However clients do not always find it possible (or know it is possible) to obtain supplies when required. The Department of Health recommends the development of Emergency Hormonal Contraception (EHC) services and suggests that pharmacies are an ideal setting for this type of service because “pharmacies are an additional access route for emergency contraception, particularly at weekends and bank holidays when other services might not be available”.1 The Department of Health also recognises that ‘pharmacists can provide easy access to EHC’.2 Schemes that provide EHC through community pharmacies have been developed elsewhere in the UK and evaluation of these schemes have showed them to be successful.3,4,5 In light of all the evidence to support the provision of EHC through community pharmacies via patient group direction, Chorley & South Ribble Primary Care Trust has developed its own scheme in order to further improve access to EHC services within the PCT’s catchment area. 1. Int J Pharm Pract 2002:10(suppl):R83 EHC Service Specification 140905 © Chorley & South Ribble PCT 2005 2 Page of 20 AIMS OF THE SERVICE The service is designed to meet the following aims: To increase access to emergency contraception (in terms of location, available times, time to be seen and type of health professional supplying) To increase knowledge of emergency contraception contraception amongst clients and healthcare professionals To reduce the rate of unwanted pregnancies within the boundaries of Chorley & South Ribble Primary Care Trust, particularly in teenagers To signpost hard to reach clients, especially young people, into mainstream contraceptive services To increase awareness of the link between unprotected sex and sexually transmitted diseases To increase partnership working and improved communication between pharmacists and other healthcare professionals To enhance pharmacists’ professional practice EHC Service Specification 140905 © Chorley & South Ribble PCT 2005 and mainstream 3 Page of 20 SERVICE DETAILS DETAILS OF CLIENTS THAT CAN ACCESS THIS SCHEME This service will be offered to any female (regardless of age) presenting at an approved community pharmacy where an accredited pharmacist is available. The client must be registered with a GP practising within Chorley & South Ribble or Preston PCT. Clients who are not registered with a GP from these two PCTs, should be made aware of the other routes of supply (eg via their own GP, Community Contraceptive or GU Medicine clinics, or TALKWIZE – contact details at the back) and referred. These clients may opt to purchase the over the counter available EHC if they wish and the pharmacist should handle this in the usual way. There are no age restrictions for clients accessing this service, however, the supply procedure (see below) may result in some young people being referred on because of issues regarding competence to consent to treatment. The pharmacist may also exclude some other individuals e.g. due to other medical problems. All these clients should be urgently referred as above. The service will be free of charge to eligible clients and is funded by Chorley & South Ribble Primary Care Trust. EHC Service Specification 140905 © Chorley & South Ribble PCT 2005 4 Page of 20 HOW DO PHARMACISTS SIGN UP TO PROVIDE THIS SERVICE? The provision of the service can be divided into six main areas, which are described below: 1. Accreditation Prior to being authorised every pharmacist must: Complete the pre-workshop CPPE task and attend the PCT workshop Complete the CPPE distance learning pack on Emergency Hormonal Contraception (the certificate will be required as evidence) Complete the PCT’s pack on Client Group Directions Every pharmacist will be responsible for training all staff in the pharmacy(ies) where they work. A standard operating procedure (SOP) must be written to cover this enhanced service. Pharmacists are also responsible for maintaining up to date clinical knowledge appropriate to this scheme by undertaking any necessary CPD. On occasion the PCT may run additional mandatory training events, e.g. when evidence from evaluation or feedback suggests that there are widespread learning needs. All authorised pharmacists must attend these events in order to continue in the scheme. It is the responsibility of each authorised contractor/pharmacist to ensure that this enhanced service is covered by their professional indemnity provider. 2. Sign up and termination All pharmacists must agree to the terms of service contained within this Service Level Agreement. All pharmacists who want to provide this service must be signed up (by the appropriate person in their workplace) to the Client Group Direction. Participating pharmacies may withdraw from the scheme at any time and the PCT may also terminate provision from a pharmacy. Any such intention must be made in writing to the same individual who receives the sign up forms, and a notice period of 28 days applies. Changes of ownership will require reapplication to provide the service. Staff changes (eg new manager or regular locum) will necessitate further applications for these individuals to provide the service. Display All participating pharmacies will be required to provide clearly visible and viewable dedicated window space for a poster (approximately A3 size) giving details of emergency contraception and how to access it locally. If the pharmacy regularly operates without a pharmacist authorised to provide the service, they may like to consider removing the poster when the service would be unavailable. Whether or not this is undertaken, pharmacy staff must be trained to clearly advise clients when the service is not available and to direct them to one of the alternative options. 