The Forms Review- The Dissertation Process Liaison performs the forms review to ensure that all required forms are available and complete

The Academic Review-The School of Advanced Studies

Academic Review Board (ARB) members are trained to evaluate the proposal (e.g., chapters 1, 2, 3) against the requirements specified in the Academic Review Checklist Form.

The Institutional Review - Two members of the University of

Phoenix Institutional Review Board (IRB) perform an IRB assessment to ensure a proposal meets standards for the treatment of research participants.

Slide 3 provides a general background on each of the proposal steps.

Email Verification- An email is sent to the full committee to ensure he/she supports the learners submission and has reviewed all forms and the final proposal.

‹

Turnaround time- Forms Review will be done within 3 business days of the learner submitting providing the Dissertation Process

Liaison has received all verifications. ARB will be done within 13 business days. IRB will be done within 5-10 business days .

Slide 4 provides general information needed for a proposal to be processed.

As a thorough review is completed on each proposal, you can expect the process to take at least 20 business days. This is only a guideline, not a guarantee. Some reviews will take more or less time, due to the type or length of the proposal. You should also be aware that during certain peak periods, such as the months leading up to the graduation ceremony, review times are generally longer.

You are eligible to submit the proposal package after you have successfully completed the year 3 residency. Please note that you must also be currently enrolled in a year 3 group study course or one of the DOC/733 courses in order to submit.

INK Signatures are required from organizational entities, in order for a learner to meet IRB requirements. The Permission to

Use Premises/Name/Subjects form must be signed prior to submission by an authorized authority. The same is true of the

Permission to Use An Existing Survey and Letter of

Collaboration forms.

However, the Informed Consent, on the other hand, must be complete, but without participant's/parent's signatures, when submitting an IRB package. The two Informed Consent form versions are for either participants over or under the age of 18 respectively.

Accessing Forms and Submitting for

ARB/IRB on Learner Homepage

This is the learners homepage. In the bottom right corner is where the learner will retrieve all forms and submit his/her proposal for ARB/IRB (see highlighted portion).

The learner will select from the box at the top of the page the type of submission - Original, ARB Resubmission, or IRB Resubmission. (MORE

The learner also be able to download the necessary forms. Place a check in each box to download the appropriate forms. Once you have selected the necessary forms, click the ‘ download ’ button at bottom of page. If you made an error and want to choose again, hit the ‘ reset ’ button also at the bottom of the page.

After completing and saving the forms to the computer. The learner will upload each form one at a time under the appropriate heading from the location all documents were saved.

Once all documents have been uploaded, select the ‘ submit ’ button at the bottom of the page. If the learner made an error while uploading, select the

‘ cancel ’ button and the documents will not be uploaded, also located at the bottom of the page

This is the top half of the Proposal Cover Page form; it is three pages long. On

Page 1 please make sure to fill in this contact information for the learner and

Committee Members. Please make sure to type all email addresses correctly.

Continuing with the Proposal Cover Page form, these are the directions for submitting the proposal and additional forms; please note, 15 business days for a response for your proposal review is not guaranteed, but to serve as a guideline.

o The Appendixes is the last section of the Proposal Cover Page form.

Here the learner selects which Appendix he/she will use; if applicable.

The learner will alphabetize all Appendixes being used. If an Appendix will not be used place an “ X ” in the “ N/A ” box.. See above for example.

o The Institutional Review Board Application. This form is very important to the approval of IRB. This form is 4 pages long. Page 1 is the general information about the Learner, along with dissertation name and purpose.

Page 2. The description of the project is placed here. Additionally, any federal guidelines that the subjects involved will fall under will be selected here.

This section also addresses NON-Exempt; also known as Protected Classes.

If in Section 3, Letters A-H are marked ‘ yes ’ , the Learner will be sent to the

Full IRB Board.

This is Page 3 of the IRB Application. The top half of this page continues the questions pertaining to federal guidelines. Under Principal Investigator the learner will place his/her demographic information.

This is Page 3 of the IRB Application. This is for the Mentor to complete. The most important item here is selection ‘ exempt ’ or ‘ not exempt ’ , as well as if the study is using archival data. Please make sure to answer the ‘ why ’ section in detail.

The last half to Page 3 is for the Review Board. Here the results are posted to determine if this proposal is approved, denied or needs to be brought to a full review board.

The Self-Assessment Academic Review Checklist is very important. Please make sure there are no names on this form. Here the learner will place a score for each section. This form is 12 pages long. Each section should meet requirements, and a 3 placed in each box.

This is what the Academic Reviewers use to determine the ARB results. This section is sent back to the committee; having chosen one of the above as the results: Approved, Approved with Changes Needed, Not Approved

Modules 1-6 and Module 15 for initial completion

If you are doing a ‘non-exempt’ study, there will be additional modules to complete.

Slide 24 discusses the modules necessary for submission. However if the learner is taking the refresher course, only Modules 1-5 are needed.

This is an example of how the CITI, also known as the Human Subjects

Training should be submitted. Please note if it has been two years since the training was last completed, please go to www.citiprogram.org to take a refresher course.

To ensure originality and to correct instances of unintended plagiarism, you are now required to include a Turnitin ® report from the Center for Writing

Excellence (CWE) Plagiarism Checker as part of the proposal package.

To access the Turnitin ® site, you should log in to your homepage https://ecampus.phoenix.edu

. You should then select the services tab and go to the CWE. On the CWE page, “ submit paper ” on the left side. Please then scroll down to the bottom of the page and fill in the appropriate information and run a plagiarism check.

On this page, there is an option to also take a tutorial on the process.

Please note: each time the report is run, you are asked to rename the file or the documents results will render 100% plagiarism.

You should submit a Turnitin ® report for all ARB original submissions and resubmissions . You should upload the proposal package including the

Turnitin ® report to the SAS Web.

***This document must be submitted in either Word or PDF format.

This is the proposal title page. It should not have any names on it when submitting to ARB/IRB

Informed Consent: Participants Under 18 Years of Age, Signatures will be obtained after IRB Approval for this form. The template letter to be sent to the participants needs to be submitted as an Appendix to be reviewed prior to

IRB Approval.

Signed Letter of Collaboration Among Institution(s), an Ink signature will need to be obtained prior to Forms submission.

Once the oral defense is completed (please make sure the learner is in class) and each member of the committee has provided his/her feedback. The Mentor will email the oral defense form to the Dissertation Process Liaison. The learner will then post his/her dissertation to the SAS Web. Visit https://ecampus.phoenix.edu

Email Verification - The Dissertation Process Liaison will email the full committee for initial and resubmissions to verify approval

(an oral defense completed form is required prior to the initial review being conducted)

Please note that with a dissertation resubmission a Change

Submitting Dissertation on Learner

Homepage

This is the learners homepage. In the bottom right corner is where the learner will submit the dissertation (see highlighted portion).

The learner will select from the box in the top corner the type of submission

(see highlighted section), either Original or Resubmission.

The learner will upload the dissertation under the appropriate heading from the location all documents were saved.

Once the document has been uploaded, select the ‘ submit ’ button. If the learner made an error while uploading, select the ‘ cancel ’ button and the document will not be uploaded.

*Note: If this is a resubmission, please make sure to select Resubmission in the top corner and also upload a Change Matrix

When the dissertation is approved, the learner will upload a final dissertation with any changes the Dean has asked back to the

SAS Web (note even if no changes were asked, please submit a final copy).

After the Dissertation Process Liaison has converted the final dissertation to PDF, information about the Ink Signature Page and ProQuest will be sent.

‹

Please do not start the signature page without having received an email from the Dissertation Process Liaison with complete directions.
