TRANSCRIPT REQUESTS

Be sure to pick up a request form in Guidance for each application or scholarship you apply for. The first 3 are free and after that they are $2.

Submit your request 10 school days before the deadline to insure application and scholarship deadlines are met. Be sure to include any items that will accompany your transcript (recommendation letters from teachers, secondary school report, transcript fee, etc.) when you submit your transcript request. Attach a legal or large envelope addressed to the college admissions address or scholarship address with 3 stamps. Put

Forest Park High School’s name and address in the return address section of the envelope. Submit your transcript requests to Mrs. Phalan in the

Guidance office. Official transcripts will be sent directly to the organization for which the transcript is required.

THE COMMON APPLICATION

The Common Application has been a time saving tool for students and counselors for over 25 years. It is a general application form used by more than 250 colleges. The Application for Undergraduate Admission must be completed ONLY once. Photocopies may be sent to any number of participating colleges. The same is true for the School Report, Midyear

Report, and Teacher Evaluation forms. This procedure simplifies the college application process by saving time and eliminating unnecessary duplication of effort. Most member colleges accept the Common

Application electronically; a few require hardcopy. Visit www.commonapp.org

THE COMMON BLACK COLLEGE APPLICATION

This application will allow students to apply to all 34 historically black colleges and universities member institutions for a one time fee of $35.00.

S.A.E. Engineering Scholarships (Various scholarships awarded by corporations and colleges ranging from $500 to full tuition) To qualify, students must have at least a 3.5 GPA, plan on earning an engineering degree and meet the minimum SAT or ACT requirements of the donor. To apply, students should obtain the scholarship packet from Mrs. Bryant and complete the necessary application(s). The application deadlines are for December 1, 2007.

U.S. Army ROTC Scholarships. Go to www.armyrotc.com to apply. The deadline is December 1, 2007.

U.S. Air Force ROTC Scholarships. Go to www.afrotc.com/scholarships/hsschol/types.php to apply. The deadline is December 1,

2007.

Toyota Community Scholars awards 100 student scholarships valued at $10,000 and $20,000 over four years for outstanding academic achievement, leadership, and community service. Forest Park can nominate one student for this award. See Mrs. Bryant in the Career

Center by Dec. 10 if you would like to be nominated.

AXA Achievement Scholarship Program in association with U.S. News & World Report. $10,000 will be awarded to 52 students (one in each state, Washington, D.C., and Puerto Rico). $15,000 additional scholarships will be awarded to 10 national recipients. Winners include, but are not limited to, demonstrated: ability to achieve in college, ambition, drive and dedication, commitment to succeed and respect for self, family and community. United States JCI Senate Foundation Scholarship Program. Each year $1,000 grants are awarded graduating high school seniors who plan to continue their education at accredited post-secondary colleges, universities or vocational schools. The number of grants awarded annually will be at the discretion of the United States JCI Senate Foundation. Visit www.usjcisenate.org for an application.

The Elks National Foundation Most Valuable Student Competition offers 500 four-year scholarships, ranging from $1,000 per year to

$15,000 per year to seniors based on financial need, leadership, and scholarship. The Legacy Awards are one-year merit-based scholarships for children and grandchildren of Elks who are members in good standing. For more information and an application, visit www.elks.org

or get an application from the local Elks Lodge. The deadline is January 11, 2008.

The Sam Walton Community Scholarship is a non-renewable $1,000 award. For more information and to apply online, visit www.walmartfoundation.org

The McKelvey Foundation awards $40,000 in scholarships for young entrepreneurs. Students must own and operate a business for at least one year (non profits are okay). Visit www.mckelveyfoundation.org

Micron Technology Foundation, Inc. has $150,000 in renewable scholarships to students who have demonstrated excellence in academics and leadership. Students must have a combined Critical Reading or Writing and Math SAT score of at least 1350 or a composite ACT score of at least 30, have a 3.5 grade point average or higher, plan to major in Engineering (electrical, chemical, material science, or mechanical), physics, or chemistry and have demonstrated leadership in school, work, and/or extracurricular activities. Burger King Scholars Program. Awards are $1,000 each. Seniors must have a minimum grade point average of 2.5, work part-time an average of 15 hours per week, 40 weeks per year, are actively involved in community service activities, demonstrate financial need, and plan to enroll in an accredited two/four-year college or vocational/technical school. Apply online at www.bk.com/scholars. The deadline is February 15, 2008.

