Introduction
The Management of this building in cooperation with the San Diego Fire Department
has compiled this manual for 4225 Executive Square, La Jolla, California to help
ensure the safety of the building’s occupants in the event of an emergency and to
comply with the provisions of the California Code of Regulations, Title 19, and the
San Diego Fire Code.
The material in this manual pertaining to Title 19 of the California Code of
Regulations, the San Diego Municipal Code and the San Diego Fire Code is
required by law. Additional procedures outlined for Earthquake, Bomb Threat,
Medical Emergency, etc., are recommendations only. For further legal requirements
and information regarding such situations, refer to the appropriate agency.
The provisions of Sections 3.09 and 3.10, Title 19 of the California Code of
Regulations require that persons responsible for new and existing high-rise buildings
comply with the Emergency Pre-Fire Planning and Evacuation Requirements as set
forth therein. Owners, managers, operators, administrators and tenants of each
high-rise building in the state of California shall comply with these requirements or
be subject to prosecution and penalties, including fines, as set forth in Title 19 of the
California Code of Regulations.
This manual and its contents shall remain the property of the building and be made
readily available to members of the San Diego Fire Department upon demand.
This manual has been prepared under the guidelines and in the format suggested
by the San Diego City Fire Department. The author of this manual does not assume
responsibility in the event of any emergency that should occur. He does not assume
responsibility if building management does not keep The Emergency Procedures
Manual current. All technical information within The Emergency Procedures Manual
was provided by building staff.

General
If you discover fire or smoke
1
Safety of life is the first priority. Remove anyone in the area from immediate
danger. Confine the fire by closing as many doors as possible as you leave the
area.
2
Notification: Activate a manual pull station in close proximity. Call the fire
department at 9-1-1. If you encounter problems with the 911 system, dial (858)
974-9891 and provide the following information:

Fighting the fire is an option only if you are trained, you have someone with you,
it is safe to do so, and if it does not interfere with the performance of your
emergency duties.

Evacuate adjoining areas and begin your assigned duties as defined by this
manual.
If Trapped In an Office or Area




Wedge material along the bottom of the door to keep smoke out.
Close as many doors as possible between you and the fire.
Call the fire department and notify them of your situation.
If windows are operable and you need air, open the window. Break windows
only as a last resort. Smoke may enter the room and it will be impossible to stop
it.
 If unable to perform your assigned duties notify other staff members.
Smoke Detectors:
Smoke detectors are provided for your personal safety. Anyone who willfully and
maliciously tampers with, damages, breaks or removes any required smoke detector
shall be guilty of a misdemeanor.

8. Provide instruction to physically impaired persons.
If you see fire or smell smoke
1. Call the fire department and activate an alarm or send someone to activate an
alarm.
2. Ensure that your emergency team members are in place.
3. Begin directing the evacuation.
4. Once all occupants have evacuated, account for all members of your Emergency
Response Team and then exit the floor together using the nearest safe stairwell.
5. Relocate to the designated refuge area and account for all employees.
6. Report any missing employees immediately to the fire department.
7. Report the location of any physically impaired persons still on the floor to the Fire
Department.

Fire Extinguisher Use
Not all fires are the same, and they are classified according to the type of fuel that is
burning. If you use the wrong type of fire extinguisher on the wrong class of fire, you
can, in fact, make matters worse. It is therefore very important to understand the four
different fire classifications.
Class A - Wood, paper, cloth, trash, plastics
Solid combustible materials that are not metals. (Class A fires generally
leave an Ash.)
Class B - Flammable liquids: gasoline, oil, grease, and
acetone. Any non-metal in a liquid state, on fire. This classification
also includes flammable gases. (Class B fires generally involve
materials that Boil or Bubble.)
Class C - Electrical: energized electrical equipment
As long as it's "plugged in," it would be considered a class C fire. (Class
C fires generally deal with electrical Current.)
D
Class D - Metals: potassium, sodium, aluminum,
magnesium Unless you work in a laboratory or in an industry that
uses these materials, it is unlikely you'll have to deal with a Class D fire.
It takes special extinguishing agents (Metal-X, foam) to fight such a fire.

It's easy to remember how to use a fire extinguisher if you can remember the acronym
PASS, which stands for Pull, Aim, Squeeze, and Sweep.
Pull the pin.
This will allow you to
discharge the extinguisher.
Aim
Aim at the base of the
fire.
If you aim at the flames
(which is frequently the
temptation), the
extinguishing agent will fly
right through and do no
good. You want to hit the
fuel.
Squeeze the top handle
or lever.
This depresses a button
that releases the
pressurized extinguishing
agent in the extinguisher.
Sweep from side to side
until the fire is completely
out. Start using the
extinguisher from a safe
distance away, then move
forward. Once the fire is
out, keep an eye on the
area in case it re-ignites.

