INDIVIDUALIZED LEARNING PLAN (ILP) Each fellow must have an individualized learning plan and review their progress at least twice a year with his/her mentor. The ILP should have a minimum of 2 personal objectives (short or long term) directed at becoming a life long learner and practice improvement. Strategies to address these areas should be included. The objectives need to be concrete and measurable (not “read more”, but “read West’s physiology and pathophysiology by a certain date” for example). It is important for your ILP to be based on reflection of your strengths and weaknesses. Things to help you with this reflection include your twice yearly evaluations by the faculty, nursing and social work evaluations which you have received, in-service examination scores, self evaluation, etc. The objectives should incorporate this reflection into practice improvement. ILPs should be updated at least annually with the mentor, adding new objectives as appropriate. In addition, the ILP should be updated at the end of fellowship with a focus on transition to the next phase of the fellow’s career and a plan for life long learning. An ILP is important because: 1. The practice of pediatric critical care medicine is constantly changing and evolving 2. Pediatric critical care specialists must engage in life long learning in order to keep up with changes and developments 3. The American Board of Pediatrics requires proof of life long learning to maintain certification An example of an ILP for an office based pediatrician is below. Objective One In addition to addressing patient specific questions in real time practice, take the time to address the broader context of these questions. Strategies: 1. Work with my partners to keep a running tab of questions that require us to search for an answer during a patient visit and list them on the bulletin board in the staff lounge 2. On a weekly basis rotate the responsibility of putting together a 15 minute discussion on one of the listed topics 3. Block 20 minutes at the end of the weekly business practice meeting for presentation and discussion of the topic Objective 2 Engage in at least one community advocacy activity on a semiannual basis Strategies: 1. Call the local public school and arrange a meeting with the principal 2. Discuss needs for either talks with students and /or parents about hot topics such as substance abuse 3. Make a commitment to address these needs by giving one such talk every six months to various groups of students, teachers, and/or parents 4. Develop written evaluation forms that can be used by the various audiences to get feedback about the content and style of my presentation with suggestions for improvement Objective 3 Prepare a talk for the next medical society meeting on community resources for patients and families Strategies: 1. Meet with the social worker at the community hospital to learn about resources that she uses for patients and families 2. Call the five or six most used agencies and ask them to send information and any available brochures 3. Prepare a PowerPoint talk to highlight each of these agencies by identifying access, eligibility and referral information for each PEDIATRIC CRITICAL CARE MEDICINE FELLOWSHIP INDIVIDUALIZED LEARNING PLAN Fellow __________________________________ Please write two to three objectives you will work to achieve during each academic year. For each objective, list at least 2 strategies you will use to achieve the particular objective. Your objectives and progress should be reviewed twice yearly at meetings with your mentor. Add new objectives as necessary. Please turn a copy of your ILP in at the end of your fellowship. Objective: Strategies: 1. 2. 3. 4.
