Guidelines:
 All nominations shall remain confidential. Students must not be advised of their nomination.
 Sections from 1 to 4, and Section 6 should be completed for all nominations
 If more than one student is being recommended from the same major, additional justification for the recommendations is
required in Section 5
 Recommendations must be considered by the Faculty Assessment Review Group
 Recommendations must be submitted by the deadline advertised in the Assessment Schedule. Any anticipated delay in
submission must be reported to the Manager, Student Administration before the advertised deadline.