Types of Fire Extinguishers
Dry Chemical extinguishers are usually rated for multiple purpose
use. They contain an extinguishing agent and use a compressed,
non-flammable gas as a propellant.
Water These extinguishers contain water and compressed air and
should only be used on Class A (ordinary combustibles) fires.
Carbon Dioxide (CO2) extinguishers are most effective on Class
B and C (liquids and electrical) fires. Since the gas disperses
quickly, these extinguishers are only effective from 3 to 8 feet. The
carbon dioxide is stored as a compressed liquid in the
extinguisher; as it expands, it cools the surrounding air. The
cooling will often cause ice to form around the “horn” where the
gas is expelled from the extinguisher. Since the fire could re-ignite,
continue to apply the agent even after the fire appears to be out.
Halon extinguishers contain a gas that interrupts the chemical
reaction that takes place when fuels burn. These types of
extinguishers are often used to protect valuable electrical
equipment since they leave no residue to clean up. Halon
extinguishers have a limited range, usually 4 to 6 feet. The initial
application of Halon should be made at the base of the fire, even
after the flames have been extinguished.

General Procedures
Definitions
PANIC: A sudden unreasoning terror often accompanied by mass hysteria. Panic is
often the result of not knowing what to do.
EVACUATION: The movement of people off of a floor. The emergency movement of
people is best accomplished in a supervised manner. Personnel trained on proper
evacuation methods will make relocation of any number of people a safe operation.
HORIZONTAL EVACUATION: The movement of people across floors to refuge areas,
stairwells, etc.
VERTICAL EVACUATION: The movement of people either down or up.
RELOCATION: The movement of one floor or several floors within a building.
TOTAL EVACUATION: The movement of all occupants from the building. Total
evacuation is not always necessary or recommended.
Total evacuation should be
considered in the event of extreme emergency situations.
Specific Evacuation Procedures
1. The audible / visual alarm activates on ONE FLOOR, the floor where the device is
activated. Upon hearing the alarm on your floor, all occupants will exit their suite,
and take the nearest stairwell to exit the building.
Always respond to an alarm as if there were a real emergency event
occurring.
 It is important that occupants remain quiet enough to hear the instructions
that may be given by the Floor Warden or the public address system.
 All occupants should be familiar with the evacuation routes to safely reach
the refuge area(s). Use caution crossing driveways or streets.
 When relocating to an outside area of refuge, for their safety, occupants
must always be a minimum of 300 feet away from the building. The current
designated refuge area is not 300 away from the building, but from the
designated refuge area, occupants have the ability to move further away
from the building due to safety issues or when directed to do so by building
staff or other personnel, such as the Fire Department.

2. While it is usually advisable to go downward in a building during an alarm, there are
times when it may become necessary to go to an upper floor or to the roof. This
should only be done if lower floors are untenable due to heat or smoke, or if directed
by building Staff or the Fire Department.
3. Once an evacuation has taken place, the Floor Warden will take a head count at the
refuge area. The Floor Warden will ask if everyone has evacuated. Make sure all
visitors are accounted for. Floor Wardens will report any missing occupants and the
location of the physically challenged to the Building Staff or Fire Department
Personnel.
Safe Stairwell Procedures
a. Move quickly, but DO NOT RUN.
b. Do not carry food and/or beverages into the stairwell as they may spill and
cause a slip and fall hazard.
c. Respond to the safest designated stairwell. DO NOT USE ELEVATORS.
d. Remove high heels to prevent injuries (carry them with you).
e. Use handrail which is most continuous (usually center).
f.
Each floor will relocate 3 or more floors below the fire floor.
g. Allow room for others to enter into an orderly flow of traffic without holding up
others.
h. Gain assistance for those who are slower moving or require assistance.
i.
Dispel any false information or rumors (to prevent panic, refrain from using
the word “fire”).
j.
Treat any injuries incurred in the stairwell at the nearest landing when
required or if practical.
k. Complete the relocation. DO NOT CONGREGATE IN STAIRWELL.
Locked Stairwell Information

Stairwell doors are locked from the stair side for security reasons. All stairwell
doors are equipped with electric locks. Upon activation of any alarm device,
these doors will automatically unlock.

A Safe Evacuation Requires
 An urgent response:
Be in the stairwell in 2 minutes or less.
 Creating a safe stairwell – eliminating items that create
slips, falls and twisted ankles, as an example. Therefore, the following
should not be brought into the stairwell.

Evacuation for the Physically Impaired
Physically challenged individuals may need assistance during an emergency. Any
person who may need the assistance of another to effectively evacuate during an
emergency is considered physically challenged. The impairment may be permanent or
temporary, and may include but not be limited to:





Persons confined to wheelchairs
Persons dependent upon crutches
Persons with significant hearing or sight impairment
Pregnant persons
Extreme obesity
The following information should be distributed to all occupants who are physically
impaired:
Pre-Emergency Actions:
1. Prior to an emergency select two assistants to assist you in an emergency. Meet
with the assistants to discuss your special needs in case of an emergency. Be
sure to tell them how best to assist you. Remember you are the expert on your
own personal condition.
2. Decide on a meeting spot. For example: at your desk or at a designated
stairwell.
3. If applicable, have assistants become familiar with various lifts and carries.
EVACUATION AND PANIC CONTROL
During a Fire Emergency:
1. Upon hearing the fire alarm, meet with your assistants.
2. Proceed to the nearest safe stairwell.
3. Enter the stairwell last and remain on the landing. Be sure to close the door
behind you. One assistant should notify Building Staff or Fire Department
Personnel of your location. For example; on the 3rd floor in Stairwell #2. The
other Assistant will remain in the stairwell with you. If further evacuation is
necessary the Assistant may assist you to five floors below the fire floor to a reentry floor or to the exterior refuge area.
4. Remain calm. Help is on the way. The Fire Department’s first priority is rescuing
people.

