What is Self Management?
Self management is about people that are living with a long term condition
being in control of their own lives.
It supports you by helping you to access the information and skills you need
to manage your condition better and your overall health and wellbeing.
What are the benefits of Self Management for me?
Self Management can put you in control of your life, helping you to make the
decisions that are right for you.
It can mean you;

understand your condition better
manage your symptoms more effectively
are more confident in dealing with professionals
are better supported when you need it
develop your own resource (self management pack) that builds all the
information into one place
Who is the programme for?
The programme is for adults living with a long term neurological condition,
apart from those with a diagnosis of stroke or dementia, and their carers.
How long does the programme last?
The programme runs for six weeks, one module per week for 2 ½ hours.
Where is the programme run?
The programme is run at various community venues within Gateshead, South
Tyneside and Sunderland.
How much does the programme cost?
The programme is free of charge. Travel expenses can be paid and light
refreshments will be provided.
What can I expect?
The modules are run in a safe, supportive environment with approximately 16
people attending, your carer can also attend with you.

Accessing Timely Information (when you need it)
Each module will include a mix of presentations by local experts and
clinicians, and group work, but no-one will be put on the spot. You will also be
provided with a self management pack and help on how to use it will be
included throughout the six weeks.
What is expected of me?
The programme runs for six weeks, although attending every module would
be of benefit, you can choose to attend the modules that you feel are most
beneficial to you.
How do I get involved?
You can be referred to the service by
• your GP
• a health professional or social worker
• your carer
• self referring – see telephone number and email address at the end of this
leaflet
You will then be given an appointment with one of our clinicians to discuss
your needs.
Interpreters
If you would like an interpreter, this can be arranged prior to your
appointment.

If you want to make a comment, suggestion, compliment or complaint
you can:
• talk to the people directly involved in your care
• complete a feedback card, available on wards. Some areas of the
Trust have electronic feedback touch screens, staff can help you to
use these. Your feedback allows us to monitor the quality of our
services and act upon issues that you or your carers bring to our
attention.
• ask a member of staff for a feedback form, or complete on the Trust
