help create
a better University.
Inside you will see a guide to what projects have been
institutionalized, what branch is directly in charge of the project and how it
is supposed to be implemented. Any changes in the manual may be done
through any resolution authored by a Legislative Assembly Representative
provided the amendment is in line with the approved latest version of the
project.
The manual’s intention is to ensure that projects are created and
executed properly. This manual may serve as a guide to create a more
efficient University Student Government.

FACILITY AND EXPANSION PROGRAM
Description:
Facility Inspection Program
Office of the Vice President for Internal Affairs
The Facility Inspection and Expansion Program (FIX) is a university-wide
project of
The Office of the Vice President of Internal Affairs (OVPIA) that intends to
ensure world-class facilities and services, create constant reports on
facilities of the university and aid in the inspection of the DLSU campus.
Our university is very privileged to have world-class facilities and services.
The issue

Here though is that there are still so many complaints and concerns that
have been either left unanswered or resolved but in a long amount of time.
There is a need to aid the university with the feedback and reporting of
facilities and equipment that need to be fixed.
To address the issue mentioned, the USG will introduce FIX, the Facility
Inspection and Expansion Program.
Mechanics:
Under the Facility and Inspection Program, a team under the Office of the
Vice President for Internal Affairs will coordinate with college presidents to
visit every university building to take note of the various facilities that need
improvement. After every inspection, a report will be passed to the
Physical Facilities office for further action. The OVPIA team will monitor the
action taken after each report. The entire team will do an inspection every
other week to ensure that there is constant feedback.

ACHIEVER SCHOLARSHIP PROGRAM
Description
The Achiever Scholarship Program is a scholarship granting activity that has
three main objectives;
1. For the Student Council’s Office of the Vice President for Academics to
foster a culture of excellence and service among Lasallians by recognizing
students from the 6 colleges who have excelled holistically.
2. For the entire La Sallian community to acknowledge and recognize these
outstanding students.

3. To challenge students to stand out not only in their academics but also in
their extra – curricular and community service activities by granting
incentives in the form of full and partial scholarships;
Mechanics:
The program Achiever Scholarship Program is implemented as a termly priority
project of the Office of the Vice President for Internal Affairs of the De La Salle
University Student Government under the direct coordination of the Achiever
Scholar Program Central Committee – a committee that is supposedly a
permanent part of the Vice President for Internal Affair’s office. The program will
follow the said criteria;
Paper requirements (50%): This included the applicant’s CGPA, extra-curricular
Involvement, faculty evaluations and awards and recognitions. This was the
largest component because we wanted to put the most weight on individual merit.
Voting (20%): This portion was not only a means of raising funds (as one vote is
Worth Php1.00), but it also determined the students’ “voice” in determining who
should represent their respective colleges as their Achiever Scholar. It was also
a test of the applicants’ ability to tap their network of friends and colleagues in
various organizations. This criterion was decreased compared to the original 30%
because we did not want the contest to be labelled as a popularity contest.
Interview (30%): This was considered the “voice” of the administration and the
Professors of each college. Whereas the original program used a student
panel, we invited distinguished members of the administration to judge our
applicants’ communication skills, leadership and Lasallian values.

Description
The Pahiram Payong is a university-wide project of the Office of the Vice
President of Internal Affairs (OVPIA) that intends to give world-class student
services students need and deserve. The Pahiram Payong is a borrowing
program that lends students umbrellas for students’ usage during rainy seasons.
Rationale:
There areas within the university that does not provide sufficient surface
area to provide shade. And the students are constantly moving around from one
building to another for their classes. During rainy seasons, students get wet while
travelling inside the campus for their classes. Because of this, there is a need of
a program that will make the students’ life more convenient and better safeguard
them during these times.
To address the issue mentioned, the USG will introduce Pahiram Payong.
There will be the various locations within the university that will provide these
umbrellas that can be located at the entrance of these places.
Mechanics:
Guidelines
Section 1: The Vice President for Internal Affairs shall;
1.1 Ensure that the project be implemented by the start of the
school year.
1.2. check and balance the quality and durability of the umbrellas at least once in
every term
1.3. Create an effective system, together with the security office, as to the lending
of umbrellas to the students in different gates.
1.4. Prepare at least 20 umbrellas in the Agno, Gokongwei, Andrew and STRC
gates for the said project
Section 2: The students borrowing the umbrellas shall;
2.1. Be responsible of the safety and quality of the umbrella during his/her time of
borrowing
2.2. Follow standard procedures in logging in and out of different log books to
ensure the affectivity of the project
2.3. Return the umbrella to the gates of the buildings by the end of
the day

2.4. Pay for 110% of the amount of the umbrella for any breakages or lost he/she
may cause

the project

ongoing system of the project

FOCUS FOUNDATION
Description:
Focus Foundation is one of the biggest projects the College of Science will
be having for its 30th Anniversary. With the goal of helping out a deserving
student and reaching the possibility of having sponsoring a scholar in our
very own college, this project aims to set different fund raising activities in
a span of three years. On the fourth year, the dean and the college
president of the College of Science will choose a freshman student that
would be sponsored on his/her entire sophomore year.
Mechanics:
The Focus Foundation is a 6 term long project that will require each batch
unit (not including the Frosh unit) to raise P10, 000.00 and the College
Government P22, 000.00 before the second term of their election ends.
On the 6th year, the set of elected officers will choose the scholar based
on the criteria bellow:

