The conditions of your offer can be viewed
by logging into UCAS Track or your Student
Portal. Instructions on meeting these
conditions and completing the relevant forms
can be found below.
The Medical Questionnaire – Appendix 1

Candidates should complete the selfdeclaration of suitability. Please ensure that
you fill in your NAME and DATE OF BIRTH,
and answer QUESTIONS 1 to 5 on page 1. The
training establishment name is the University

name is Thomas Kidd. For each ‘YES’ answer
you ticked, you will need to provide a detailed
account of the circumstances on page 2. You
may attach and sign an additional sheet if more
space is required to account any incident(s). It
is important that you SIGN and DATE the back
of the form to validate its authenticity. Once
fully completed please scan the document and
upload to your Student Portal.

original certificates for GCSE in Mathematics,
English Language, and Science; and degree
award confirmation ONLY if these were not
brought to interview. If you do not have access

obtain duplicate certificates from the relevant
Examination Board. If that is not possible, we
will need signed and dated verification of your
awards and grades from the school/institution
where you took the examinations. Please send
us your original certificates WITHIN THREE
WEEKS of accepting our offer of a place.
Certificates will be returned to you by recorded
delivery. For those applicants still at university
please send an original transcript as soon as
your grades are confirmed in the summer. ALL
applicants are requested to scan and upload
copies of their certificates to their Student
Portal.
Pre-course Literacy and Numeracy Skills Tests
All applicants to teacher training courses are
required to pass the skills tests before starting
their course. The tests do not replace GCSE
grade C equivalence requirements.
The numeracy and literacy skills tests cover
the core skills that teachers need to fulfil their
professional role in schools, rather than the
subject knowledge required for teaching. This
is to ensure that all teachers are competent
in numeracy and literacy, regardless of their
subject strengths. Candidates are strongly
advised to review the online support material
before attempting the first of their professional
skills tests.
The DfE will update its website when further
information becomes available. The tests are
computerized and can be taken at any of
approximately 150 test centres throughout the
UK.
Initial teacher training providers will use the
results of skills tests to help them decide the
suitability of an applicant, so all applicants are
advised to take the tests as soon as possible
after submitting an application to avoid delays.

The Primary/Secondary School Experience
Report Form – Appendix 3 and 4 respectively
All PGCE Primary and BEd applicants are
required to have carried out at least two weeks
of satisfactory (primary) school experience post
interview and prior to starting the course in
September.
All PGCE Secondary applicants are required
to complete a minimum of three to five days
school experience. Ideally this experience
should be undertaken during the school year
immediately preceding your course start, and
should be based in a department most related
to your PGCE subject area, and within a UKbased state secondary school setting.
If you are unable to find experience that meets
any of this criteria, please contact admissions@
glos.ac.uk in the first instance, to discuss and
seek approval for any alternative arrangements.
This period of school experience will enable
you to:
• make an informed decision about your wish to enter training.
• demonstrate that you are fully prepared for the demands of teacher training.
• observe and work with children and school staff.
• participate appropriately in all aspects of the life of a school.
It is hoped that you will be able to assist with
small group activities, work with individual
pupils, prepare materials, complete routine
chores and assist with outside visits.
This school experience is a condition of offer
and must be completed. It is your responsibility
to contact a school of your choice and to seek
permission to make arrangements to attend.
You should be aware that schools are not under
any obligation to accept university applicants,
therefore please be tactful with your approach.
Guidance notes are provided with the form.
Please read and follow the instructions carefully.
If you have gained experience from more than
one school then please use a separate form for
each school.

Candidates should upload the completed
School Experience Report form to their student
portal BEFORE FRIDAY 29 JULY 2016. A letter
of introduction to the school is available for
your convenience. Please note that School
Experience Report forms must be completed
to a satisfactory standard. They must be well
presented, legible and FULLY completed.
Obtaining the Disclosure and Barring
Service Disclosure
The university is committed to the creation and
support of a balanced, inclusive and diverse
community. It also has a duty to ensure that
it makes safe recruitment and employment
decisions. The Disclosure and Barring Service
(DBS), previously known as the Criminal
Records Bureau (CRB), was set up by the Home
Office to improve access to these criminal
records. This is done via a Disclosure Check,
which is a process for gathering information
about someone’s spent and unspent criminal
convictions and other cautions, reprimands and
final warnings given by the Police. This service
enables organisations in the public, private and
voluntary sectors to make safer recruitment
decisions by identifying candidates who may
be unsuitable, or present a risk, for certain work,
especially when working with children and/or
adults classed as vulnerable. Having a criminal
record will not always prevent the individual
from studying at the university. This will depend
on the nature of the position, the circumstances
and the background to the offences.
DBS certificates are not normally transferrable
from another institution. However, applicants
can register their DBS certificate within 19 days
of the issue date for an update service at a cost
of £13 for a 12-months period. Further details
about the Update Service can be found at
www.gov.uk/dbs-update-service
The cost of an enhanced DBS verified through
the university is £52. This is paid through the
GBG Online website once the documents
have been verified at the University of
Gloucestershire. If you wish to verify your
documents at a post office then the cost is £58.
The Post Office will accept payments via cash
or debit/credit card. Please make a note of your
E-number as this will enable you to track the
process of your application on the DBS website
(https://secure.crbonline.gov.uk/enquiry/
enquirySearch.do).
Please note that applicants currently registered
to the DBS update service will not be charged
for a new DBS check. (Please notify the
Admissions Team when you have received your
DBS certificate.)
If you do not receive your enhanced disclosure
certificate within 60 days, please contact the
DBS to check the status of your disclosure. Full
details are on a separate sheet enclosed with
the DBS application form. Please note that your
offer is not made unconditional until the DBS
has finished processing your application for
clearance, which can take several weeks. It is
therefore advisable to get your DBS application
completed at the earliest possible date. The
University of Gloucestershire regrets that it
cannot accept liability for the costs of DBS
checks undertaken by prospective students.
Please note that applicants currently registered
to the DBS Update Service will not be charged
for a new DBS check provided it is an enhanced
disclosure. Further details about the Update
Service can be found at

Please note that applicants from the European
Union who have not lived for the past five or
more years in the UK will require additional
safeguarding checks. The DBS website has a list
of European countries and contact details as
to where certificates of good conduct/security
check can be obtained.

do not change and are the same for everyone.
