Бизюков Item. The brief characteristic of the organization In branch experts estimate a condition differently. Officials speak that in public health services the big rise. For example, they count a parameter of well-being excess of the previous and scheduled consolidated charges in public health services - in 2006 they have made 15,8 billion roubles. For two years (2005-2006) this parameter has increased for 33,7 %. Counting upon one inhabitant charges in public health services have increased from 4141, 8 roubles up to 5568,3 in one year. Besides equipment by the diagnostic equipment for this period from the federal budget within the framework of realization of the program "Health" has made 245 million roubles (453 units of equipment) so by results of this job of 41 % ЛПУ are harmonized with standards of equipment by the diagnostic equipment. Experts from among ordinary workers and trade-union leaders mark a high tension in labour attitudes{*relations*} because of increase in intensity of job and change of conditions of a payment. In a course of realization of the project "Health" the set of new problems{*tasks*} with which the medical staff did not work earlier has been put. On the first place the trade union on a urgency had problem{*task*} to provide the fair and proved payment медработникам: « With realization of the national priority project "Health" long-term hopes of physicians for improvement of the position have started to come true, but for the present there are not enough workers by whom the level of wages » is essentially raised{*increased*}. Only by the end of 2006 agreements between ЛПУ and territorial bodies of public health services (mountains and райздравами) about the order of financing and distribution of wages have been achieved. According to Department of public health care of Kemerovo area (ДОЗН) « the average wages of doctors have increased in 2,1 times, at average medical staff for 34,2 % ». However in the statistical directory and on the data соцопроса lead{*carried out*} in May - June, 2007, it was not possible to find acknowledgement{*confirmations*} to these data (see the comparative data of statistics and on соцопросу in the application). Certainly, the level of a payment has increased, but conditions of this increase for medical staff appeared new and not always quality of services rendered in public health services concerned. For example, in area some programs of free-of-charge health services (pensioners than 70 years are more senior and children - in 2007 on them it is directed 200 million roubles from the regional budget) work, but conditions of reception of this of services are those, that « the minimal budget » these programs does not allow to make properly treatment. There is a latent discontent with these conditions in the environment of medical staff and patients. The regional organization of trade union of workers of public health services (ПРЗ) unites the primary organizations of establishments of public health services of a regional and federal level, territorial (city and regional) the trade-union organizations. In total by the beginning of 2007 in structure of Kemerovo regional organization ПРЗ 258 primary organizations were totaled. Regional organization ПРЗ directly submits and reports Central Committee PRZ at a federal level in Moscow. Concerning attitudes{*relations*} and subordination with Federation of the Trade-union Organizations of Kuzbass (ФПОК), heads hold the opinion, that now at them a waiting attitude: « After change of chairman of Federation we managed to lead{*carry out*} two large decisions: one is a displacement of the head by system of trade-union training; the second is to advance our offers in the Kuzbass agreement ». Number of members ПРЗ has made 50,5 thousand чел. (the data for May, 2007) For the period since 2003 the number of members of trade union was reduced to 5 %. This period is characterized by re-structuring in branch, therefore under pressure of this reason basically there was a leaving workers, and, hence, and trade-union membership was reduced. In structure of members of trade unions the biggest share of average medical staff. In 2006 the National project "Health" was realized, it has caused significant changes in financing and supply by the equipment and medicines, but organizational transformations "were in a fever" the personnel because of set of defects and omissions regarding payment and working conditions of workers of public health services. As a problem developed last three years experts sound leaving{*care*} from public health services of young qualified personnel, including owing to disagreement with the bureaucracy which is carrying out the project "Health". With ДОЗН Kemerovo area Обком of trade unions of workers of public health services develops formally. For the period of research two times the head of Department has exchanged, it and has caused that attitudes{*relations*} on regulation of the labour rights in branch at a level of acceptance of the Branch agreement have not developed. Thus chairman is invited to all significant sessions and decisions of Department обкома. Therefore it is impossible to say, that attitudes{*relations*} no, but also as it was not possible to adjust real productive interaction. The text of the Branch agreement three years as is ready, for it{*him*} very for a long time and closely{*attentively*} prepared in обкоме ПРЗ, generalizing a significant stream of offers from первиных trade-union organizations (ППО) but to coordinate it{*him*} with ДОЗНом up to the end it was not possible. Leaders of trade union put a problem{*task*} of signing of the Regional branch agreement today as paramount: « Signing of the Branch agreement is very important, especial for city town committees ПРЗ where there was no attitude{*relation*} with municipal authority ». Now the trade union strongly supports замгубернатора: « I would like to note, that the role of heads of the trade-union organizations should raise …. The working group together with representatives the regional organization of trade union has developed a package of documents on a payment. We have brought in such position to the law of the Kemerovo area: all extra charges are adjusted by the collective agreement. And to conclude the collective agreement, it is necessary to have the trade-union organization. The conclusion arises: in each medical establishment there should be a