T T H E E M P L O Y E E S P R O V D E N T F U N D TH HE EE EM MP PL LO OY YE EE ES SP PR RO OV VIIID DE EN NT TF FU UN ND D INCAPACITATION WITHDRAWAL PURPOSE This withdrawal allows you to withdraw all of your EPF savings should you become physically or mentally incapacitated to work, having achieved the level of Maximum Medical Rehabilitation (MMI) to work to help earn a living. APPLICATION ELIGIBILITY (i) (ii) A Malaysian Citizen; OR A Malaysian Citizen who has made Leaving The Country Withdrawal before 1 August 1995 and later opted to re-contribute to the EPF; OR (iii) A Non-Malaysian Citizen who: Has become an EPF member before 1 August 1998; OR Has obtained a Permanent Resident status (PR). You have not reached the age 55 at the time the EPF receives your application (Malaysian citizens who are already 55 years old are advised to make withdrawal under the Age 55 Withdrawal); AND You still have savings balance in your EPF Account. You are no longer employed at the time the withdrawal application is made. TERMS OF WITHDRAWAL 1. You have been confirmed incapacitated, physically or mentally, having achieved the level of Maximum Medical Rehabilitation (MMI) to work by a doctor who has examined you AND/OR by a medical doctor in a Medical Board/Panel Clinic appointed by EPF. 2. Your application must be supported with a medical report dated not more than 1 year from the date the application is received. 3. You are no longer working at the time the application is made. 4. Requirement to attend an interview session: You will be required to attend an interview session conducted by EPF Officers. WITHDRAWAL AMOUNT ELIGIBILTY All your EPF savings. LIST OF DOCUMENTS REQUIRED NO. MAIN DOCUMENTS 1. KWSP 9L (AHL) Withdrawal Form. 2. Personal Identification Card a. MyKad / Military Identification Card / Permanent Resident Identification Card (MyPR) OR b. Police Identification Card AND Verification Letter from Employer stating that the Police number and Identification Card number refer to the same person (if without MyKad) OR c. Passport. If the member is not a Malaysian citizen. 3. Payment Via Direct Crediting Into Applicant’s Bank Account Bank Passbook / Savings Account Statement OR Current Account Statement OR Verification Letter of Account Holder’s Details from Bank OR Account Holder’s Details Printed via Website Note: Owns an account with the panel bank appointed by EPF The bank account must still be active and Personal Account type only However, Joint Account type is only allowed for withdrawal amount of more than RM100,000.00 (Please bring original documents for verification) 4. Payment Via Mail Application for Member’s Registration and Amendments of Member’s Particulars (KWSP 3) NO. SUPPORTING DOCUMENTS 1. Original Medical Report from the Specialist Medical Institution / Non-Specialist (Hospital / Government Clinic or Hospital / Private Clinic) In accordance to the format as stipulated by EPF and does not exceed 1 year from the date the application is received Complete with the patient’s information and details of illness The Medical Institution’s stamped name and address The doctor attending to the patient’s signature, together with the stamp of full name, designation, MPM registration number and field of work Additional acceptable documents (not exceeding 1 year from the date the application is received), either: Decision / Recommendation by SOCSO Panel of Doctors (Invalidity) – PKS: (P) 9; OR Your Retirement Report which indicates health reasons; OR Invalidity Confirmation Letter from SOCSO (for Invalidity Pension calculations) – PKS: (P) 19. Note: You must submit a copy of the medical report together with any other related supporting document, such as X-Ray, Echo, MRI, ECG, EMG, EEG, HPE and blood test result to the Medical Board or EPF Panel Clinic when attending the incapacitation checkup. 2. Resignation Letter from your employer (if terminated within one year from the application date) Stating the reason(s) for termination of employment Former employees of public services must produce a resignation letter. 