Listed below are some things that you can do for yourself if you are alone during
an emergency:
1. Proceed to the stairwell. Instruct all others who are exiting to tell the Fire
Department which floor and stairwell you are in.
2. Be prepared to ask for assistance. Tell them what your condition is and be
prepared to give instructions on how you can best be helped.
If you cannot leave your office:
1. DO NOT PANIC.
2
Close as many doors as possible between you and the fire.
3. Immediately call the Fire Department at 9-1-1 or if a problem occurs with the
9-1-1 system, call (858) 974-9891. Tell them you cannot get out. - Tell them you
are physically impaired and in what way. - Give them your address and suite
number. - Give them the nearest cross street. - Give them the number you are
calling from: This is important because the Fire Department may have to call
you back to get more information.
4. Stay where you are. Help is on the way.

Earthquake Procedures
General Information
Earthquakes are potentially the most devastating and dangerous form of physical
phenomena. Historical instances report variations in effect from faint rumblings and
light vibration to total destruction of major cities.
Earthquakes will occur suddenly with little or no advance warning. Tremors, vibration
and shaking of the ground will occur. The shaking may become violent with subsequent
upheaval and lateral displacement of the ground. Structures above ground level may
become unstable or toppled; below-ground installations, such as utility and
communication lines, may be disrupted or destroyed.
While such occurrences are unpredictable, advance preparation, planning, and training
may help protect people from injury or death.
Before the earthquake

Identify alternate escape routes in the building in case one or more areas
become blocked with debris. Be aware of locations for fire alarms and fire
extinguishers.
Once the shaking starts

Duck and cover under a hard surface like a desk or table.
Or

Move to an interior wall away from glass, get down on your hands and knees and
protect your head and neck.

Face away from any windows.

Stay clear of tall objects that may tilt and topple over.

STAY IN THE BUILDING. Do not run outside. Debris may be falling.
Once the initial shocks have subsided

Remain calm. Be prepared for aftershocks.

Check for injuries and give first aid. You have no duty to come to the aid of a
stricken person. However you may voluntarily assist the victim to the degree that you
are trained.

Do a check for hazards that may have been caused by the quake.

Use emergency supplies if necessary.

DO turn off electrical devices.

DON'T turn on any lighting or electrical devices.

DON'T use the telephone except in extreme emergencies.

Wait for reliable information to confirm your path of travel to your destination before
evacuating. Roads and highways may be inaccessible.

Employees should exit the building by the stairs, never use the elevators.

Stay out of, and away from, damaged buildings; after-shock may topple damaged
structures.
Floor Warden
Before
1. Know the location of possible Refuge Areas outside and away from the building
in case evacuation is necessary. Generally, it is safer to remain inside the
building.
2. Know location of utility shutoffs - gas, water and electric.
3. Know location of nearest Essential Service Phone - usually a pay phone.
4. Reduce non-structural hazards, for example: secure top-heavy objects such as
bookcases, filing cabinets, etc. to structural elements of the building. Remove all
heavy objects from top shelves. Secure cleaning liquids and other chemicals on
shelves to help prevent spillage.
5. Maintain battery operated portable radios (with extra batteries) to receive
Emergency Broadcast Reports.
6. Have a First Aid Kit (check & maintain quarterly.)
7. Maintain battery operated flashlights in working condition.
8. It is also recommended that you have: - heavy gloves in case of broken glass; a
pair of rubber soled shoes; replacement glasses or contacts - if you are on any
medication, have a 72 hour (minimum) supply with you at all times; water: store
water and rotate the bottles regularly.

9. Have a plan for reuniting your family. You will not be able to function at work
effectively if you are worried about your family. Make sure they know what to do.
10. Have an out of state contact person. So when you are able to use a phone, you
can call to see who is accounted for.
11. First Aid Training is highly recommended for occupants of the building.
12. For more information on Earthquake Preparedness, contact the American Red
Cross or your local Fire Department.
13. It is recommended that Occupants be prepared for a 72-hour period (water,
food, medical needs, sewage) in accordance with the local Emergency Disaster
Services.

1. Get under a desk, table or other sturdy object and hold on; or brace
yourself against an interior wall in the core of the building. Protect your
head.
2. Stay away from windows, bookcases, filing cabinets and any objects that may
fall or shatter.
3. Do not dash for exits, since stairwells may be damaged. Do not use elevators
until they are checked for safety.
4. Do not smoke or use matches, candles or lighters in case of gas leaks.
5. Do not be surprised if electricity goes off.
NOTE: DOORWAYS SHOULD NOT BE USED AS A REFUGE.
If you are outside: Stay there. Move away from structures, power poles, lampposts or
retaining walls that could fall during the quake and avoid fallen electrical lines. If
possible, move to an open area.
After
BE PREPARED FOR AFTERSHOCKS
1. All building staff should communicate their current location and the condition of
their location.
2. Report to fire alarm panel, if safe to do so. Assess emergency systems.
3. Receive reports from Building Staff regarding utilities and condition of building.

