Additionally Dianella has an extensive salary packaging
program details of which are available on request.
The position will have access to a Dianella pool car for work
related travel.
Located at Broadmeadows however staff may be transferred to
other locations in Hume depending on space and service
demand.

POSITION SUMMARY
The objective of the Hume School Focused Youth Service is to ensure that at risk young people (aged 10 to 18
years) are well supported in their communities, that risk factors are addressed or mitigated and that they
continue to be engaged and connected to education. This will be achieved by:




Coordinating the provision of community based services in partnership with schools and community
agencies to support schools to address the needs of at risk young people
Fostering an integrated service delivery model through developing shared protocols and systems
Enhancing existing partnerships and creating new partnerships and pathways between schools and
community organisations to support at risk young people
To assist all schools to this endeavor including Government, Independent and Catholic schools in their
support for at risk young people, and in particular, by working closely with and complementing the
Student Support Services.
DUTIES AND RESPONSIBILITIES

Plan, implement, review and evaluate the Hume SFYS work plan and collect data as appropriate.
Adhere to the SFYS 2013 – 2015 guidelines.
Coordinate care to vulnerable young people and their families by fostering an integrated service model
between schools and local service providers.
Chair and coordinate the SFYS Governance and Advisory Group and to ensure active membership by key
stakeholders such as SSS Network Executive Chairs and Co-ordinators, Catholic Education and
Independent Schools representations, Hume Whittelsea Local Learning and Employment Network
(HWLLEN), Hume Council, Mental Health, Child First.
Identify the psychosocial needs and emerging issues of the community and of individuals and identify at
risk/priority groups and work pro-actively with schools and community agencies to address the needs and
reduce risk.
Ensure the Hume SFYS is responsive to local issues, emerging issues and trends by accessing and utilising
local and broader data as well as utilising outcomes/measuring tools such as the Student Online Case
System (SOCS), the Australian Early Development Index (AEDI), and the Victorian Child and Adolescent
Monitoring System (VCAMS).
Work in partnership with service providers, community agencies and groups and health professionals within
and external to Dianella to develop implement and evaluate other integrated health promotion programs.
Utilise a range of health promotion interventions and be prepared to work in settings outside the centre to
promote social and emotional wellbeing.

Position Description

To coordinate the use of flexible funding as per the SFYS work plan and ensure funds are accounted for by
working collaboratively with Dianella Accounts Payable/Finance.
Professional Development:
o Participate in individual and group supervision.
o Plan, implement and evaluate a professional development work plan.
o Utilise and contribute to the evidence based research on at risk youth and disseminate this information
accordingly in appropriate internal and external forums.
o Contribute towards social work fieldwork education by maintaining links with professional bodies and
educational institutions, and through the support of undergraduate student placements.
o Ensure a high quality social work service by updating own professional skills and knowledge through
attendance at appropriate courses/workshops/seminars.
General responsibilities
o Maintain Duty of Care.

o Assist in the planning, development, implementation and evaluation of programs that respond to the
needs of the community.
o Perform duties within the scope of practice of the role and the standards and guidelines set out by the
funding and accreditation bodies.
o Undertake appropriate activities as directed by management to ensure the quality and safety of services.
This includes participating in quality development activities, clinical audits, conduct of client satisfaction
surveys, and program evaluation as relevant.
o Supervise volunteers or students as required.
o Participate in the annual performance appraisal process.
o Promote distribute and share knowledge and information within the organisation and relevant parties.
o Develop and maintain working relationships with appropriate local and regional service providers.
o Participate in the development and maintenance of effective team processes.
o Provide a best practice model of service delivery.
o Be professionally accountable.
o Identify work processes that could be improved and present possible strategies for service.

Coordinators as above.
o Promote, develop and maintain relationships with local community agencies as above.
o Maintain relationships with other SFYS agencies and consortia in the coordination of broader regional
services.
o Represent Hume School Focused Youth Service at network meetings and advocate for the objectives of
the program through collaboration and within the spirit of community health.
o Represent Dianella Community Health in a positive and professional manner to other organisations, funding
bodies and the public.
o Participation in other relevant professional networks.
MAINTAIN PROFESSIONAL SKILLS AND QUALIFICATIONS

Maintain eligibility for registrations with relevant professional bodies and fulfil the requirements of the
Association including maintaining their code of ethics.
Participate in continuing professional education and development in order to maintain the highest possible
service standards.
Continually upgrade skills and knowledge through attending seminars and workshops and reading
professional journals and newsletters.

Minimum of 2 years field experience.

setting.
Demonstrated knowledge and experience in working within a social model of health and an
understanding of the social determinants of health.
Knowledge of health promotion principles, including population based health promotion, teaching goal
setting and self management skills.
An ability to deliver health promotion programs in a school and community setting.
Experience in the provision of services to people of non-English speaking backgrounds, including newly
emerging refugee and asylum seeker communities.
Demonstrated knowledge and experience in working with women and children who have experienced
family violence.
Demonstrated knowledge and experience in the supervision of undergraduate social work students.
Another language relevant to the community would be an advantage.
