Health Triangle Project
Follow the directions to figure out your total health by making a health triangle.
Answer the questions below with an (A) Always, (S) Sometimes, or (N) Never.

CONSTRUCT YOUR TRIANGLE:
1. For each “always” answer give yourself 1 inch on a ruler. For each
“sometimes” give yourself ½ inch. Example: On physical health I had 4 “always”
and one “sometimes”. I would give myself 4.5 inches for the physical side of my
triangle. Total your numbers for each side.
2. Using a PENCIL FIRST, use a ruler to build your triangle. Take your time and
make sure your triangle closes.
3. Label each side of your triangle using a different color for each side.
ANSWER THE FOLLOWING QUESTIONS ON THE SAME PAPER AS YOUR TRIANGLEPLEASE USE COMPLETE SENTENCES
1. Explain your triangle, what does it look like? Is it equal?
2. What is the area(s) you are strongest in and why?
3. What area(s) need work? What can you do to improve in this area(s).
FINISH YOUR TRIANGLE:
1. Add pictures that represent all 3 sides of your health to the inside of the triangle.
2. Add a title
3. Put your name in the upper right hand corner.
4. Remember neatness and creativity count…check your rubric.
