Ref: ________________ Date: _______________ To, Mr. /Ms. _________________ M/S _____________________ _________________________ Dear Sir/Madam, Sub: AMC for Pest Control Services at Sofitel Mumbai BKC for the period starting from to - This has reference to your proposal dated ___________ and the discussion you had with undersigned, we are pleased to enter into Comprehensive/ non-comprehensive Annual Maintenance Contract for Pest Control, on the following terms and conditions: This contract made at Mumbai on _____________ (date), between Sofitel Mumbai BKC, owned by M/s Shree Naman Hotels Pvt Ltd, a company registered under the Company’s Act, 1956 and represented by its Constituted Attorney, having its Registered Office at C 31, Naman Centre, Bandra Kurla Complex, Bandra (East), Mumbai - 400051, India; hereinafter referred to as the COMPANY on ONE PART AND M/S _________ (contractor) represented by Mr. ____________ referred to as the CONTRACTOR, on the OTHER PART, WHEREAS the company is, interalia, the owner of Sofitel Mumbai BKC, hotel of the international standards, situated at ----------------- (address), hereinafter referred to as the said hotel. AND WHEREAS, on representation from M/S _________________ (name of the contractor) who as a contractor undertakes to render best quality Pest Control Services at the premises of ---------------------- (name of the hotel unit) at the above mentioned address. The following requirements are to be carefully noted: a) The services offered by you shall leave no scope for complaints regarding the pest control measures, on any day, at any time. b) One person will be available round the clock i.e. there would be at least one person available in each shift who would be relieved by one person at the end of the shift. c) Your staff will report to the Housekeeper or any other person so assigned by the General Manager of the hotel, and will operate under her/his instructions. Daily logbook will be put up to the Housekeeper for her/his information and signature. You will have a weekly meeting with the Housekeeper. d) Your services shall include all residential rooms, all restaurants, all kitchens, all public areas, banquet rooms, the car park and garages, the health club, all the offices and outlets at the all the levels, the surrounding areas, time office, receiving and the garbage areas as well as the rest of the west basement. e) Each time Sofitel Mumbai BKC is serviced by your company (inside or outside the facility) you must provide the following: The amount of chemical used The specific area treated Sofitel Mumbai BKC is an entity owned by Shree Naman Hotels Pvt Ltd. The batch code of each chemical The method of application The pest the treatment is to control f) You must provide Sofitel Mumbai BKC the frequency of your visits g) You must provide Sofitel Mumbai BKC the pests you intend to control h) You must Provide Sofitel Mumbai BKC with an updated rodent and cockroach bait station location list: if you change locations Sofitel Mumbai BKC must be updated with the new locations within 6 hours of dong so. i) Ensure all of your insurances, professional qualifications and updated licenses are kept with us on file upon the signing on this agreement. j) All pest control employees on site must be provided (by you) a visible name badge identifying the employees complete with name, photograph, company name and employer telephone number. k) All on site employees are to be provided (by you) with a mobile phone whose updated number is on file with the head of housekeeping, the Executive chef, the director of rooms, the Chief steward and food and beverage manager. Following would be the various pest control measures for our entire Hotel property broadly covering all the areas of the property is as under. Dis-infestation: This treatment will comprise of a thorough insecticide spray in common portions of the buildings including gutters, shafts and manholes. For the restaurants, bars, kitchens, stores, banquet rooms, guest rooms, offices, receiving areas, you would primarily resort to bait and spraying application treatment, which will be carried out in all vulnerable areas, nooks, and necessary you will resort to insecticide spray application interspersed with the bait treatment. This treatment will be carried out monthly in the kitchens, stores, restaurants, bars, banquet halls, all public areas and common portions. For guestrooms and offices, you will resort to quarterly treatments or earlier if necessary. Dis-infestation will be carried out also at the kitchen, in various offices and in stores etc. This treatment will be carried out on a monthly