4. Building Staff will check building for damage and for injured occupants.
5. Receive status reports from Building Staff, Suite/Floor Wardens or Monitors
regarding extent of injuries, hazards and building damage on each floor.
6. Assess damage and take emergency actions. Order evacuation or relocation as
necessary.
7. Assign staff or occupants to act as “runners” to relay messages to Suite/Floor
Wardens.
8. Call appropriate outside emergency services (for example: Fire Department,
Paramedics, utility companies, etc.) if necessary. If telephones are not working,
assign staff or occupant to use nearest essential service phone or a cellular
phone.
9. Generally, it is safer to remain inside the building. If evacuation is necessary,
alert all staff to begin evacuation of occupants. Assemble occupants at a refuge
area outside and away from the building. This area cannot be predetermined. It
will depend upon the earthquake damage.
10. Listen to radio for emergency reports/information.
11. Keep occupants quiet and calm. Keep occupants informed to prevent spreading
of rumors.
12. Cooperate with Public Safety Officials.
NOTE: If you are in an elevator, sit on the floor or brace yourself against the wall.
Remain calm - the elevator is designed not to fall.
When can occupants go home?
It is best that in the event of an earthquake or community wide disaster during normal
working hours, all occupants remain at work. It may be too dangerous or improbable to

and roads that have sustained damage. Discourage leaving until they know roads are
undamaged and traffic is moving. Encourage occupants to assist Wardens as
necessary.
Earthquake Evacuation
Determine in advance the safest exit from your work location and route you will follow to
reach that exit in the event an evacuation is necessary. Also establish an alternate
route to be used in the event your first route is blocked or unsafe to use.

What if you are in an elevator?
If you are in an elevator, you are probably better protected than most people. The
elevator is designed to not fall down the shaft and nothing heavy can fall on you.
However, some elevators will stop in any moderate earthquake. Building personnel will
contact each elevator car as quickly as possible and advise you how rescue will occur.
Upon being rescued, take directions from the Floor Warden on that floor.
If you have a medical or other emergency, pick up the receiver in the elevator car and
speak with the operator. Be sure to tell the operator the number of the elevator car you
are in.

Bomb Threats
Background Information
Research has resulted in the following insights:
Because there are groups—small in number—dedicated to harassing industry and
government by doing damage through explosive charges, it is not prudent to ignore a
threat when it is received. This is so even though statistically it can be shown that the
underlying motives in most threats are to cause disruption, harassment and excitement,
and not physical destruction.
Threats generally fall into the following two broad categories:
1. The person making the threat claims he is going to do something, for example,
place an explosive in the building.
2. Claims he has already done something, for example, placed an explosive in the
building.
Most threats are communicated by telephone and are almost impossible to trace.
A number of actual bombings to date have taken place in buildings during hours when
few employees are known to be working. Also, in most cases, when an explosion has
actually taken place, the caller has been specific as to the time the explosive will
detonate. A pattern has also developed indicating that the bomber will notify the police
authorities in addition to notifying the building in which the bomb has been placed. The
calls have generally been received from 20 to 40 minutes prior to the detonation. This
short time span is undoubtedly designed to prevent a thorough search of the premises
or facility to locate the explosive charge.

Upon Notification of Bomb Threat
Notify the proper authorities:



Police Department 911- if a problem occurs, then dial (619) 531-2000.
Give exact location and all known facts. Be guided by the instructions of the Police
Department.

Ensure that, "threat conversation" is documented as accurately as possible and as
soon as practical. To assist the Police and as an aid to completing reports use the
Bomb Threat Report in this manual.
If Your Floor Is Affected
Gather with the other Monitors to assist in a search and/or evacuation procedure.
All suites/rooms must be systematically contacted and advised of the situation.
A quick visual search is advisable and should be accomplished by those FAMILIAR WITH
THE AREA. Work from the walls to the center of room, and the floor to the ceiling.
LOOK and LISTEN. If lights are found OFF, LEAVE them OFF.
After an area has been searched and Occupants have evacuated that area, reentry must be
restricted and controlled by proper authorities. All items not carried in by arriving personnel
and readily identified as inert shall be left to the proper authority for inspection. All suspicious
objects should be immediately reported to the Police and Building Management.

If evacuation is ordered, consider:
1. Size, location and structure of building.
2. Number of people on duty. It is difficult to evacuate large facilities with many
people.
3. Proximity and danger to neighborhood buildings or to other businesses sharing
the same building.

5. Need for evacuation of entire building. Some areas with built-in security versus
other areas accessible to the public.
6. The availability of outside area to which employees can be evacuated.
7. The liabilities involved if an explosion occurs and the building was not evacuated.
8. Overreaction resulting in repeated immediate evacuation could lead to continuing
threat calls from pranksters.

Conducting a Search
1. If the caller states or implies that a bomb is in a particular suite, be aware that the
individuals working in that suite will be called upon to conduct a search of their area.
These individuals are the best people to determine whether something doesn't belong
or if something has been moved, or is out of place.
2. Each tenant should develop and maintain a search plan for their office suite. There
should be two Searchers per team. If the suite is large, divide it into quadrants and
permanently assign searchers to a specific quadrant.
3. Conduct your search by assessing the room utilizing the following method:

Power Outage

Gather your Monitors together and assess your resources, collect all available flashlights
and check if cell phones are working.

Send your Elevator Monitor to the Elevator Lobby to stop people and to listen for possible
people trapped in an elevator. Advise passengers that Building Personnel will advise
them of the time and rescue method.

