60 AMW Quarterly Award Guidance (Interim)

General Information:
- Group Commanders and the Director of Staff send a maximum of one nomination submission
for each category
- A write-up, limited to 10 lines (NOT including headers), single-spaced, size 12 font, using only
the AF IMT 1206-version 5, Nomination for Award
- Use bulleted, short statements or key points
- Include specific facts, achievements, and examples showing the nominees are exceptional
- Achievements should distinguish nominees from his/her peers
NOTE 1: Standard Air Force acronyms and abbreviations are acceptable; spell out all nonstandard acronyms upon first use
NOTE 2: Acronym legends (listing acronyms accompanied by explanation below the 10 lines)
are permitted.
NOTE 3: Headers will NOT contain any additional text
NOTE 4: Board members are NOT to score packages within their respective groups.
NOTE 5: This guidance contains all guidance for Volunteer Award Categories
NOTE 6: Family Member Volunteer Award and Key Spouse Awards are to be routed through
the SPONSORS unit.
Airman, NCO and SNCO Categories: Nominations must include the following areas and cite
examples that occurred only during the award period:
LEADERSHIP AND JOB PERFORMANCE IN PRIMARY DUTY (6 Bullets Max): Describe
significant leadership accomplishments and how well the member performed assigned primary
and additional duties. Define the scope and level of responsibilities and the impact on the
mission and unit. Include any new initiatives or techniques developed by the member that
positively impacted the unit and/or the mission. Include results of Air Force, MAJCOM,
Numbered Air Force-level inspections and/or evaluations. Include awards received; e.g., NCO
of the Quarter, Maintenance Professional of the Year, and so forth.
SIGNIFICANT SELF-IMPROVEMENT (2 Bullets Max): Show how the member developed or
improved skills related to primary duties; e.g., formal training, career development course
enrollment or completion, on-the-job training, certifications, off-duty education related to
primary duties, and so forth. Include completion of any professional military education (PME)
as well as awards earned during in-residence attendance. Include any off-duty education not
directly related to primary duties; e.g., class, course, degree enrollment and/or completion, and
grade point average. Cite any other relevant training or activity that significantly enhanced the
member's value as a military citizen.

BASE OR COMMUNITY INVOLVEMENT (2 Bullets Max): Define the scope and impact of
the member's positive leadership and involvement in both the military and civilian community.
Include leadership, membership, or participation in unit advisory councils, professional military
organizations, associations, and events; e.g., President of Top 3, enlisted dining-out committee,
member of Air Force Sergeants Association or Air Force Association, Sunday school teacher,
and so forth.
CGO Category: Nominations must include the following areas and cite examples that occurred
only during the award period:
LEADERSHIP AND JOB PERFORMANCE IN PRIMARY DUTY (6 Bullets Max): Describe
significant leadership accomplishments and how well the nominee performed assigned primary
and additional duties. This could include job knowledge and leadership qualities applied to a
specific Air Force problem, combat situation, or development of new techniques, procedures, or
processes which result in increased mission effectiveness.
SIGNIFICANT SELF-IMPROVEMENT (2 Bullets Max): Show how the nominee developed or
improved skills related to off-duty schooling, membership in professional or cultural societies or
associations, or development of creative abilities. Show how the nominee demonstrated ability
as an articulate and positive spokesperson for the Air Force.
BASE OR COMMUNITY INVOLVEMENT (2 Bullets Max): Define the scope and impact of
the nominee’s positive leadership and involvement in both the military and civilian community.
Include participation in social, cultural, or religious activities in the military or civilian
community which, tangibly or intangibly, contributed to community or group welfare, morale, or
status. Include Air Force and civilian awards, prizes, titles, certificates of appreciation, etc.,
obtained as recognition for personal services rendered or contributions made to military or
civilian community life.
FGO Category: Nominations must include the following areas and cite examples that occurred
only during the award period:
LEADERSHIP AND JOB PERFORMANCE IN PRIMARY DUTY (7 Bullets Max): Describe
significant leadership accomplishments and how well the nominee performed assigned primary
and additional duties. This could include job knowledge and leadership qualities applied to a
specific Air Force problem, combat situation, or development of new techniques, procedures, or
processes which result in increased mission effectiveness.
NOTE: The focus for the FGOs will solely be on the above category.
NOTE: FGO nominee cannot be a Commander.

