Big Heart Awards 2014 (to be presented in March, 2015)
The Visionary Mission of the Heart and Stroke Foundation is “Healthy lives free of heart
disease and stroke. Together we will make it happen.”
The Big Heart Awards were developed to recognize the accomplishments of Manitobans
who assist in the promotion and delivery of our mission statement. The Award is
presented to individuals, businesses or organizations that demonstrate leadership and
serve as role models in their community.
The purpose of the Big Heart Award is to recognize:
 Individuals, businesses and organizations who are making significant
contributions to achieving the mission of the Heart and Stroke Foundation in
Manitoba
 Individuals, businesses and organizations who are supporting the fundraising
initiatives of the Foundation and thereby facilitating the accomplishment of the
mission
Award Categories
Award categories include:
 Individual
 Business or Corporation
 Organization or Group
Award Criteria and Eligibility
Individual:
 This award is open to all volunteers in Manitoba.
 Volunteer work must have occurred for the benefit of the Heart and Stroke
Foundation in Manitoba.
 Consideration will be given to any volunteer who shows commitment, dedication,
and above all excellence in fulfillment of the Heart and Stroke Foundation
mission from the perspective of both the Foundation management and his or her
peers.
 Nominees must have performed such service on a wholly volunteer basis and not
for pay.
 Individuals of any age are eligible.
Individual accomplishments may have been achieved in any number of ways:
 implementation of an innovative idea, program or activity which has provincial
significance
 demonstrated leadership, integrity, vision and/or strategic ability
 demonstrations of significant and/or sustained contribution to the Heart and
Stroke Foundation in Manitoba
Business or Corporation:
This award recognizes the essential business/corporate support to programs and
activities of the Foundation.
This may have been achieved in any number of ways:
 any business or corporation that has been actively engaged with the Foundation
in activities that have benefited the Foundation.
 encouragement and support of employees to volunteer for the Foundation.
 are innovative in their use of resources (time, funds, people) towards the
Foundation’s mission.
Organization or Group:
This award recognizes the outstanding contribution that an organization has made
toward the programs and activities of the Foundation. Any organization or group that has
been actively engaged in activities that have benefited the Foundation and has
demonstrated outstanding support of the Foundation’s mission statement and which sets
an example for others to follow is eligible.
Nomination and Selection Process
The Nomination Form must be completed, signed and all supporting materials included.
The package will then be reviewed by the Selection Committee, which is made up of
three Board members plus the CEO.
The winners will be notified prior to the event and the awards will be presented at the
Heart and Stroke Foundation in Manitoba’s Annual Meeting.
Any staff person or volunteer can nominate an individual, business, corporation or group.
Employees of the Heart and Stroke Foundation in Manitoba are not eligible.

Nomination Package
In order to be considered, a nomination package must be delivered to the Heart and
Stroke Foundation in Manitoba and include the following:
1. Completed Nomination Form.
2. Statement of Nomination:
Using point form, please reply to the following questions:
i) What are the nominee’s outstanding achievements (including involvement outside of
the Foundation if possible)?
ii) How has the nominee contributed to the fulfillment of the Heart and Stroke
Foundation in Manitoba’s visionary mission?
iii) How has the nominee shown commitment and dedication to the Foundation?
3. Letter of Support:
Provide one letter of support for the nomination. Letters should be addressed to
the Selection Committee.