Determine if there is a need to evacuate the floor by listening for instructions from
management. The building emergency generator will begin to operate ten seconds after
the main power fails and will provide power to bring all elevators down to the lobby and
then only one elevator will operate in each bank of cars until normal power is restored.
Elevators are not to be used for evacuation.

Notify Building Management at 858-450-2880. If the telephone is not operating, walk
down the stairs and report to Security in the Lobby of the building.

Turn on battery powered radio to determine if the power is out in your area, or just your
building.

Unplug ALL electrical equipment light switches. WHEN POWER RETURNS IT MAY
SURGE AND BLOW OUT LIGHTS AND ELECTRICAL EQUIPMENT IF LEFT ON.

Open window covering to provide natural lighting if during the daytime.

If evacuation is necessary, use of flashlights or light sticks is recommended, as building
emergency power provides a reduced amount of lighting, and it only runs for four days.

Power Outage
1. Remain calm and in place.
2. Look around your area to see if the outage is within your suite only, the entire floor
or area. If it appears to be within your suite only, call the office of the Building at
858-450-2880. If it appears to be the entire area, turn on a battery powered radio to
find out what is happening.
3. Gather your Monitors together. Check elevators for trapped individuals. Notify
Security of any problems.
4. Inform co-workers to:
a. Remain calm. Generally, there is no immediate need to evacuate.
b. Unplug all electrical equipment, projectors, televisions, audio/visuals and
light switches. When the power returns, it may be in a surge and blow
out light bulbs and other equipment.
c. Open window shades or drapes to provide natural lighting. Depending
upon the time of the year, (if sun is bright) you may not want to open
shades/drapes as this may heat up the office area.
d. Check flashlights and light sticks in case evacuation is necessary.
NOTE: This building has an emergency generator which will power:





Emergency lights in the stairwells, elevators, corridors,
restrooms, tenant space, Engineering Office, Fire Control
Room and on the parking levels;
Elevators – all elevators will come to the Lobby Level
one at a time, and then one elevator in each bank will
operate on emergency power;
Fire Alarm System – Fire Alarm Panels, PA System,
audible/visual alarm, stairwell pressurization fans and
jockey pump;
Security Console;
EXIT signs.
5. If evacuation is necessary, inform all occupants to begin evacuation. Use
flashlight or light sticks to assist you in evacuating.

Building Systems and Equipment
4225 Executive Square is a 16-story high-rise with an adjacent parking structure
consisting of 3 above and 2 below ground parking levels. This emergency systems
section will apply specifically to the high-rise building. 4225 Executive Square is fully
sprinklered.
Life Safety Systems
a. Fire Alarm Panels: The Fire Control Room is located on the first floor, in the lobby
of the building. From there, the Fire Safety Director and the San Diego City Fire
Department will coordinate the building’s emergency operation.
The Alarm panel in the Fire Control Room indicates the device activated and location
of the device. Systems monitored and controlled include:
 Manual Pull Stations
 Smoke Sensors - Including Elevator Lobby, Area, and Duct
 Sprinkler - Water Flow Alarms and Valve Tamper Switches
Other systems which can be monitored from the Fire Control Room include:
 Elevator Recall
 Emergency Generator
 Fire Pump
 Stairwell Pressurization
The public announcement system for the building is located in the Fire Control
Room.
b. Fire Departments Lock Box: The Knox Box is located 8’ high on column in front of
the building facing toward the building.
c. Security: Security is on site 24 hours a day, 7 days a week. Security personnel
maintains roving patrols in the common areas of the complex, including parking, on
a 24-hour basis. Security is provided by Universal Protection Service.
d. Fire Alarms: The fire alarm emits a “Whoop, Whoop” (followed by short message)
“Whoop, Whoop” sound with a flashing strobe. The audible and visual alarm is
designed to sound on the alarm floor only. A fire alarm signal is also enunciated at
the fire alarm panels and at an outside monitoring company (Central Monitoring
Station). There is a short message that is said when alarm goes off stating there is a
fire in the Bldg.
ALL ACTIVATED ALARMS WILL SOUND ON THE ALARM FLOOR AND WILL BE
TRANSMITTED TO AN OUTSIDE MONITORING COMPANY (CENTRAL
MONITORING STATION) FOR AUTOMATIC DISPATCH OF THE SAN DIEGO
CITY FIRE DEPARTMENT.