3. Interview Form You are required to attend an interview session conducted by EPF officers and this form will be filled in by the officers. NOTE: EPF reserves the rights to request any additional documents if required and reject any incomplete applications and do not comply to the terms and conditions. ATTENTION AND NOTICE TO APPLICANT The Incapacitation Withdrawal is subject to the following: (i) Confirmation by the Medical Board or EPF Panel Clinic and/or the doctor who treated you relating to your disability status to work. (ii) The Incapacitation Benefit will be paid subject to the current requirements imposed by EPF. Confirmation of Disability to Work: (i) If you are receiving treatment from a specialist doctor in a government / private hospital, EPF will request for the incapacitation confirmation for you to work or any related document from the said doctor (if required) (ii) If you have produced a medical report from a government / private hospital’s medical doctor, you will be referred to the Medical Board or EPF Panel Clinic to confirm your incapacitation. (iii) If the medical report confirms that you are unable to manage yourself and your finances, the payment will be made to any eligible individual at EPF’s discretion. (iv) If you have produced an Invalidity Pension Report (SOCSO), you will be referred to the Medical Board or EPF Panel Clinic for incapacitation confirmation to work. Incorrect or False Declaration or Furnishing False Documents If the applicant provides incorrect or false declaration or furnishes false documents, the applicant is considered as has committed an offence and shall, on conviction, be liable to imprisonment for a term not exceeding three years or to a fine not exceeding RM10,000.00 or to both [Section 59, EPF Act 1991 (Amendment) 2007]. MODES OF PAYMENT (i) Payment in Malaysia All withdrawal payments will be credited directly into your account subject to the following: a. You have an account with a panel bank appointed by EPF; AND b. Your account is still active; AND c. Your bank account is a Savings / Current Account (personal) OR Joint Account for withdrawal amount of more than RM100,000.00. d. Your identification number matches with the bank’s record e. Payment is made in Ringgit Malaysia (RM) However, if the payment cannot be credited into member’s bank account due to causes such as bank account is not active or your identification number does not match with the bank’s record, payment will be made via banker’s cheque. (ii) Remittance of Payment to Foreign Countries Payment is made via Foreign Bank Draft. Types of currencies are as follows: Foreign currency as per member’s choice stated in the application form subject to the currency is available in EPF’s list for the purpose of payment via Foreign Bank Draft; OR In US Dollar if the currency stated by the member in the withdrawal form is not available in EPF’s list for the purpose of payment via Foreign Bank Draft Note: EPF will make a verification with the member again if the type of currency chosen in the application form is not available in the allowed list or before changing the type of currency to US Dollar. (iii) Payment to Guardian If the doctor’s report indicates that you are unable to manage yourself and your finances, the payment will be made to a qualified person at EPF’s discretion after a visitation has been made by EPF officers. For this purpose, the qualified person must complete a stipulated EPF form. ADDITIONAL INFORMATION Incapacitation Benefit You are eligible to receive an Incapacitation Benefit from EPF amounting to RM5,000.00. This payment is in addition to your total savings amount and to express EPF’s sympathy of your condition. The benefit will be paid to you only once. You are eligible if you: (i) Apply for Incapacitation Withdrawal within 12 months from the date of unemployment/ termination. (ii) Your final service duration must be in at least 6 continuous months. (iii) You were terminated by your employer due to your incapacitation to work and not based on voluntary basis or disciplinary action. Re-employment and Your Nomination Status If you re-join the employment once you have made the Incapacitation Withdrawal, it is compulsory for you to re-contribute to EPF. You are also required to do a new nomination since your previous nomination is considered null and void after your Incapacitation Withdrawal. Release of Investment Controls If you have made an investment with any of the approved Fund Management Institutions via the Investment Withdrawal, EPF will release all controls on the investments you have made with the institution(s) once you have made the Incapacitation Withdrawal. Further claims are between you and the Fund Management Institution(s). MODES OF SUBMISSION Submission can be made at any EPF branch either at the counter OR via mail. SUBMISSION AT THE COUNTER Please bring along ALL original documents for verification purposes by the EPF Officer. However, if the photocopied documents have been verified, please ensure the photocopies have been verified by the authorised officer by EPF, complete with signature, name, designation and official stamp.
