The undersigned manufacturer or distributor of tennis balls requests the United States Tennis
Association to test the following brand of tennis ball to determine whether such balls comply with
the Rules of Tennis Rule 3 and Appendix 1 for the year 2012.

In order to induce the USTA to perform the aforesaid service, the undersigned hereby agrees as
follows:
1) To pay, simultaneously with this application form, a testing fee of $1,900 for standard balls
2) Submit a test sample of four (4) dozen tennis balls for standard ball types
3) The tennis balls provided become the property of the USTA.
4) The undersigned further agrees that with respect to tennis balls used in USTA Sanctioned
National Championship Tournaments or events, it will not use the name of any club, tournament,
or event on boxes, cans, containers, or wrapping, or in any advertising or literature, except in
programs sold only at such USTA tournaments or events and its literature.
