GREEK ODYSSEY REGISTRATION FORM: ART/CLA/HOA/HST/HUM/ 300.2: The Greek Odyssey Ten-day field seminar to Greece, the Aegean & Rome Program Dates: August 28- September 6, 2011 By submitting this form, I acknowledge that I have read through and agree to the following terms: The cost of the seminar will be approximately $2,150, billed by Syracuse University as a Program Fee. The final program fee depends upon total number of participants and will be communicated to all participating students once program registration is complete. Registration deadline is Friday, April 15th. A deposit of $300 must be submitted in order to register (see page 2 for instructions). The deposit will be applied directly to the Program Fee. The remainder will be billed by Syracuse University to the billing contact on file with the University. I understand that the deposit is non-refundable if I withdraw from the seminar after May 1, 2011. The Seminar carries 2 credits, and will be included in my fall 2011 schedule. I further understand that, if I register for more than 19 credits total for the semester, I will be charged additional tuition fees for each credit above 19. I acknowledge that I must participate in the Signature Seminar group flight, organized by Advantage Travel. Information on the seminar flight will be sent via email. Group size is limited to 30. o SU Abroad will maintain a wait list if more than 30 students apply o If there is insufficient interest in the Italy Signature Seminar, I understand that SU reserves the right to cancel the seminar and refund/credit my deposit. Your name and SUID number must be written on the memo line of the check. Checks need to be mailed directly to the SU Abroad Office at the address below: Syracuse University Abroad Greek Odyssey 106 Walnut Place Syracuse, NY 13244 Option 2: Paying online with a credit card or by EFT (Electronic Funds Transfer) Payment information can be found by visiting www.bursar.syr.edu, click “Parents” then “Payment Options.” Both methods of electronic payment will provide you with a receipt/confirmation number at the conclusion of the transaction. You must email this information to Amy Sturgeon at SU Abroad (asteve03@syr.edu) so your deposit can be recorded and you can be cleared for participation on the Seminar. NOTE: You will need to log into MySlice to pay by credit card so please have your NetID and password ready when you go to make your payment. You will also need your SUID. The first person to use NetPay in association with a particular student account must use the STUDENT sign-up option.) A list of payment types will appear.