The audible and visual alarm is activated by any alarm condition - manual pull
stations, water flow, and smoke detectors.
e. Manual Pull Stations: Manual Pull Stations are located by the stairwells and in the
elevator lobbies. To activate: PULL DOWN. Flashing lights are located in the
elevator lobbies and in the corridors.
Activation of a pull station will: activate the audible and visual alarm on the alarm
floor; activate an alarm condition at the fire alarm panels, and at the outside
monitoring company and close all “fire doors” throughout the building.
f.
Smoke Detectors: Smoke detectors are located in the elevator lobbies, adjacent to
doors equipped with magnetic door holders as well as the return air plenums in the
base building air conditioning ducts, and electrical and mechanical rooms.
Activation of a smoke detectors will: activate the audible and visual alarm on the
alarm floor; activate an alarm condition at the fire alarm panels, and at the outside
monitoring company; shut down the heating, ventilation and air conditioning system
throughout the building and close all “fire doors” throughout the building.
NOTE: Only activation of an elevator lobby smoke detector will recall the elevators.
Elevators will return to the lobby level automatically unless the alarm is that level in
which case the elevators will recall to an alternate floor.
g. Sprinklers: Sprinklers are activated when the heat responsive element releases at
a temperature of approximately 165 degrees Fahrenheit. Each sprinkler head
discharges approximately 15 to 18 gallons of water per minute.
Sprinklers for the building are on a loop system and can be shut off on the individual
floors by closing the valves at each floor in both stairwells.
Activation of a sprinkler head will: activate the fire pump, activate the audible and
visual alarm on the alarm floor, activate an alarm condition at the fire alarm panels
and at the outside monitoring company shut down the heating, ventilation and air
conditioning system throughout the building and close all “fire doors” throughout the
building.
h. Fire Pump: The building has a Diesel fire pump. The pump is located on the G2
level, south side of the parking structure. The fire pump is activated by a loss of
pressure from a sprinkler or standpipe. An alarm signal is activated at the fire alarm
panels. An 8,000 gallon water reserve tank is located on the G1 level at the
southeast corner of the parking structure. The fire pump has it’s own fuel supply
located in the fire pump room.
i.
Emergency Power: The building has a diesel generator which is designated for
emergency power of the life safety systems. The generator is located on the roof.
Fuel for the generator is supplied by a 500-gallon double-wall underground diesel
fuel oil tank located southwest of the building.

Stairwell #1: Located west of the elevator lobby and serves floors 1 through 16
Stairwell #1 exits on the west side of the building near Regents Park Row.
Stairwell #2: Located east of the elevator lobby and serves floors 1 through 16.
Stairwell #2 exits on the east side of the building near Executive Square.

Elevators:
The building has 5 passenger elevators which service floors 1 – 16. Elevator #5 is
designated as the freight elevator and serves all floors. Elevators can be manually
recalled from the Fire Control Room and the elevator lobby.
The adjacent parking structure for 4225 Executive Square has 2 passenger
elevators serving 5 levels (G1 through G5).
In a fire emergency: Upon activation of an elevator lobby smoke detector,
elevators will recall to the ground level automatically. Elevator doors will open and
elevators will shut off.
In an earthquake: Elevators will stop momentarily then move up or down to the
closest floor - opposite its counterweight. Doors will open at that floor and elevators
will remain there until inspected and reactivated by an elevator mechanic.
In a power failure: Elevators will stop momentarily; the emergency generator will
start up and provide power so the elevators will move to the first floor one at a time.
Once all elevators have returned to the ground level, one elevator will begin to
operate on emergency power.
Each elevator cab has emergency lights and a phone for two-way communication to
the Mitsubishi Elevator Company. To activate the Emergency Phone, Push Button
and talk.
m. Magnetic Door Holders: Doors in the passenger elevator lobbies on all floors 2
through 16 are equipped with magnetic door holders. Activation of any alarm device
will release these doors throughout the building. Doors with magnetic door holders
will also release in case of a power failure.

n. Communications: Radios for use by Building Management, Engineering, Security,
and Emergency Response Teams. Public Address System, located in the Fire
Control Room for communication to a single floor, multiple floors or the entire
building. Voice activated phones are in the Fire Control Room for use by the Fire
Department.
o. Standpipes: Standpipes are located in Stairwells 1 and 2, and are for use with Fire
Department hoses. When the standpipes are activated, a water flow alarm will
enunciate at the fire alarm panel and at the remote monitoring station.
p. Fire Equipment: Type ABC fire extinguishers are located on all floors. Type ABC
extinguishers are for use on fires involving wood, paper, cloth, rubber, plastic,
grease, oil and electricity.

Floor Warden Duties
At 4225 Executive Square, the audible/visual alarm sounds on ONE FLOOR, the floor
where the device was activated. Upon activation of any alarm on your floor, Floor
Wardens and Monitors will begin their emergency procedures (If you hear or see the fire
alarm…).
 Upon activation of any alarm, a Floor Warden on the floor above and the
floor below the floor in alarm may be called by the Fire Safety Director to
investigate the conditions on their floor.
 Upon activation of any alarm, Floor Wardens on the floor in alarm, the floor
above and below the floor in alarm, may receive a PA announcement giving
emergency instructions. Follow all instructions.
Emergency Responsibilities
1. Upon hearing the fire alarm, the Floor Wardens are responsible for the complete
evacuation of their floor/suite during an alarm condition.
2. If there is a fire alarm in the building (but not on their floor), Floor Wardens may
receive a call from the Building Management to check their floors for adverse
conditions (fire or smoke). Please report all findings to the Fire Safety Director
at 858-450-2880 If there are adverse conditions, initiate your emergency
procedures (activate the alarm and evacuate the floor.)

Floor Wardens and Suite Monitors are responsible for overseeing occupant instruction,
supervising and ensuring safe and complete evacuation during a fire, or other
emergency or fire drill; reports to the Fire Safety Director. On multi-tenant floors, the
Suite Monitor will also assume the responsibilities of the searcher.
Search Monitors
The Search Monitors will assist the Floor Warden by checking their floor or suite for
occupants and alerting them to evacuate. After each office is checked for occupancy,
the Search Monitor should begin closing the doors to all rooms/offices. Each door
should be tagged with a post-it note to signify the office has been checked and vacated.
The post-it notes should be placed on the lower portion of the door approximately two
feet from the floor. Ensuring that all doors are closed will help to prevent the spread of
fire and smoke in the building.
Group Leader (Multi-Tenant Floors)
On multi-tenant floors, the Group Leader will be responsible for 10 to 15 individuals,
making sure these individuals do not use the elevator and do use the stairs safely while
as they lead their group to the designated refuge area. If there were 20 to 30 individuals,
two group leaders should be assigned. The Group Leader is essentially taking care of
the Elevator and Stairwell Monitor responsibilities.

