INSTRUCTIONS:
(1) This notification shall be submitted before any pre-consultation or
submission of building plans to the Commissioner of Building Control .
(2) The registered Project Reference Number and Title is valid for a period of two
years from date of acknowledgement by the Commissioner of Building Control.
(3) * Delete where not applicable.
(4) One copy of this form is to be submitted and two copies of Appendix 1 to be
submitted
(5) This form need not be submitted for simplified A/A submission for
conventional residential houses.
(6) This form is to be filled in BLACK INK only.
