4. Update from Documentation Workgroup and Structured Documents Work Group (SDWG)
a. The Documentation work group reviewed the consolidated IG to see if it meets
the requirements enumerated in the Merged Publication Considerations document.
Their assessment of the requirements is summarized in the requirements table.
Pete noted the requirements are actually recommendations.

6. Plan of Care
a. CCD:
i. Single template for multiple acts
1. A plan of care activity (templateId 2.16.840.1.113883.10.20.1.25)
SHALL be represented with Act, Encounter, Observation, Procedure,
SubstanceAdministration, or Supply.
2. Restricts moodCode use
b. HITSP:
i. C83-[CT-124-3] The Plan of Care Section MAY include entries conforming
to the Medication, Immunization, Encounter, and Procedure modules to
provide information about the intended care plan
ii. Determined that HITSP does not place additional restrictions on the CCD
Plan of Care section.
c. IHE:
i. Determined that IHE has a Plan of Care section in the PCC Technical
Framework Supplement

ii. Recognized that IHE’s Plan of Care section takes the CCD Plan of Care
section and applies further constraints.
Decision: The harmonization analysis for the Plan of Care entries will consist of a
comparison between CCD and IHE since HITSP doesn’t add any specific
constraints.
7. Social History
a. CCD:
i. CONF-CCD-243: The value for “Observation / code” in a social history
observation SHOULD be selected from LOINC
(codeSystem2.16.840.1.113883.6.1) or SNOMED CT
(codeSystem2.16.840.1.113883.6.96), or MAY be selected from
ValueSet2.16.840.1.113883.1.11.20.18
SocialHistoryTypeCodeSTATIC20061017.
ii. CONF-CCD-244: Observation / value can be any datatype. Where
Observation / value is a physical quantity, the unit of measure SHALL be
expressed using a valid Unified Code for Units of Measure (UCUM)
expression.
b. HITSP:
Decisions:
i. Decided to bind entry level vocabulary at the document level. Recognized
need to make format and methodology for such binding publically available.
ii. Agreed upon the following:
1. cda: effectiveTime = SHOULD
2. cda: code/@code = SHOULD and SHOULD
2.16.840.1.113883.3.88.12.80.60 STATIC
3. cda: text = SHOULD
4. cda: value = SHOULD
c. IHE:
i. Allowable <code> values
ii. Explicitly excludes <interpretationCode>, <methodCode>, and
<targetSiteCode> elements
Discussion: It was suggested that text be added to explain why the three
aforementioned elements should not be included
8. Appendix Change Tracking
a. Brett indicated that there was a request to create a change tracking log for the
consolidated IG.
b. Robin Barnes from USHIK indicated that this would be very helpful for
implementers familiar with the particular previous templates.
Decision: Decided that Brett will assemble a proposal based on the Plan of Care and Social
History Observation sections to share with the Harmonization group.