Elevator Monitor
This person will be stationed in the elevator lobby area to ensure that occupants do not
use the elevator during a fire emergency. People getting off the elevator as well as
people wanting to use the elevator should be directed to the safest stairwell for
evacuation.
Stairwell Monitors
One person should be positioned at each stairwell to assist with the safe and quick
evacuation of all floor occupants. The Stairwell Monitor should remind those exiting the
floor to stay to the center of the stairwell while holding onto the railing. This is important
since emergency personnel from the Fire Department etc. may also need to travel in the
building stairwells at the same time. The Stairwell Monitor is also responsible for
directing building occupants to the proper Safe Refuge Area.
Assistants for the Impaired
Two persons should be assigned to each physically impaired individual whose limited
mobility may prevent them from evacuating the floor. A pre-arranged meeting place
should be established and the evacuation procedure discussed. One assistance monitor
will take note of the floor and stairwell in which the person is located and will report this
location to the Building Management and the Fire Department personnel.
Training Responsibilities
1. With the assistance of the Fire Safety Director:
a. Each tenant must select a Floor Warden, Stairwell Monitors, Search Monitors,
and Elevator Monitor.
b. Assign Monitors to assist anyone who is physically impaired. A person who is
physically impaired is anyone who will need assistance getting to the stairs or
walking down the stairs.
2. Read the Floor Warden Manual.
3. All Floor Wardens & Monitors should review procedures to make sure all duties are
understood. Discuss procedures through specific “what if” situations.
4. WITH THE ASSISTANCE OF THE FIRE SAFETY DIRECTOR:

Do not carry open containers of liquids. If it spills, someone may slip and fall.
Use hand rail and keep to one side.
Walk down.
Allow room for others to enter into an orderly flow of traffic, but do not hold
up traffic unnecessarily.
 Gain assistance for those who are slower moving or handicapped.
 Dispel any false information or rumors, etc.
 Treat any injuries incurred in the stairwell, at the nearest floor landing when
required and if practical.
 Complete evacuation, do not congregate in stairwell.
e. Location and use of fire alarms and fire extinguishers.
f. Location of First Aid Kits and emergency supplies.
g. Familiarize occupants with responsibilities of Floor Wardens and Monitors.
It is required that the Occupant Information be duplicated and distributed to all
employees. All new employees must be instructed within 14 days of occupancy.
Document all training.
5. Participate in all fire drills. Complete Fire Drill Report after all fire drills.
6. Be prepared to communicate status reports to Fire Safety Director/Security
regarding the emergency or fire drill.
7. Keep up-to-date list of all occupants who are physically impaired.
8. Keep up-to-date list of all employees. Be sure to keep lists readily available.
9. Have identification and flashlight readily available.
10. Make periodic review of Monitors and alternate Floor Wardens. Assign new
members as needed.
11. Develop awareness for safety conditions, fire violations, and potential hazards. For
example: stairwell or corridors blocked open, improper lighting, frayed cords,
overloaded outlets, obstructed halls and corridors, trash build-ups, etc.
12. If for any reason you should discontinue employment in this building, you are
requested to notify the Fire Safety Director.

SUPPLEMENTAL
Fire Drill Instructions
To have an effective Fire/Life Safety Program, it is necessary that comprehensive
training be given to everyone in the building. Fire Drills are required by C.C.R. Title
19, Section 3.10. All building occupants must participate. Participation is
mandatory.
The purpose of the fire drill is to instill in the minds of all occupants the correct
procedures necessary to ensure the safety of life and the joint testing of the building
emergency equipment and staff duties. FIRE DRILLS ARE FOR YOUR SAFETY AND
PROTECTION.
A fire drill would begin with the sounding of the fire alarm on one or more floors.
Observers, (Building Staff or Alternate Floor Wardens) should be stationed at strategic
locations throughout the drill to observe the actions of Floor Wardens, Monitors, and
occupants as the alarm sounds. FIRE DRILLS ARE A LEARNING EXPERIENCE FOR
TENANTS AND STAFF.
Observations will be made for:
1. Quick response of all Floor Wardens and Monitors to their designated stations.
2. Response of occupants in evacuating the floor.
3. Closing of all office doors to help confine the fire and limit the spread of fire and
smoke.
4. Following correct evacuation procedures.
5. Taking a head count at Refuge Floor/Areas.
6. Reporting to the Building Staff at location of any occupants who are physically
impaired and that the evacuation is complete.
CRITIQUE
1. Points which should be covered: not hearing the alarm; fire equipment blocked or
unusable; exits and hallways blocked; debris in stairwells; doors propped open;
office doors not closed; duties not understood or carried out, including procedures
for the “physically impaired” and response of occupants.
2. All Floor Wardens shall turn in a completed Fire Drill Report in keeping with the
requirements for annual fire drill documentation.

