DEFENCE HOME OWNERSHIP ASSISTANCE SCHEME—
SUBSIDY AUTHORISATION REQUEST FORM

1.Since its inception on 01 July 2008, the Defence Home Ownership Scheme (DHOAS) has
attracted a high level of interest from members. As at 30 December 2010, almost 12 500
members of the Australian Defence Force were receiving the DHOAS subsidy.
2.To be eligible for the Scheme, members must meet the eligibility provisions under the
Defence Home Ownership Assistance Scheme Act 2008 (DHOAS Act). Subsidy becomes
payable to members when all conditions of eligibility are met, a DHOAS loan is opened and
reported to Department of Veterans’ Affairs (DVA) by a Home Loan Provider, and the loan is
secured by a mortgage over an interest in land.
3.Effective from 01 February 2011, members who are seeking to draw down a DHOAS loan
will be required to complete and submit a Subsidy Authorisation Request Form before DVA
can authorise the payment of subsidy. The Subsidy Authorisation Request Form will provide a
member declaration that ensures that DVA is provided with concise and accurate information
about the use and purpose of the loan, details about the DHOAS property, and eligibility to
the lump sum payment (if applicable). The Declaration Form also ensures that members are
aware of specific conditions that apply to DHOAS benefits and the relevant sections under the
DHOAS Act that set out these conditions.
4.Important information about the Subsidy Authorisation Request Form will be incorporated in
the ‘About Your Subsidy’ hand-out DVA sends to members in conjunction with Subsidy
Approval Letters which are generated when a member applies for a DHOAS Subsidy
Certificate.
5.The Declaration Form can be completed online on the DHOAS website at
http://www.dhoas.gov.au. When completed, it can be printed and either mailed to DVA, or
sent to the administrator by email or fax.
6.Additional information about the Subsidy Authorisation Request Form will be posted on the
DHOAS website with the Declaration Form. Members can contact the DHOAS hotline on
