I use the word “psychiatrists” to remain within my position in Montgomery County’s Behavioral Health and Crisis Services, “Chief Psychiatrist,” but I’m well aware that the County has many outstanding clinicians of other professions and would welcome suggestions from any of them on how this Comprehensive listing could be improved. It is important that all of us utilize the DSM definitions. There are conditions, however, of interests to mental health clinicians that are not in DSM-5. For some of them, we provide a prototype definition in this edition of the Comprehensive. We will provide more in future editions of the Comprehensive. In terms of nomenclature and coding, this is a confusing time for American psychiatry with the starting date of using DSM-5 varying among payers varying among government agencies. A second confusion is when will we, if ever, switch from ICD-9-CM to ICD-10-CM. All we can say now is that the switch cannot take place before 1 October 2015. Sometime during 2015, it will become clearer as top the switch. Since the US Congress has become involved, the timing remains uncertain. An example that highlights the confusion: Disruptive mood dysregulation disorder. DSM-5 Task Force decided to add this new diagnosis, which is not in ICD-9-CM or ICD-10-CM, to DSM-5. Yet, they had to select a code in ICD-9-CM and in ICD-10-CM. The DSM-5 Task Force is not allowed to create a new code, has to select a code already in those two books. So, “296.99, Other specific episodic mood disorder” was selected. Thus, 296.99 has at least two names. A psychiatrist puts in 296.99 and selects the name she wants, “Other specific episodic mood disorder” or “Disruptive mood dysregulation disorder.” Same problem with ICD-10-CM, so “F34.8 Other persistent mood disorder” was selected by the DSM-5 Task 2 Force. This means that F34.8 has two names as far as DSM-5 followers are concerned. The APA has gone to the National Center for Health Statistics, authors of the ICD-CMs, and asked that “Disruptive mood dysregulation disorder” and other additions and changes introduced by DSM-5 be added to ICD-9-CM and ICD-10-CM. At this time, we do not know their decision. As the Multiaxial System gets phased out, it should be easier to encourage clinicians to make use of V-Codes. V-Codes were somewhat preempt by Axis IV. With the removal of Axis IV, hopefully, we will see more frequent use of V-Codes, bringing psychosocial situations into our diagnosing in a way that allows the collection of data. Axis IV lack of coding left no convenient way to collect that information.. Concepts Often Mentioned in DSM Discussions. “Sensitivity” = the system being used will correctly identify all the people with this particular condition. “Specificity” = the system being used will NOT incorrectly identify someone with a particular condition. It can be very difficult to achieve both high sensitivity and high specificity at the same time. High sensitivity can lead to many false positives, that is, low specificity. High specificity can lead to many false negatives or low sensitivity. A goal of all seven DSMs was to increase the reliability of the diagnosis system through defining the disorders. “Reliability” only means consistency between clinicians as to use of the terms. DSM-I and DSM-II used a brief description [“prototype matching”] of each condition in an attempt to achieve reliability. DSM-III, IIIR, IV, IV-TR, and 5 used criteria sets as a way to achieve reliability. One of the measures of reliability is the Kappa coefficient, a numerical index of inter-rater agreement, “1.00” being perfect, “0.00” meaning that the system does not have any increased reliability over chance. DSM-5 thinking: Kappa > 08, miraculous 3 Kappa 0.6 – 0.8, great Kappa 0.4- 0.8, realistic goal Kappa 0.2- 0.4, acceptable Kappa <0.2, unacceptable “Validity” is a term used to reflect whether the system measures what the system is intended to measure. Discussions of validity can rapidly become obtuse. “Phenotype” = the result of an interaction between a person’s genetic manifestations and their environment. There are no phenotypes in DSM-5. There are a number of key decisions that will need to be addressed: Timing of the publications There have been 7 to 19 years between the DSMs in the past. This is a problem in two ways: 1] Diagnostic changes in psychiatry should be based on the timing of the scientific developments, not on publications dates. 2] Even though DSMs have been quite similar since DSM-III [1980], the massive number of minor changes with each edition has meant that clinicians are faced with having to learn hundreds of minor details with each DSM, whereas a few changes each year would be more clinician-friendly. As a result of the above, there has been talk of future DSMs, being published much more frequently and titled "DSM-5.1," "DSM-5.2," and so forth. This concept has been tied to making the DSMs of the future a “living document.” Diagnostic by Criteria Sets or by Prototype Matching DSM-I [1952] and DSM-II [1968] defined conditions the way most of medicine does, prototype matching, a paragraph describing the key features of the conditions, for example as seen in Merck Manual. DSM-III [1980], DSM-IIIR [1987], DSM-IV [1994], and DSM-5 [2013] used criteria set because it was felt that criteria sets would improve the reliability of the