Preparation of the Form 48 (Academic Plan)
The Planned Academic Plan, Form 48, is a mandatory document prepared by the
individual (cadet) to project coursework required to meet graduation requirements and
ensure being commissioned on schedule and to compete for or activate a scholarship.
This plan should include summer sessions.
- If the course titles of electives are not known, list "Elective" and enter the number of
proposed credit hours.

Use the following instructions for completion of the Academic Plan
Section I - Administrative Data.
Block 1 - Self-explanatory; Last Name, First Name, Middle Initial
Block 2 (Academic Institution) - Enter the institution from which the cadet will receive
their degree and the AFROTC unit they are attending. (Virginia Tech/Det 875)
Block 3 (Academic Major) - - Enter one major field of study (ex. Industrial Systems
Engineering). Include alpha-numeric AFROTC Major Code (ex. 4LYY).
- Minors and double majors CANNOT be reflected on the academic plan if doing such
will impose on satisfying program objectives (receiving a baccalaureate degree). Cadets
are authorized to pursue a double major or obtain a minor only upon receiving an
academic plan signed by an academic advisor establishing a valid date of commission
for ONE major. Once this is complete, the cadet may add additional hours to already
existing certified academic plan to meet the requirements of a minor or a second major.
Adding additional hours must not place the cadet at risk of taking too many hours a term
or require unnecessary terms in completed status; or, change the already established
commissioning date.
Block 4 (Institutional Official Review).
- Each cadet’s advisor will review and sign the completed Form 48 with student
BEFORE the APAS term counseling review. The student's college or university
academic advisor must approve the academic plan by signing IN INK on the front page
(Initial Review Section) and on the current semester.
- If the advisor will not review the plan, contact your instructor.
Include type of degree (B.A. or B.S.), date of graduation, date of commissioning, and
tentative field training date (usually summer after your sophomore year). Engineering or
architecture majors should plan a 5-year course of study.
Cadets skip the comment section -- instructor use only.

Section II - Academic Plan.
- Enter the number of hours required to obtain your degree (bottom right corner of front
page).
- List all projected course work term-by-term to complete the academic major
requirements.
Term.
- Enter the term designation such as Fall, Spring, Summer, and year (i.e. 2003) for each
Academic term projected.
Course Department.
Enter the Department for the course (i.e. MATH)
Course Number.
- Enter the appropriate information as listed in the school catalog (i.e. 1274)
Course Title.
- Provide short, abbreviated course titles when possible.
For example: "General Psychology" could be "Gen Psy;" "Inorganic Medical Chemistry"
could be "Inorg Med Chem;" etc.).
- Use generic terms when you are unsure of specific course work, such as “Elective”,
“Technical Elective”, or “Core Requirement”
Hours.
- Enter the number of credit hours for the specific class as specified for the course in the
college catalog, regardless if the credit hours directly apply to the major.
- Once the credit hours for each course are annotated, your instructor will add all hours
under the column “Hours” and place the sum value in the “Term Credit Hours” block.
- When the semester is completed your instructor will enter your credit hours towards
your degree in the “Total Credit Hours”.