basis. Rodent control: Your rodent treatment includes a relatively safe and effective single dose anti-coagulant rodenticide. You will carry out a regular baiting and keep a constant vigil on rodent population and migrants, if any. You will also resort to the use of traps wherever necessary. Fly control: Fly control measures in the Receiving area, garbage area, car park area, car wash area & other areas wherever necessary, will be carried on a daily basis and will be intensified during the rainy season. Sofitel Mumbai BKC is an entity owned by Shree Naman Hotels Pvt Ltd. Mosquito control: You will carry out fogging operations for adulticiding on a fortnightly basis. In addition, every fortnightly you will carry out fogging operations for the exterior area including the porch area and Gardens. The termite control & treatment against wood borers: The above-mentioned treatment is not included in this contract. In the event of occurrence of termites / woodborers, a separate proposal for carrying out the treatment would be provided by you. We expect you to be vigilant to this infestation and let us know as soon as the need arises for the same. Scope of Work: Pest Schedule Tentative service frequency 1. Cockroaches F/B areas and Guest Rooms - Monthly 2. Rodent Control Fortnightly 3. Mosquito Control Weekly 4. Fly Control Weekly + Seasonal Additions 5. Millipedes/Centipedes As and when required basis 6. Lizard Control Monthly 7. Red Ants/Black Ants Monthly 8. Psocids/Booklice As and when required basis. 9. Snail and slugs control As and when required basis. 10. Spider Control Once in six months 11. Silverfish/Bedbugs Control As and when required basis. 12. Nocturnal Insect Control As and when required basis. (Occasional Invaders) 13. Carpet pest (beetles) As and when required basis. The above mentioned service frequency is tentative and will be altered if need arise. Pests not mentioned in the scope of work Sofitel Mumbai BKC is an entity owned by Shree Naman Hotels Pvt Ltd. would be out of purview of the contract. E.g. Subterranean termite The above mentioned service frequency is tentative and will be altered if need arise. Pests not mentioned in the scope of work would be out of purview of the contract. E.g. Subterranean termite Areas under Contract: 1. Guest Rooms Floors:- All Guestrooms/floor, corridors, lift lobby, house cleaner room, linen room, AHU per floor. 2. F/B Areas:- All operational and non operational F&B areas 1. Cafeteria + Kitchen 2. Restaurants + Kitchen 3. Bar 4. Butchery + Bakery + Garde manger + Commissary 5. Banquets + Kitchen 6. Lounge 7. Freezers, Cold Storages, walk in coolers 8. Raw Material Stores 9. Raw Material receiving 10. Banquet Material stores etc 3. Non F/B Areas:- 1. Lobby 2. Car Parking 3. Lockers 4. Surrounding Focus of Work: Major focus of pest control measures will be concentrated on not only F&B service and production areas but also on all the areas of the hotel. The guest rooms/Floors will be serviced monthly or as per the instructions of the Housekeeper. Manpower: - 24 Hours, 365 days basis service will be provided to the Hotel property. Free call back: You undertake to revisit and retreat any area of the Hotel, where we find a problem with the services performed or feel the need for additional treatment. This service will be provided within 24 hours of our making the request and there will be no additional charge for the same. Sofitel Mumbai BKC is an entity owned by Shree Naman Hotels Pvt Ltd. Charges: Total cost towards carrying out the above-mentioned treatment will be Rs. xxxxxx (Rs. Xxxxx/-) per month + service Tax extra as applicable, payable within 30 days after completion of the month. Subject to deduction of Income Tax at Source at prevailing rate. Period of the contract: From xxxxx to xxxx Workmen compensation, ESIC, PF, Insurance and other Legal requirement: It will be your responsibility to undertake all the statutory requirements of the workmen and staff employed by you. In the event of non-compliance by you and in the event of damages or compensation payments made by us, the same will be adjusted against your bills or you shall undertake to reimburse the payments. Safety: All the work force shall follow safety guidelines and protect the property and personnel. Basic safety tools like helmets, safety goggles, safety shoes, work overalls, hand gloves, first aid kits, etc. shall be made available by you and shall be used by the personnel at site. Termination: Please note, in case of dissatisfactory services we are at liberty to terminate your contract without any notice and without assigning any reasons whatsoever.
