If the child is between the ages of 16 and 18, but does not have the necessary proofs of identitv, helshe must subm.it a MV-45 statement of Identity by a Parent or Guardian, and hisher parent or guardian must be present to sign the form in the presence of a DMV representative. a Fees The fee for an EDL is $30 in addition to the fees requiredfor your driver license or non-driver photo ID transaction. In comparison, the cost of a U.S. passport is more than $100.