The National Co-Op Scholarship Program. This scholarship is for students who choose to pursue a college cooperative education program. Students must have a grade point average of 3.5 or better. Visit www.co-op.edu to apply and receive more information. The deadline is February 15, 2008.

NVTA (Northern VA Theater Alliance) has two $1,000 scholarships. One scholarship will be awarded in theater performance and one in technical theatre to high school seniors. Applicants must have a GPA of 2.5 or higher, submit one letter of recommendation as specified in the application, as well as several other selection criteria components. Applications are available in the Career Center.

Applications (including a picture) must be postmarked no later than February 16, 2008. Applicants for the technical scholarship should call (as soon as possible-do not wait until February 16) Anne Ridgeway at (703) 330-2787 to receive pertinent specifications for completing your design.

The Society of the Cincinnati in the State of Virginia will award four $3,000 scholarships to seniors who have been admitted to a fouryear college or university in Virginia. Students must write an essay about a historical document that played an important role in the establishment of our nation. This year’s topic is “The Declaration of Independence”. Applications are in the Career Center. The deadline is February 22, 2008.

Johnson & Wales University National High School Chef of the Year-Step Up to the Plate. More than $600,000 in scholarships will be awarded to this year’s Chef of the Year winners. Your entry would win a full scholarship to Johnson & Wales University, the worldrenowned culinary school. Stop by the Career Center for an entry form. The deadline is March 3, 2008.

American Legion High School Oratorical Contest: Students demonstrate their knowledge of the U.S. Constitution and its applications as well as their oratorical skills. Generous scholarships and recognition by the Freedom Foundation at Valley Forge and the American

Academy of Achievement. Visit www.legion.org/orator.htm for more information. The deadline is March 3, 2008.

Montclair Lions Club William Alan Chapman Senior Memorial Scholarship. Two scholarships in the amounts of $2,000 and $1,500 will be awarded to recognize the local community service efforts of graduating seniors residing in the Montclair and surrounding area.

To apply, students must complete the required application, obtain two letters of recommendation, and write a 400-word essay on the topic “The Importance of Community Service at the High School Level.” An interview may also be required. Applications are available in the Career Center, and they must be completed and submitted to Mrs. Bryant in the Career Center before April 11, 2008.

Effie Lee Nelson Speiden Scholarship sponsored by the Manassas Chapter, Virginia Division United Daughters of the Confederacy. A one-time $1,500 scholarship is available to a young woman attending a Virginia college. The General and Virginia Division

Scholarship programs offer several multi-year scholarships. Eligibility includes proof of Confederate ancestry and a minimum 3.0 grade point average. Virginia Division Scholarships must be provided to the Chapter’s 2 nd

VP no later then April 17, 2008. Applications for the Effie Lee Nelson Speiden Scholarship must be provided to the 2 nd Vice

President, Manassas Chapter, no later than May 1, 2008. Applications for the Effie Lee Nelson Speiden Scholarship are available in the Career Center.

The Gloria Wille Bell and Carlos R. Bell Scholarship Program for the University of Michigan, Ann Arbor, Michigan. During their lifetimes. Gloria Wille Bell and Carlos R. Bell, long time residents of Gloucester County, Virginia, created a trust fund to provide scholarship support to help students pursuing degrees in engineering, math, physics, computer science or another related scientific filed at the University of Michigan. The scholarship support is based on academic achievement rather than financial need. The program will provide $80,000 over four years of undergraduate study to students selected from Virginia, with a preference given to residents of the Gloucester, VA area. VFW Voice of Democracy Scholarships: Up to $30,000 awarded. To enter, students must make a 3-5 minute tape on the theme,

“Freedom’s Challenge.” More information is available by contacting VFW National Headquarters or your local VFW Post. You can also visit http://vfw.org.

