Your next steps in getting connected Forms & Waivers HEALTH HISTORY & IMMUNIZATION FORMS: Deadline August 1, 2013 All students will receive separate communication from the Health & Wellness Center which will include a Health History Form and Immunization Form with related instructions. All students must complete and return their Health History Form and Immunization Form by August 1, 2013. IMPORTANT NOTICE: Students who do not meet this deadline will have a nurse hold placed on their account, which will block them from course registration as well as prevent them from receiving housing assignments or financial reimbursements. The Saint Leo University U360 online community is a forum exclusively for students who have deposited and intend to enroll at SLU in the fall.The community provides a great way to interact with your future classmates, ask questions and make friends. But it is also a great source of information and is the perfect place to meet potential roommates if you’ll be living on campus. In fact, you will complete your housing survey using the Roommate Finder tool inside U360, then find potential matches based on your responses. Finally, you will submit your housing request to us via the U360 community. You can read more about this in the section on Residence Hall Agreement/Housing Request. STUDENT HEALTH INSURANCE/WAIVER: Deadline for Waiver: September 10, 2013 Saint Leo University requires all full time undergraduate students attending University Campus through our traditional program to carry health insurance. As a result all students are billed for a health insurance policy upon full time registration. Students who have their own coverage through an individual, group or family plan and would like to avoid double coverage may waive the University Policy and have the fee removed from their account. Please review the Student Health Insurance flyer in your New Student Packet. For additional information or instructions on completing the waiver please visit www.saintleo.edu/insurance. Waivers must be completed online at no later than September 10, 2013 for students attending full time in 2013 Fall 1 semester. Submission of insurance information in any other format will not be sufficient to waive coverage. International students may waive the insurance. Please note coverage used to waive the insurance must be provided by a US based insurer.Travel medical polices will not be sufficient to waive coverage. RESIDENCE HALL AGREEMENT / HOUSING REQUEST: Deadline: JULY 5, 2013 If you plan to live on campus, you must complete the Residence Hall Agreement. In order to guarantee housing, your signed Residence Hall Agreement must be received by July 5. Agreements received after July 5 will be processed on a space available basis. In addition to completing and returning your Residence Hall Agreement, you will also want to log on to the U360 online community and utilize the Roommate Finder tool. This tool allows you to do several things. First you will complete your housing survey. It is important to be honest on this survey! Once you have completed this, you will be able to use the Roommate Finder tool to search for a roommate based on your survey responses. The discussion boards in U360 are another great way to get to know potential roommates. When you are ready, you will use the Roommate Finder tool to submit your Housing Request. Housing Requests must also be submitted by July 5. When you do this, you have several options: 1.You may request a specific individual by entering that person’s name in a free-form field.You should tell the person you are requesting to be sure to submit their form indicating you as their roommate preference. 2.You may request that we select someone for you. 3.You may request that you be placed with someone from your same major or from your same athletic team. When you complete your Housing Request, you will also be able to select your residence hall preference (freshmen are generally placed in Marmion/Snyder/Henderson/Benoit). You will also be able to use the Housing Request to make Residence Life aware of any special accommodations or health issues that have a bearing on your housing. U360 Online Community for New Students EMERGENCY CONTACT FORM Saint Leo University wants to be certain to have valid contact information in the event that emergency notification is necessary. Please be sure to complete the Emergency Contact Form found in your New Student Packet and return it with your other documents. Academics ETS PROFICIENCY PROFILE Deadline: August 1, 2013 All students attending Saint Leo University as a first-time, first-year college student are required to complete the ETS Proficiency Profile assessment. This is an online tool that is intended to document your skills as you enter the university and is not used to place you in classes.Your scores are not used to compare you to other students.You should not stress or study for this assessment. Student IDs & Parking Decals All students attending Saint Leo University are provided with a student ID. In addition, any student who will be bringing a car to campus as either a resident or commuter must obtain a parking decal. Help us have your student ID and parking decal ready for you when you arrive on campus. Information on requesting your Student ID and ordering your parking decal in advance is available online. Student IDs Each student attending Saint Leo University is provided a student ID. These are used for a variety of purposes including building access and obtaining meals in the dining hall. In addition, ID cards can be loaded with “Monarch Money” and used for purchases at the Lion’s Lair and the Follett Bookstore on campus. Student IDs may be requested online or in person at Trane Stop at University Campus in Saint Edwards Hall. Completing the request online will help you to avoid long lines at the beginning of the semester. Simply visit www.saintleo.edu/studentID and follow the instructions for requesting a student ID. Once we receive this request we will make the ID and it will be available for you to pick up when you arrive on campus at the beginning of the semester, provided it is requested prior to August 9, 2013. There is no charge for your initial student ID. In the event that your ID must be replaced due to loss, damage or theft, a replacement fee of $25 will be assessed. Parking Decals Students who will be bringing a car to campus must register with the University for a parking decal. This includes both resident and commuter students. Students may do this online or in person at Trane Stop located in Saint Edwards Hall on University Campus. We strongly encourage students to register their vehicles online to help avoid long lines at the beginning of the semester. Vehicles may be registered online in advance. Additional information regarding this process will be available at www.saintleo.edu/parkingdecals after July 1, 2013. We will assign a decal to you and bill your student account. Parking decals are $60 for residential students and $30 for commuters. Your parking decal will be available for pick when you arrive on campus for the semester, provided it is requested prior to August 9, 2013. The deadline for completing this form is June 15, 2013. Students should complete the form only once. Class schedules cannot be created without the creation of this form. PLACEMENT TESTING First-time, first-year students may need to complete placement testing prior to enrollment at SLU.