1.0 The Candidate must:
1.1 Have not received any scholarship grants BEFORE/DURING

1.5 Be a member of a recognized organization in DLSU
1.6 Have stayed in the College of Science for at least 3 terms.
1.7 Not be an elected University Student Government officer of the College
of Science.
2.0 On the process
2.1 Anyone who matches may the criteria may apply for the FOCUS
FOUNDATION
Scholar
2.2 There will be a selected board of evaluators composed of the ff:
2.2.1 The Vice Dean of the College of Sciences
2.2.2 The Vice Chairs of the respective departments of the College of
Science
2.2.3 The College President of the College of Science
2.2.4 All University Student Government elected officers of the College of
Science
2.3 After application, the board of evaluators will choose the top 5
applicants
2.4 An interview will be conducted by the board of evaluators on the
discretion of the
Board
2.5 A voting process will then be carried out for duration of 5 university
working days (1
Week)
3.0 Criteria on choosing the scholar
3.1 The decision of the board of evaluators will account for 50% of the
criteria (based on the interview portfolio and other things requirements
submitted to the board)
3.2 Voting criteria by student population will account for 10% of the criteria.
3.3 The CGPA of the applicant will account for 30% of the criteria
3.4 Extra-curricular activities of the candidate will account for 10% of the
criteria
4.0 On the chosen scholar

4.1 He/she must maintain said CGPA of at least 3.000

STUDENT ASSISTANT GRANT FUND
Description:
SGAF is a project of the University Student Government - Office of the Executive
Treasurer, which provides 0% interest loans to DLSU students. The SGAF will be
providing additional loan opportunities such as calamity loans, thesis loans, and
emergency loans. Loan grants range from 10% -100%, depending on the

situation, and all payments must be made on or before the week before finals of
the current term.
Mechanics:
Application procedures
1. Get a set of SGAF Application form at sgaf_otreas1213@yahoogroups.com.
This includes the Student’s Guide, Information Sheet, Promissory Letter & Online
Interview.
2. Accomplish the Information Sheet, Promissory Letter & Online Interview. Paste
a 1x1 picture on the upper right hand of the Information Sheet. Do not leave any
item unaccomplished.
3. Other requirements:
i. Original copy of the lower portion of EAF (temporary EAF may initially be
submitted for students who have not yet been cleared by the registrar; however,
the original EAF should be submitted as soon as it is available).
ii. Letter from parents / guardian stating your reason of application
iii. Proof of address and contact numbers
4. Fasten all documents in short light brown folder and place it inside a short
brown envelope with your name & ID number written in pencil for both the folder
and the envelope. Submit this at the OTREAS table in the USG office at Br.
Connon Hall Room 302 on or before January 8, 2013. For late applicants, please
submit this at the SG Treasurer’s table (SGAF forms Tray or in the Pigeon Hole
of Carlo Inocencio at the Student Government Office.
5. SGAF Applicants will be informed of their status of application (through E-mail
or Yahoo! Group or SMS message) after the interviews have been conducted.
Once accepted
1. The Accounting Office charges a P200 installment fee. This amount is to be
shouldered by the grantee and submitted to the Executive Treasurer as soon as
possible.
2. It is highly recommended that all requirements be submitted early. This is to
allow adequate time to process the papers and to avoid additional surcharges.
The rates imposed by the Accounting Office are as follows:
Payment before start of term – no surcharge
during the 1st week – P200
during the 2nd week – P300
during the 3rd week – P400
All additional amounts shall be shouldered by the SGAF grantee and submitted
to the Executive Treasurer.

3. The SGAF Team will not attend to late submission of complete requirements. If
the student wishes to continue with the loan, he/she must pay for his/her own
tuition (loan from SGAF) at the accounting office and submit the yellow receipt to
“SGAF SUBMIT” drawer.
4. The registrar would hold the applicants EAF for the next term. You would be
cleared as soon as you have paid for your loan.
Payment of the Loan
1. The loan should be paid on or before the scheduled deadline. When paying at
the Accounting Office, get a SGAF Payment Slip from the Office of the Executive
Treasurer’s bulletin board (SG Office) and submit it together with the payment.
This slip would inform the Accounting Office that the payment you have given is
payment for your SGAF loan.
2. Submit the photocopy receipt to the USG Treasurer’s Table. This would serve
as proof of your payment and for documentation purposes.
3. The SGAF team would immediately process your clearance from the registrar.
Please check your Mylasalle account to determine if you have already been
cleared. Once cleared, you may already get your original EAF.
4. If you intend to apply for SGAF again, please pass the original EAF as soon as
possible to the Executive Treasurer.

Locker Rentals
Description:
Locker Rentals is a project under the Executive Branch that aims to aid
students in getting/reserving lockers for their everyday needs. With
different lockers placed in different buildings, the project aims to create a
systematic way for the students to avail the lockers they want for an
affordable price.

Locker Rentals will be done every term by any office of the executive
branch. The reservation of lockers will be done through the USG Website,
making the P10.00 profit from the rentals be allotted for the maintenance of
such. The people who reserve the lockers through the website will go to
the USG office to claim their stub and pay for the amount needed to be
paid. The rental of the locker will be good only for a duration of an entire
term.

PAHIRAM LIBRO
Description: Pahiram Libro is a university-wide project that is
spearheaded by the Office of the Vice President for Internal Affairs. A
collection of books gathered from both the lost and found and through
donations are made available for students to borrow. This provides the
students with an easier access to books that could spare them from
spending. This is an alternative to the University Library and provides more
convenience as they could borrow the books for an entire term.
Mechanics:
Section 1: The Vice President for Internal Affairs shall;
1.1. Ensure that the project be implemented by the start of the
school year.
1.2. Check that the collection of books are in good condition
1.3. Create an effective system that will be accessible to the
students
1.4. Publicize details of the project to the student body
Section 2: The students that will borrow from Pahiram Libro;
2.1. Access the Pahiram Libro link on MLS to borrow the book
2.2. Be responsible of the safety and quality of the book he/she
borrowed
2.3. Return the book on or before the first and up until the last day
of finals