4. Consultations All consultations must take place in a suitable area/room according to the specifications set out for the provision of Advanced Services in the new community pharmacy contract. Only authorised pharmacists can undertake consultations. No part may be handled by any other individual, although other pharmacists and pre-registration students may, with the client’s consent, observe for training purposes. It should be reinforced at the start of every consultation that the service is completely confidential (the only exception being when the professional judges that the client is at risk and involvement of others is necessary, eg child protection – more information to support this can be found below and on the client assessment form). This client assessment form provides a helpful aide memoire to ensure all relevant aspects are covered in the consultation and that only clients who meet the criteria set out in the client group direction are supplied EHC. One of these forms must be completed for every consultation. The service will consist of: A consultation with the client Performance of a pregnancy test if required by the PGD and subject to the pharmacy having suitable facilities (see RPSGB professional guidance). If a pregnancy test is indicated but the pharmacy cannot undertake one, then the client should be referred to one of the alternative routes of supply Supply of EHC and supervision of consumption: this is a requirement of the PGD and there can be no reasonable objections from a genuine client. If they feel sick, advise them to have a snack and return shortly Discrete supply of six condoms; eg in a pharmacy bag (condoms will be made available by the PCT, reordering should be undertaken using the monthly return form*) Discrete supply of appropriate leaflets (on family planning and contraceptive services and sexually transmitted infections, as available); eg in a pharmacy bag (leaflets will be made available by the PCT, reordering should be undertaken using the monthly return form) EHC Service Specification 140905 © Chorley & South Ribble PCT 2005 6 Page of 20 Provision of all appropriate advice and counselling on EHC and sexual health, plus referral to other agencies where necessary Completion and submission of all necessary documentation [ * Please note that these condoms should not be used for other purposes: quantities used will be compared to consultations completed and supplies may be declined if it appears stocks have been used elsewhere, ie condoms have run out before enough consultations have been recorded to use this amount. In such a situation the pharmacy will be expected to supply condoms that they provide themselves. ] 5. Referral Regardless of whether a supply is made all clients should be advised to consult their GP, Community Contraceptive clinic or TALKWIZE, or GU Medicine clinic regarding future contraceptive and / or sexual health needs. All services are completely confidential. In some situations the pharmacist may, on the basis of the information obtained, be unable to supply under the terms of the client group direction. In these situations the scheme requires the pharmacist to urgently refer the client to an alternative provider. The client must be made completely aware of the 72-hour time limit for EHC and its decreasing effectiveness with time. The scheme also requires pharmacists to make referrals to other agencies, eg Child Protection or GU Medicine, as appropriate, even if a supply is made. 6. Record keeping A copy of the client consultation form must be fully completed for every consultation, regardless of whether a supply is made. The form must be signed by the client but if they refuse the pharmacist may sign to indicate this, as long as the client is aware they are agreeing with the statement. Completed client consultation forms should be kept in a secure and confidential manner e.g. in a filing cabinet or office but not on the shelf which anyone can access. These forms must be kept for 8 years. Electronic patient medication records must be made for every supply/non-supply made. At the end of each calendar month the pharmacist should fully complete the Monthly Return Form and post this to the PCT by the 5 th working day of the following month. Pharmacists are also advised to keep a copy of this form for future reference. If you have not carried out any consultations the PCT still requires you to fill out a monthly return. This is for internal audit procedures. Payments Payment