PREPAID PROVIDER DIRECTORY
The following steps will enable you to access your Prepaid dental care benefits.
SELECTING A DENTIST

Select a general dentist from this directory. For ease of selection, we have listed general dentists alphabetically and
geographically.
List the dentist's ID # (see ID# next to the dentist's name) on your enrollment form.
If you are unable to choose a dentist at the time of enrollment, you must call the Customer Service Department on
or after your effective date of coverage, and select a dentist before you can make an appointment.
CHANGING YOUR SELECTED DENTIST
You may select another participating General Dentist by calling the local TDAHP Customer Service Office during normal
business hours and requesting a change of dentist. Requests must be received by the 15th of the month to be effective the 1st
day of the following month. Updated directories of participating dentists can also be obtained from the local TDAHP
Customer Service Office.
MEMBER ID CARDS & PLAN BOOKLETS
A Member Identification Card will either be delivered to your employer or mailed to your address. However, this ID Card is
not required to visit your selected participating general dentist. Dentists verify membership from a list they receive each
month. Please retain your Enrollment and Employee Plan Booklet for referencing the details of your Plan.
MISSED APPOINTMENTS
You must give a 24-hour notice of your appointment cancellation to your dentist's office, or you will be charged a missed
appointment fee as listed in your Schedule of Benefits.
REFFERALS TO SPECIALISTS
Prior authorization from TDAHP is required for some plans. Please consult your Schedule of Benefits.
ORTHODONTIA SERVICES
To make an appointment, call one of the Orthodontic Offices listed in this directory and identify yourself as a TDAHP
member.
EMERGENCY CARE PROCEDURES

Member should always FIRST attempt to obtain emergency care from their designated Plan Provider when the
Member is within the area served by their designated Plan Provider.
If the Member is seeking emergency care during normal business hours and the Member's selected Plan Provider is
not accessible, the Member may contact the Plan for assistance at Phoenix Metro Area (602) 266-1995 or Outside
of Phoenix Metro Area 1-888-422-1995.
When a Member's Plan Provider is not accessible or when the emergency occurs outside the area served by the
Member's Plan Provider, then the Member should seek emergency dental care from a licensed dental health
professional to control bleeding, relieve pain, including local anesthesia, or eliminate acute infection. Medications
that may be prescribed by the dentist but must be obtained through a pharmacy are excluded. If it is necessary to
have additional treatment, it must be done by the Member's Plan Provider in order for benefits under this
Agreement to be applicable.