College Specific Scholarships

The following schools have sent information about scholarship opportunities:

Virginia Tech Pamplin Scholars Program consists of two scholarships: the Pamplin Leader Award and the Pamplin Scholar Award.

The Pamplin Leader Award is a one year academic merit-based $1,000 scholarship. The award is presented annually to one qualified student from each public high school in Virginia. The Pamplin Scholar Award is chosen from the pool of Pamplin Leader Awardees.

A student awarded the Pamplin Leader is automatically qualified to apply for the Pamplin Scholar Award in the fall semester of their first year at VT. Students must have a 3.75 GPA or higher and/or be ranked in the top 10% of his/her class. This award is open to students in all majors. Visit http://www.univhonors.vt.edu/html/scholarships.html

The University of Tampa has writing scholarships for students pursuing a degree in English or writing of $2,500 per year. Students may also be eligible to receive additional merit or need-based financial aid.

Calvin College – The Howard Hughes Medical Institute Scholarship is awarded to top students who intend to pursue a career in research and/or post-secondary education in biology or a related field of chemistry, biochemistry, physics or math. Awards of $2,500 are given. Application deadline is January 31, 2008.

Johnson & Wales University – Chancellor’s Scholarship awards $7,500 to full tuition renewable scholarships for high academic achievers. Students must be accepted into the school’s honors program.

Johnson & Wales University – Presidential Scholarship - $5,000 annually.

Ohio Wesleyan University – Schimmel Scholarship. This award is for full tuition, room and board to a freshman woman who ranks near the top of her class with honors/advanced classes. Complete school application by February 1 to be assured of consideration for this scholarship.

Ohio Wesleyan University – The Phillip J. and Nancy LaPorte Meek Community Service and Leadership Scholarships offers awards ranging from $4,000 to $10,000 to students with an outstanding record of leadership demonstrated through commitment to community service.

Ohio Wesleyan University – Helen Crider Smith Women as Leaders Scholarship is a full tuition award to a freshman woman with an outstanding record of academic achievement and demonstrated leadership.

The University of Maryland-College Park has dance scholarships for gifted and talented freshman dance majors. For application forms, contact the Scholarship Committee, Department of Dance, University of Maryland, College Park, MD 20742, 301-405-3180, or dancedept@umd.edu. Applications arte due February 17, 2008; auditions are on March 2, 2008.

Ithaca College – Martin Luther King Jr. Scholar Program. For exceptional freshman from ethnic or racial backgrounds. Each MLK scholar receives up to full tuition in aid, with a minimum merit-based scholarship of $17,000. Stop by the Career Center for more information.

University of Connecticut Merit Scholarship – from full-tuition scholarships to research fellowships

Davis & Elkins College – The Art Program. Scholarships range from $500 to $3,000. Stop by the Career Center for more information.

Ithaca College Park Scholar Awards (all expenses paid).

Appalachian State University. Over two million dollars are offered to incoming students each year who demonstrate the ability to complete highly competitive college-level work. The application deadline is November 15, 2007; go to www.firstconnections.appstate.edu to apply. Academic Promise, Phi Theta Kappa, and College of Education scholarships have a

March 3, 2008, deadline.

Davidson College in North Carolina -The Davidson Scholars Program and the Davidson College Bryan Scholarship are for top senior scholar athletes.

Roanoke College. Receive up to $137,000 over four years with Roanoke College Scholars Program. Application deadlines are:

November 9 & 16, February 15, 18, & 22, and March 14 and 17. Go to www.roanoke.edu/scholars to apply. Application deadlines are

October 8 for the Fall competitions and January 15 for the Spring competitions.
