Complete Part I below with the information requested. Submission of this form will indicate you have read and understand the
Transfer Credit Limitations under Part I.
Complete the first four columns in Part II under the heading Transfer Institution Course Information.
Obtain course descriptions for the courses you wish to have evaluated for possible RHIT transfer credit.
Submit this form AND course descriptions to the appropriate RHIT Department Chair* for evaluation (*see next page).
Department Chairs will return this form to the Registrar’s Office after their evaluation process is complete.
Upon completion of your course(s), request an official transcript with final grades from the institution attended.
Have your official college transcript mailed to the RHIT Registrar’s Office (address below) for processing of the approved
transfer credit. Your official transcript should be mailed to:

In order to have transfer credit awarded, we (the Registrar’s Office) must have your official college transcript
with final grades AND this form completed by both you and the appropriate RHIT Department Chair.
Instructions to Department Chair: Please complete and return this form to the Registrar’s Office as
soon as possible. The approved transfer credit may affect this student’s upcoming fall registration.
We will maintain this form on file in our office for processing upon receipt of student’s transcript. If no credit is to be awarded, please
note form accordingly and still return it to the Registrar’s Office.

Transfer Credit Limitations:
1. Transfer credit is accepted only from accredited colleges and universities.
2. The grade earned for a course taken at another institution must be equivalent to a grade of “C” or better.
3. Transfer credit will not be granted for a course taken on a Pass/Fail basis.
4. Normally, courses taken by correspondence, online or as a video course are not accepted. In cases where it is accepted, full documentation of the course will
be required and a personal interview with the Department Chair may be required.
5. Grades earned in Transfer Credit courses are not recorded on the student’s Rose-Hulman permanent record and are not averaged into the cumulative GPA.

Transfer of credit into Rose-Hulman must have the approval of the head of the department in which that course normally
would be offered or which the course being considered for transfer is most nearly related. Please forward the Transfer
