The comprehensive student plan is provided through Viva Health, a member of the renowned UAB Health System. If you choose student health insurance, your primary care physician will be part of the Student Health Service, or you will be referred to another UAB doctor. Eligibility Health insurance is required for all students enrolled in the schools of Dentistry, Health Related Professions, Medicine, Nursing, and Optometry; all international students or scholars. These students will be automatically enrolled unless they sign a waiver. Spouses and children may be covered for an additional premium. Spouses will need to select a UAB doctor as a primary care physician, and children may choose a participating pediatric primary care provider. How to Participate Call Viva Health Member Services at 205-558-7474 or 1-800-294-7780 and request a student insurance packet. Be sure to ask any questions you might have. You must purchase the undergraduate plan each semester. To participate, you must enroll and pay for the insurance by the third day after the last day to add classes each semester. Dental You are automatically enrolled in the School of Dentistry’s Health Professions Student Dental Program, and a student dental fee is charged to your UAB account. Clinic hours are 5:00 pm – 8:00 pm, but someone is on call at other hours. For more information, call 205934-5234. If you already have dental insurance, you can request a waiver of that fee. To do so, make a copy of your insurance card and fax it to Dorothy Andrews (fax 205-934-3057). Include your full name, your student ID number (B00#), and the term for which you want to waive the fee (i.e. Fall 2010, Spring 2010). For questions about the waiver, contact Dorothy Andrews (ddandrew@uab.edu or 205-934-5234). 5|Page On-Campus Health Services The Student Health Service Fee on your tuition bill gives you unlimited visits to Student Health Services for preventive and acute primary care, injury care, minor office surgery, women’s health, mental health, and nursing care visits with NO co-payments. Other services are provided at an out-of-pocket cost. Student Health Services are located at 930 20th Street South, Suite 221 (corner of 10th Avenue South and 20th Street). Many restaurants close to the apartment. Cons: Noise because the apartment is directly across from the emergency department (not bad enough so you can’t sleep). Other: One bedroom units are large enough for more than one person. Cons: Slow elevator. Some 5th floor units have leaky roofs during heavy rains. Grocery store is far away. Other: Walking distance to many restaurants and bars downtown. Fultondale/Gardendale: Chapel Creek Apartments 51 Chapel Creek Lane Fultondale (205) 849-0225 Rent: 1BR/1BA = $690 2BR/2BA = $830 3BR/2BA = $940 Commute: 12 minute drive (9 miles) Pros: Quiet and safe family neighborhood. The staff is very friendly and welcoming. Less than one mile from many restaurants, Target, Bestbuy, and Books-a-Million. You can get everything you need at your own exit. Wal-Mart is only one exit away in Gardendale. Cons: none Other: Chapel Creek is a great place to live in order to get a short distance from school where you can relax and get all the things you need nearby without the hassle of downtown shopping. If you're not convinced, just stop by for a visit! Highlands: Carlisle Terrace (managed by Highlands LLC) 1223 29th Street South (205) 581-0077 Rent: Studio: $475 1BR and 2 BR also available Commute: 7-10 minute drive (1.5 miles) Pros: Cheap rent and a great location in Highlands. Short commute. Cons: Parking is difficult, and you often have to park a couple blocks away from your room. Building is old. Poor maintenance staff. Management takes a long time to respond to concerns. Other: Gas/electric (~$75) and cable/internet (~$115) are not included in rent. Pawnee Square Apartments 2912 Pawnee Avenue Rent: 2BR/1BA: $750 1BR also available Commute: 10 minute drive 11 | P a g e Pros: Short commute to campus. Close proximity to restaurants/bars, coffee shops, and libraries. Walking distance to all three parks in the Highlands area. Cons: Apartment has a mix of college students, graduate students, and non-students, and can be loud sometimes. Parking is also a problem because it is not monitored. Entrance to the apartment is not good for people with low cars because you can scrape the bottom. No washer/dryer in unit. Other: Laundry facility on site. Usually 15 min. during morning rush, and can be 20+ during peak of afternoon rush. However, our schedule usually allows me to get home before the rush. Pros: Cheap. 2 bathrooms. Spacious living room. Reasonable commute. Quiet and safe. Central to Southside, Brookwood mall, Vestavia, Mountain Brook, Hoover, and downtown Homewood. Pool and small fitness center. Connects with the Lakeshore Trail for running and biking. Wal-Mart, Publix, and Chick-fil-a are all less than 5 minutes away. Cons: On the older side, so carpet and kitchen fixtures aren’t new and shiny. Edgewood (Garage Apartment) Rent: 1BR/1BA: $600 Commute: 11 minute drive Pros: Cheaper than 1BR units downtown. Unit has a full living room and kitchen with marble countertops, hardwood floors, and new appliances. Since it is a garage apartment, you don’t pay for water (and can sometimes mooch internet/satellite off your landlord). Light traffic from Edgewood to campus. Cons: none Other: Power/gas cost an average of $75. Edgewood is a quiet place to live. You are surrounded by families and the area is really safe. If you enjoy running and being outside, this is a good place for you to live. UAB Campus Card The UAB campus card is your official university ID card and is used to gain admission to the campus rec center, athletic events, participate in intramural sports, purchase discounted tickets to community events, and check out material from UAB libraries. Your campus card is also used to print items from the school computers and can be used as a declining balance card for purchases at on and off-campus dining and retail locations. Once you get to campus, you can obtain your UAB campus card by going to the Campus Card Office found room 158 of the Hill University Center. Permits will not be available for purchase until after midJuly. First year medical students have the option of parking in decks on either 10th Avenue or 9th Avenue. The 9th Avenue deck has a waiting list and is more expensive, but it is closer to Volker.
