MADISON . . . The Department of Regulation and Licensing announced that the following
disciplinary orders were issued in August 2003.
American Homepatient, Inc., Madison – Prescription Drug Distributor. The Pharmacy
Examining Board reprimanded American Homepatient Inc. and ordered payment of a forfeiture.
An inspection revealed that the facility had inoperable or missing audible alarms for its five entry

Westen Anderson, Minneapolis, MN – Funeral Director. The Funeral Directors Examining
Board accepted the surrender of Westen Anderson’s license. Anderson did not complete
required education prior to renewing his license. [LS0308051FDR]
Dale and Donna Baker, Brookfield – Barbering and Cosmetology Establishment Owners.
The Barbering and Cosmetology Examining Board reprimanded Dale and Donna Baker and
ordered payment of a forfeiture for operating an establishment without employing a manager at
the establishment. Dale and Donna Baker are the owners of European Nail and Skin Center in

Howard Bessette, Hazelhurst – Registered Nurse. The Board of Nursing suspended and
limited the license of Howard Bessette for diverting morphine and another drug from his
employer, for using the drugs himself, and replacing the morphine with saline. Bessette was
convicted in Oneida County Circuit Court of crimes because of his drug theft and use.

Patricia Carran, Waupun – Registered Nurse. The Board of Nursing reprimanded and
limited the license of Patricia Carran for failing to properly care for a patient at the Waupun
Correctional Institution. [LS0308082NUR]
Kathleen Davis, Oregon – Registered Nurse. The Board of Nursing accepted the surrender of
Kathleen Davis’ license. Davis tested positive for a controlled substance, left a partially filled
syringe in a patient’s room and made a series of documentation errors. [LS0308081NUR]
Brenda Dicks, Elkhorn – Manicurist. The Barbering and Cosmetology Examining Board
ordered Brenda Dicks to pay a forfeiture for performing manicuring services at Long Nail Salon
in Lake Geneva without a current license. [LS0308048BAC]
Phan Doan, Lake Geneva – Manicuring Establishment Owner. The Barbering and
Cosmetology Examining Board ordered Phan Doan to pay a forfeiture for employing an
unlicensed person, offering aesthetics services to the public without a current aesthetics
establishment license and because of violations of sanitary requirements at Long Nail Salon in
Lake Geneva. [LS0307091BAC]
James Gill, Appleton – Barbering and Cosmetology Establishment Owner. The Barbering
and Cosmetology Examining Board ordered James Gill to pay a forfeiture for employing an
unlicensed person to provide aesthetics services to the public at Salon Aura in Appleton.

Bonnie Kirchberg, Beaver Dam – Practical Nurse. The Board of Nursing suspended the
license of Bonnie Kirchberg. Kirchberg failed to properly care for a diabetic patient assigned to

Tuan Phan, Milwaukee – Barbering and Cosmetology Practitioner. The Barbering and
Cosmetology Examining Board ordered Tuan Phan, owner of Sun Nails in Milwaukee, to pay a
forfeiture for employing an unlicensed person to provide barbering and cosmetology services.

Pharmacy Buying Group of America, Mequon – Distributor. The Pharmacy Examining
Board reprimanded Pharmacy Buying Group of America and ordered a forfeiture. The facility
failed to meet security requirements and engaged in activities requiring a distributor license

Reesasue Pieper, Green Bay – Barbering and Cosmetology Manager. The Barbering and
Cosmetology Examining Board accepted the surrender of Reesasue Pieper’s manager’s license
and establishment license for Reesa’s Hair Hut, based upon criminal convictions.

Geralyn Sauter, Greenville – Barbering and Cosmetology Manager. Trade Secrets #7733,
Racine – Barbering and Cosmetology Establishment. The Barbering and Cosmetology
Examining Board ordered Geralyn Sauter, manager of Trade Secrets #7733 in Racine, to pay a
forfeiture for employing an unlicensed person to provide manicuring services to the public. The
establishment was also ordered to pay a forfeiture. [LS0308044BAC] [LS0308041BAC]
Douglas Scheffler, Appleton – Barbering and Cosmetology Manager. The Barbering and
Cosmetology Examining Board ordered Douglas Scheffler to pay a forfeiture for employing an
unlicensed person at Salon Aurora in Appleton, to provide aesthetics services. Scheffler is the
manager at the salon. [LS0308043BAC]
Select Specialty Hospital, Milwaukee – Pharmacy. The Pharmacy Examining Board
reprimanded Select Specialty Hospital and ordered payment of a forfeiture because the
establishment failed to have a centrally monitored alarm system. [LS0308133PHM]
Tara Sheehy, Sun Prairie – Barbering and Cosmetology Manager; Dale Nelson, Madison –
Barbering and Cosmetology Manager; Head Games Salon, Madison – Barbering and
Cosmetology Establishment. The Barbering and Cosmetology Examining Board ordered the
managers of Head Games Salon, an establishment located in Madison, to pay a forfeiture for
renting a booth to a person who did not have an establishment license or a written lease

Regina Young, Milwaukee – Registered Nurse. The Board of Nursing suspended and limited
the license of Regina Young for unauthorized use of a controlled substance. [LS0304222NUR]
