Documentation Guidelines
It is important for the students to document their activities during their
Industrial Training, to assess whether they have fulfilled the objectives and
requirements of Industrial Training.

Progressive Reports
Students must document their activities for each week in their
Progressive Reports 1 and 2. The Progressive Reports 1 and 2 must
be commented and certified by the company supervisor, and must
contain the following sections:

(a)
The softcopies of the Progressive Reports are to be sent by
email only to the MMU supervisor. Hence, company
supervisor signature is not needed at this stage.
(b)
The certified copies of the Progressive Reports 1 and 2 (same
as 2.1 a) are to be kept by the students and to be included in the
Appendices of the Final Report. They are to be stamped and
signed by the company supervisor.
(c)
MMU supervisor shall visit the students during internship period.
(d)
The students shall also ensure the Employer’s Evaluation Forms
are collected from the employer in a SEALED envelop to be
handed over to the MMU supervisor or alternatively
scanned/faxed over to the coordinator of Industrial Training.
Note that the students are not allowed to open the sealed
envelop. Failure to abide this rule shall mean that the student
will fail in these two components.
Final Report
The Final Report documents all the activities throughout the Industrial
Training sessions. The report should provide details of all the projects,
tasks, or assignments during the training. Additional information such
as application of skills towards the tasks, problems encountered and
recommendations would provide better understanding of the
experiences during the training. This will assist in the evaluation that
the student has gone through sufficient practical experience and
fulfilled the requirements of the Industrial Training. See the detailed
guidelines on Final Report in Template B.
3.1
Format
One black comb-binding copy is to be submitted to the MMU
supervisor at the end of the Industrial Training. The maximum number
of pages is 30, excluding the Preliminary Pages and the Reference
Materials (including Appendices).
3.2
Deadline and Mode of submission
23-05-2014 before 5.00pm
Personal service (hand delivery) or by prepaid registered post or by
courier service.

The final report shall be submitted to the respective supervisor of the
Industrial Training programme.
