JOB DESCRIPTION POSITION TITLE: Case Manager - Disability PROGRAM: WorkBC Sources, Cloverdale REPORTS TO: ESC Manager ALL POSITIONS DESIGNATED BY SOURCES COMMUNITY RESOURCE CENTRES REQUIRE A CRIMINAL RECORDS REVIEW PRIOR TO AN APPLICANT BEING EMPLOYED IN ANY CAPACITY. CERTAIN POLICE RECORDS MAY PRECLUDE APPOINTMENT TO CERTAIN POSITIONS. Position Summary Reporting to the Program Manager, the Case Manager is responsible for providing specialized employment services for persons with disabilities; managing a full case load including assessment, needs determination, employment counselling, development of return to work action plans, provision of required services, referral to community based services, monitoring and follow-up. The Case Manager will maintain both a manual and computerized case-management system (ICM) for recordkeeping purposes as required and determine eligibility for financial assistance and administer financial supports as required for specialized services including disability related supports, selfemployment, apprenticeship, wage subsidy and skills development interventions; determine level of assistance, administer supports and complete documentation that complies with Service Canada and the Employment Program of British Columbia (EPBC) policy and guidelines. The Case Manager will assist with employment based workshop facilitation; employment preparation, job search, career planning. The Case Manager will work as part of a staff team and adhere to policy and procedures as required by Sources and COA (Council on Accreditation) standards, market to promote services within the community, and liaise with community service providers. Accountable To The Case Manager is directly accountable to the Manager of the Employment Services Centre. Qualifications 1. An undergraduate degree in related field or considerable experience combined with education (Career Development Practitioner Certificate, Social Services Diploma) in the community, social services, employment/career development field. RRP and CCDP designation preferred. 2. Minimum 3 years direct experience in employment counselling, case management and/or job development specific to persons with disabilities. 3. Experience working with diverse client populations; individuals with multiple barriers to employment and persons with disabilities; developmental and mental health disabilities, learning disorders and cognitive disabilities. 5. Individual and group based facilitation and instructional experience pertaining to personal management, job search preparation and career planning. 6. Familiarity with skills development and wage subsidy interventions; determine eligibility, needs assessment, negotiating financial assistance, complete requirements as per MSD- ELMS and Service Canada policy, maintain ongoing communications and monitoring of progress with client through to completion of intervention until successfully employed. 7. Knowledge and experience regarding provision of specialized assessments; referral to assessment services for psychological vocational assessments, learning disability assessment, functional capacity assessments, etc. 8. Working knowledge of community based resources with experience as a third party liaison; funders and community service providers. 9. Self-motivated with strong problem solving skills and the ability to work independently, take initiative and work under pressure with challenging client populations in a results oriented work environment. 10. Excellent verbal and written communication skills. 11. Strong administrative ability and computer proficiency with emphasis on MS Office (Word, Excel, Outlook), Internet usage and customized databases. 12. Ability to prioritize and manage a demanding caseload. 13. Current First Aid training equivalent to WCB requirements (willing to obtain if not currently valid). Key Job Responsibilities Includes but is not limited to the following: Client: 1. Conduct information sessions and individual orientation to disseminate information on services, resources, eligibility, guidelines and policies. 2. Provide initial intake to determine service eligibility for the range of EPBC services including case management services. 3. Complete a comprehensive formal needs assessment to assess job readiness and level of employment needs as related to job search, career decision making, work experience, skills enhancement and work maintenance and determine appropriate assessments and support services to address individual needs. Facilitate one on one and group employment preparation workshops including job search techniques (resumes, cover letters, hidden job market, interview skills, negotiations with employers, et al), career decision making/ planning (identification of suitable employment/career objective, occupational & educational research, etc.), and employment maintenance and essential skills (self-esteem, workplace behavior and communication, time management, stress and anger management, working with others, etc.) 5. In collaboration with the client, provide comprehensive employment counselling, coaching and support for job search, personal management, career decision making, employment maintenance and address barriers to employment, explore and identify suitable employment options, access and refer to appropriate support and specialized services and develop realistic action plans that define goals and strategies that support clients’ transition to employment. 6. Work in conjunction with the Employer Liaison / Job Developer and clients’ networks to further determine, coordinate and secure suitable work experience, community and job placements. 7. Assess and identify eligibility and suitability for program financial supports for the following service interventions deemed appropriate; disability related supports, skills development, wage subsidy and self-employment. 8. Provide ongoing support and monitor progress of client action plan until service objectives have been met; employment or community attachment. 9. Maintain accurate and current documentation of all client communication and activities utilizing ICM (Integrated Case Management). 10. Monitor all clients on a regular basis throughout the case management process to ensure followthrough and appropriateness and effectiveness of service delivery. 11. Provide information and referrals to community supports to address basic needs promoting employment readiness, including financial, housing, legal, food, addiction, etc. 12. Refer for specialized assessment (ie. psychovocational, credential evaluation, language) and /or services as required (immigrant, disability, youth and women at risk, substance abuse, etc.) 13. Provide regular follow up and support to clients up to 24 weeks following employment attachment to ensure employment maintenance and success. 14. Liaise with funders, agency partners, service providers, community agencies and employers as required during the course of service provision. 15. Maintain client, service and statistical data related to the case management process as required by the funder and Sources, and forward monthly submissions to the Manager using an established reporting format. Community: 1. Promote Employment Service Centre (ESC) within local community with emphasis on developing and maintaining strong community connections to increase awareness of and access to employment services and foster business relationships for employment opportunities for local residents. Maintain regular contact with service providers, training facilities and employers to monitor client progress. Program: 1. Take an active role in the work group by participating in staff meetings, team days, and other activities that foster positive working conditions and enhancement to service to clients. Participate in design and implementation and revision of program materials and processes as it pertains to ESC service delivery. 2. Provide temporary replacement/relief coverage as required for all ESC staff including facilitation, case management, reception, and self-serve / resource centre. 3. Liaise with various levels of government and external programs/agencies. Agency: 1. Ensures that Sources Policies and Procedures are followed as well as the operating procedures of the Employment Services Centre. 2. Adherence to the Canadian Standards and Guidelines for Career Practitioners. 3. Participate in quality improvement and evaluation processes implemented by the Executive Director and/or ESC Manager. 4. Ensure that the rights, dignity and confidentiality of clients is maintained at all times. 5. Ensure the Health and Safety of all clients throughout the course of working relations.