definitions, would improve the chances that two clinicians examining the same patient would arrive at the same diagnosis. There is some talk of future ICDCMs having definitions of disorders that would be in the form of prototype matching. If that comes to pass, then it may be appealing for psychiatry to return to prototype matching. If so, there would be a strong wish to select prototype-matching wording that would achieve the reliability of the criteria set approach. DSM Decision Making What should be DSM decision making process? In the development of recent DSMs, much of the decision-making power had resided in whoever held the position of Director of Division of Research of the APA Headquarters. Harold Pincus and Darrel Regier have carried out their role as head of the Research office very admirably. What has been missing is APA governance into DSM content issues until nearly the end. In the future, APA governance should participate in content decisions from the beginning. 168 Can Future DSMs Facilitate Identifying Etiologies? Can Future DSMs Indicate Treatment Choices? Can a future DSM better facilitate the uncovering of etiological agents than has been true of recent DSMs? Outside sleep conditions, the only new etiological agent in DSM-5 after 18 years of using DSM-IV is Lewy body neurocognitive disorder. A dozen years ago, Steve Hyman, past Director of NIMH highlighted the issue: Steve Hyman, Past-Director of NIMH: • “Despite these successes [of the DSMs], there are clear problems and unresolved controversies related to DSM-IV-TR, the most recent version of DSM. If a relative strength of DSM is its focus on reliability, a fundamental weakness lies in the problems related to validity. Not only persisting but looming larger is the question of whether DSM-IV-TR truly carves nature at the joints – that is, whether the entities described in the manual are truly ‘natural kinds’ and not arbitrary chimeras. In reifying DSM-IV-TR diagnoses, one increases the risk that science will get stuck, and the very studies that are needed to better define phenotypes are held back. Except for IQ tests to diagnose mental retardation and polysomnography to diagnose sleep disorders [polysomnography was inexplicitly excluded from DSM-IV-TR criteria sets], diagnostic tests for mental disorders do not yet exists.” More recently, 2011, Hyman sent one of us [RP] the following email, using, R = Revision: • “The most important goal is to help the APA get out of the DSM-III-R-R-R rut without blowing up clinical practice. Whatever it takes.” Of course, DSM-5 did not get American psychiatry out of the rut. DSM-5 is the third “R” in his quote. Getting out of the rut might be achieved by moving closer to the style of diagnosing in the rest of medicine. Approaches to getting unstuck might include: 1] Not totally focused on defining DSM’s psychopathology by syndromes, but focus more on defining signs and championing the concept that signs alone can be satisfactory identification of a person’s psychiatric condition. Recent DSMs have structured all psychopathology into syndromes. Where the etiology is known, this can lead to a stretch of the word, “syndrome.” For example two of fourteen signs constitute a “syndrome” for inhalant use disorder. Mental trauma can be a contributing factor to almost every Disorder in DSM-5, yet there are only about a dozen disorders that include mental trauma in the definition. Restated, where the etiology is known, such as the substance-related disorders, the manifestations as to somatic signs can sometimes reach the meaning of “syndrome” -- but not the mental signs. 169 2] Not including “clinically significant distress or disability” in the definition. The rest of medicine does not separate the definition of a condition by with “clinically significant distress or disability.” Psychiatry should do the same. “Laceration,” “contusion,” and “headache” can all be defined without including the phrase, “clinically significant distress or disability.” Psychiatry should do the same, should define “delusion,” “dysphoric,” and “phobia” without trying to build into the definition whether it is clinically significant. 3] Including treatment results in conceptualizing psychopathology. DSM-IV and DSM-5 avoided any criteria that reflected treatment results. DSM-IIR allowed only one criterion set to mention treatment results. Allowing treatment results to be part of the definition of psychopathology might help uncover etiologies. Even if not “etiological,” a diagnostic system closer to treatment results could move the DSMs closer to the needs of clinicians. 4] Working harder to establish hierarchies of diagnoses. It seem quite unlikely that the large number of patients who end up with two or more psychiatric disorders using a recent DSM approach to psychopathology is the way nature has divided up psychopathology. Sometime in the future, when we have a better understanding of course of conditions, many patients now diagnosed with several conditions will be diagnosed with only one. It remains important than County psychiatrists adhere to DSM-5’s definitions. We do not want an idiosyncratic language. At the same time, it is important to think through how we can become more like the rest of medicine to where etiologies can be identified or to where the diagnostic system indicates which treatments would be indicated..