will be made direct to the nominated business or person specified on the sign up form, within 30 days of receipt of properly completed monthly return forms. Incomplete forms will be returned to the pharmacy for amendment. The scheme payments will be: A one-off set-up payment of £100 per pharmacy. This payment is subject to full participation in the scheme. If the pharmacy withdraws from the scheme before undertaking 100 consultations or after less than 12 months (whichever occurs first) or does not provide the service, it will be required to be repaid to the PCT. It cannot be reclaimed if a pharmacy withdraws from the scheme and subsequently rejoins. £10 for every consultation completed, regardless of whether a supply was made The list price for Levonelle 1500mcg tablet stated in the Drug Tariff £5 per pregnancy test undertaken Fees will be reviewed annually in conjunction with the LPC. CLINICAL GOVERNANCE ISSUES A. Clinical effectiveness Results from a recent clinical study (Lancet 2002; 360: 1803-1810) showed that a 1500 microgram single dose of Levonelle 1500 (taken within 72 hours of unprotected sex) prevented 84% of expected pregnancies (compared with 79% when the two 750 microgram tablets were taken 12 hours apart). B. Audit As with any new service, initial and ongoing evaluation is necessary to ensure it is working correctly, meets the needs of clients, healthcare professionals and the NHS and to check whether any improvements could be made. It is anticipated that periodic audits will be provided for pharmacists to complete. More details will be provided as these are produced. The completion of these occasional evaluations is included in the reimbursement for the service. From time to time the PCT may select a number of pharmacies for more in depth verification checks. This will be undertaken by a variety of methods, but most often cross referencing monthly return forms with the consultation records held in the pharmacy. Risk management Child Protection Issues Pharmacists already provide care for children but widening this remit to include provision of EHC may bring new issues to the fore. For this reason participating pharmacists are required to have regard to child protection/child in need issues and must understand the Fraser competencies. Please refer to service specification 2.3.11 of Essential Service 8. Pharmacists should act in accordance with the RPSGB guidance and Chorley & South Ribble Primary Care Trust child protection guidelines. Confidentiality Due to the sensitive nature of this service providers of this service must take care to adhere to service specification 2.7.2, 2.7.3 and 2.7.4 of Essential Service 8 that deals with Information Governance issues. Pharmacists and their staff must also adhere to the guidance from the Medicines, Ethics and Practice as follows: “The public is entitled to expect Pharmacists and their staff to respect and protect the confidentiality of information acquired in the course of their professional duties. The duty of confidentiality extends to any information relating to an individual, which pharmacists or their staff acquire in the course of their professional duties. Confidential information includes personal details and medication, both prescribed and not prescribed.” Supervised consumption Pharmacists must witness the client consume the EHC on the premises. This is to ensure that the EHC is not being fraudulently obtained. Standard Operating Procedures (SOPs) A SOP must be written to cover this supplementary enhanced scheme. The SOP must be specific to the pharmacy and staff it applies to. See service specification 2.3.7 of Essential Service 8. D. Client and public involvement Pharmacies must run their own internal complaints procedures consistent with the NHS’ requirements and apply the standard NHS complaints procedure, if required by a client. Refer to service specification 2.1.5 of Essential Service 8. Pharmacists should also note that participation in any scheme such as this also entails agreement to the right of inspection from NHS Patients’ Forums. Information governance Client Confidentiality Refer to Risk Management section above. Records A record must be made of all consultations and supplies made in this scheme on the clients’ PMR on the computer. Refer to specification 2.7.5 of Essential Service 8. Client Assessment Forms must be kept in the pharmacy for 8 years (in accordance with Chorley & South Ribble PCT’s Clinical Records Policy). F. Education and training All pharmacists must complete the following accreditation to become authorised to supply EHC in this scheme: I. Complete the pre-workshop CPPE task and attend the PCT workshop II. Complete the distance learning CPPE course on emergency hormonal contraception III. Complete the PCT’s Client Group Direction learning pack Refer to specification 2.6.2 of Essential Service 8. G. Staff management Staff must be supported and fully trained. They must know of their responsibilities on confidentiality. They must know how to deal with customers requesting to access the scheme with tact and diplomacy – include this in your SOP.