Civilian Categories I/II/III:
- Category I: includes all employees in grades GS-/GG-01 through 08, WG-/WL-01 through
08; NA-/NL-/NS-01 through 08, CC-01 through 05, and NF-I and II
- Category II: includes all employees in grades GS-/GG-09 through 12, WG-/WL-09 and
above, WS-01 through 12, NA-09 and above, NL-/NS-09 through 12, and NF-III and IV
- Category III: includes all employees in grades GS-/GG-/GM-13 and above, WS-/NL-/NS-13
and above, and NF-V and above

Nominations must include the following areas and cite examples that occurred only during the
award period:
LEADERSHIP AND JOB PERFORMANCE (6 Bullets Max): Characterize the nominee’s
personal leadership as evidenced by character, conduct, and willingness to accomplish goals and
accept additional responsibility. This could include leadership qualities applied to a specific Air
Force problem or development of new techniques, procedures, or processes which resulted in
increased mission effectiveness. Provide factual and substantiated examples of the nominee’s
significant achievements and performance above general job requirements. Describe how well
your nominee improved operations; highlight any support he or she provided for special projects;
explain any other outstanding services the employee provided that benefited the organization’s
mission. All accomplishments must have occurred during the 3-month nomination period.
OTHER SIGNIFICANT ACCOMPLISHMENTS/COMMUNITY EFFORTS (4 Bullets Max):
Describe the nominee’s self-improvement efforts, if applicable. This may include training and
education activities, additional duties, community and/or civic activities, and family
enhancement.
Team of the Quarter: For the purpose of this award, a “team” is defined as two but not more
than eleven (think team leader plus 10 members) personnel working in concert to achieve an
outcome. Nominations will include specific facts, achievements, and examples showing the
team is exceptional and must include the following areas and cite examples that occurred only
during the award period:
TEAM COMPOSITION: List rank, name (last, first, middle initial), unit, and office symbol of
team leader/members.
AWARD JUSTIFICATION: Describe team’s achievements/accomplishments/impact/results.

60 AMW Commander’s Eagle Trophy Award (Squadron of the Quarter):
Selections will be based on a combination of the following areas.
COMMANDER’S PROGRAMS: Significant squadron programs supporting successful mission
attainment, squadron effectiveness, and safety. (5 Bullets Max)
CONTRIBUTIONS TO TEAM TRAVIS: Squadron efforts that contribute tangibly or
intangibly to the military or civilian community's welfare, morale or status. (5 Bullets Max)
NOTE: Deputy Group Commanders score the nomination packages for this category.
Military Volunteer Award: Nominations will include the following areas and cite examples
that occurred only during the award period:
BASE OR COMMUNITY INVOLVEMENT: (6 Bullets Max) Define the scope and impact of
the nominee’s positive leadership and involvement in both the military and civilian community.
Include participation in social, cultural, or religious activities in the military or civilian
community which, tangibly or intangibly, contributed to community or group welfare, morale or
status. Include Air Force and civilian awards, prizes, titles, certificates of appreciation etc.
obtained as recognition for personal services rendered or contributions made to military or
civilian community life.
Civilian Volunteer Award (Retiree, Non-DoD Affiliated & DoD Civilian: Nominations must

Max) Define the scope and impact of the nominee’s positive leadership and involvement in both
the military and civilian community. Include participation in social, cultural, or religious

community or group welfare, morale or status. Include Air Force and civilian awards, prizes,
titles, certificates of appreciation etc. obtained as recognition for personal services rendered or

Family Member Volunteer Award (Spouses & Youth) Volunteer Award: Nominations must
include the following areas and cite examples that occurred only during the award period:

Key Spouse Award: Nominations will include the following areas and cite examples that
occurred only during the award period:
BASE OR COMMUNITY INVOLVEMENT/SIGNIFICANT ACHIEVEMENTS :(6 Bullets
Max) Characterize the nominee’s contributions as evidenced by character, conduct, and
willingness to achieve unit goals. Provide factual and substantiated examples of the nominee’s
significant achievements and performance of Key Spouse duties. Describe how well the
nominee improved unit family cohesion; explain any other outstanding services the Key Spouse
provided that benefited the unit's mission.
