Community Advisory Task Force Members Meetings The charge of the Community Advisory Task Force is to provide strategic leadership and to guide decision-making as we work to advance community and patient engagement, integration across the continuum of care and population health improvement. This includes: Advising on priorities, providing input on the development of strategies and identifying resources/support necessary to implement strategies and achieve the MAHM goals described above Providing guidance on communications, consumer engagement and culturally appropriate outreach Discussing strategies for integration of services across care delivery settings and alignment of approaches across providers/payers/communities Providing support on opportunities to leverage or build upon existing investments in technical infrastructure, including electronic health records and other health information technologies Offering advice and assistance in educating communities on new models of care delivery and strategies for patient engagement Developing strategies to identify and share best practices, success stories and evidence of local impact Leading discussions to identify approaches for sustainability of activities and alignment with related efforts The specific charge of the Community Advisory Task Force will evolve as the MAHM project is implemented, but will continue to focus on using MAHM activities to achieve the Triple Aim of improving population health, improving the experience of care for patients and decreasing health care costs. The Task Force may establish short-term or ad hoc technical groups to advise on specific aspects of its work, as needed.
