Vocelli’s Pizza Day Order Form
St. Pius X will have six pizza days throughout the school year where students can
order Vocelli’s cheese pizza for $1.50 per slice.
 Fill out the form below by circling the number of slices your student will be
ordering. Please fill out ONE ORDER FORM PER STUDENT.
**ATTENTION: IMPORTANT COMPLETION PROCEDURE**
 Please send ONE CHECK PER STUDENT made out to St. Pius X Regional
School. No cash please. Do NOT combine payment with other payments.
For example, 2 students in a family = 2 order forms + 2 checks.
 Enclose the order form and check in an envelope clearly marked “Pizza Day”
with your child’s name and homeroom. Please return to your child’s teacher by
Friday, September 25, 2015. No late orders can be accepted.