trade-union organization, she should be enough strong to protect interests of workers ». For 2006 it was possible to develop typical position about payment and stimulation of work of medical workers within the framework of job on ОМС, in this job participated обком trade unions together with department, territorial fund OMS, the Regional информационно-analytical center, the insurance medical companies. Between trade union and the employer in public health services always there were intense attitudes{*relations*} concerning distribution of the means received on paid services, step in decrease{*reduction*} in heat of contradictions, обком counts also development{*manufacture*} together with department of typical position about rendering of paid services to the population. On the lead{*carried out*} checks regional ДОЗНа in first half of 2007 it was revealed, that the majority of heads ЛПУ incomes in tens times exceeding have average wages of doctors. Such position by Department is recognized as unsatisfactory, therefore замгубернатора areas on social questions one has precisely formulated a position of a management{*manual*} on this question « to three (the parity{*ratio*} of the salary of the doctor and the head physician), is base factor, on his{*its*} increase the rating of establishment, results of financial and economic activity, social support of collective » can affect. To the beginning of second half-year 2007 ДОЗН and обком ПРЗ have developed « Time typical position about payment and awarding of work of heads of medical institutions ». Into problems{*tasks*} обкома ПРЗ now enters to generalize offers from workers of six medical institutions who participate in the pilot project on realization of this position and to finish job above this document for general introduction. The basic problems of the regional trade-union organization last years were: 1. The biggest problem of 2006 was adjustment of system of a payment within the framework of realization нацпроекта "Health"; 2. обком "pressed through" both a question of certification of workplaces and itself participated in this process to lower danger and harm of working conditions in hospitals. 3. An establishment of attitudes{*relations*} with regional department of public health care; 4. Organizational construction of the regional organization, formation of regional, city and regional committees of trade union, maintenance of membership; 5. The organization of training of leaders and active workers ПРЗ in connection with changes in ТК, realization of demographic politics (distribution of means of " patrimonial certificates »), realization нацпроекта "Health"; 6. Creation of " Youth advice{*council*} » for attraction and deduction in trade union. The description of the trade-union organization History ПРЗ takes the beginning in 1944. From archival documents experts ПРЗ managed to establish sequence of change of chairmen of trade union and substantially to estimate changes in directions of job. If to try to assess having place changes in job обкома ПРЗ alongside with traditional directions of job on protection of the labour rights of workers, labour safeties, improvements of a material - conditions of life of work, distribution of the best practices, organizational and cultural job, now have appeared new directions of job, and the some people were filled with the new maintenance{*contents*}. It is necessary to attribute{*relate*} job to new directions of job on formation of social partnership in branches, creation and participation in job of Associations of the trade-union organizations of workers of budgetary sphere, creation of new funds in the budget обкома, the organization of financial service of the primary organizations. Development of system of training of trade-union leaders, development of system of the legal control and consulting, purposeful job is possible to rank as development of already existing directions of job on prophylactic medical examination and improvement of medical staff and their children, job with youth, cultural and sports job. To side benefits and guarantees of members of trade union concern material aid from обкома trade union, with 2000 for 2006 she is rendered more than 300 workers of public health services; the Fund of Solidarity created in 1990, is used for material support injured from acts of nature, fires and charity, and from Fund of social support under the decision of presidium is given more than 300 interest-free loans for a total sum 3 million roubles. Number of members ПРЗ has made, as was already spoken, about 50 thousand чел. Number of trade-union activists - 5633 person, percent{*interest*} of trade-union membership - 68,4 % (the data on the middle of 2007). Dynamics{*changes*} of membership for last three years negative, but membership in trade union is under control Обкома ПРЗ. The reasons of such position became reorganization in public health services, purposeful politics of administration of some establishments of public health services, politics of expression of trade union; change of the proprietor also conducts to losses in trade-union membership; transfer of trade-union organizations of children's medical institutions to other trade union (educations). Among priority directions of job обкома constantly there is a theme of motivation of trade-union membership. For maintenance of information base and formation of a feedback with rank-and-file members and trade-union leaders in May, 2007 sociological research (results look in the application) has been lead{*has been carried out*}. About 60 person are elective members обкома ПРЗ. For maintenance of stability of numerical structure PRZ in обкоме it is developed and the program on amplification{*strengthening*} of motivation of trade-union membership for 2006-2008 takes root. Paramount problems{*tasks*} in this direction a management{*manual*} обкома see in involving youth in numbers{*lines*} of trade union, through activization of trade-union activity in educational institutions and jobs with young experts, through maintenance of the status of the young expert - carrying out of professional competitions and involving during a public life of collective (in plans for 2008 there is a project of monitoring of position of students in Медакадемии). Besides informing and training of trade-union active workers sees to a management{*manual*} обкома as an obligatory component of this process.
