Commemorative Certificates and Certificate Boxes
If you wish to order a commemorative certificate or certificate box, please complete the order
form at the end of the choices sheet.
Payment can be made by cash, cheque or card.
If you are paying by cash or cheque please send payment with a print out of your choices sheet
and order form to the office arranging your ceremony.
Payments by card can be made to the office arranging your ceremony on the numbers above.