Fire Prevention Tips
1. Never throw matches or cigarette butts into waste containers.
2. Observe NO SMOKING signs. THIS BUILDING IS A NON-SMOKING
BUILDING.
3. Check for frayed or damaged electrical cords. Report them to your
supervisor.
4. Do not run electrical cords under carpets or chair pads.
5. Do not overload electrical or power strips; only one power strip per outlet.
6. Do not use power strips as extension cords by plugging one power strip
into another.
7. Make sure all electrical equipment has been tested by an approved
laboratory. Most often you will see an UL stamped on the equipment
which indicates the equipment has been tested by a certified laboratory.
8. Tenants should refrain from using space heaters. Flammable items
[paper, curtains, etc.] within 36 inches of the space heater could create
conditions for a fire.
9. Do not let trash overflow in wastebaskets or collection areas.
10. Do not block corridors and stairwells.
11. Do not prop open stairwell, corridor or other fire doors. If these doors are
propped open and there is a fire, smoke can easily spread throughout the
building.
12. Never store anything in the stairwells. There should be nothing in the
stairwells that can restrict traffic flow.
13. Check lighting in corridors, stairwells and exit signs. Report any
malfunctioning lights.
14. When you leave your office, keep doors closed. In the event of a fire,
closed doors will limit the spread of fire and smoke.

Legal Requirements for High Rise Buildings
Title 19 Sec. 3.09 – 3.10 (Pertaining to high-rise)
 Emergency procedures information shall be provided to all ambulatory, nonambulatory, and the physically disabled, instructions to be followed in the
event of an emergency. Emergency procedure information shall include, but
not limited to the following:
 Location of exits and fire alarm initiating stations, if required;
 What the alarm, if required, sounds and looks like (audible and visual
warning devices);
 Fire Department emergency telephone number 9-1-1; and,
 The prohibition of elevator use during emergencies, if any.
(c) High rise office buildings, and Group L Division 1 and 2 occupancies as
defined in the State Building Code.
1. Emergency procedure information printed on a floor plan shall be
posted at every stairway landing, at every elevator landing, and
immediately inside all public entrances to the building. The
information shall be representative of the floor level and be posted so
that the bottom edge of such information is not located more than 4feet above the floor, where it can be easily identified. Emergency
procedure information shall be printed with a minimum of 3/16 nondecorative lettering providing a sharp contrast to the background.
2. Emergency procedure information shall include, but not limited to that
described in subsection (a) (2) (A-D).
(d) Owner(s) and operator(s) of high-rise office buildings and Group L
Division 1 and 2 occupancies as defined in the State Building Code shall
appoint a Fire Safety Director, who shall:
(1) Report to owner(s) or operator(s);
(2) Coordinate fire safety activities of the facility with the authority having
jurisdiction;
(3) Conduct, or cause to be conducted, all training described in
subsection (e), for all building employees and maintain records of
dates, subjects, and attendance of each training session; and,
(4) Develop and maintain a written facility emergency plan acceptable to
the authority having jurisdiction. Upon request, the facility emergency
plan shall be made physically available at the respective facility to the
authority having jurisdiction. Facility emergency plans shall include,
but not limited to the following:
a. Fire Department emergency telephone number 9-1-1;
b. Other emergency response telephone numbers;

c. Evacuation or relocation plan for the building occupants;
d. Duties of Fire Safety Director and other designated
emergency personnel;
e. Building employee responsibilities in case of emergency,
including individual assignment and reporting responsibilities;
and,
f. Procedures to identify and assist the non-ambulatory and
physically disabled.
(5) Assure that the requirements of subsection (d)(4)(F), procedures to
identify and assist the non-ambulatory and physically disabled are
accomplished as follows:
a. Owner(s) or operator(s) of high-rise office buildings shall
maintain a list of all permanent building tenants who have
disabilities. Building owner(s) or operator(s) shall be notified
in writing by those who have disabilities. Information provided
in the list shall include any special emergency evacuation
needs and permanent work location of such physically
disabled persons. The list shall be located in the building
manager’s office.
(d) High-rise office buildings shall conduct annually emergency procedures
training for individuals listed in subsection (d) (3). Group I, Division 1 and
2 occupancies as defined in the State Building Code.
(1) Fire Safety Directors and their designated emergency personnel shall
receive training in the identification and use of facility safety
equipment, communications procedures, people movement
procedures, fire prevention practices, and their duties outlined in their
respective emergency plan. The training curriculum shall be
approved by and made available to the authority having jurisdiction.
(2) Individuals designated in subsection (d)(3) shall receive training
covering the identification and use of facility fire safety equipment, fire
prevention practices, and appropriate procedures to follow in the
event of a fire.
(3) Actual evacuation or relocation of building occupants pursuant to
procedures contained in the emergency plan shall be conducted at
least annually by those individuals designated in subsection (d)(3).
Appropriate records, including dates, floors or building involved, and
persons conducting evacuation or relocation procedures shall be
maintained and made immediately available to the authority having
jurisdiction upon their request. The authority having jurisdiction shall
be notified not less than 48 hours in advance of such planned
evacuation or relocation.
Fire drills should be a practice of the building’s emergency procedures and a learning
experience for tenants and Building Staff.

Title 19 Section 3.09 (d)5(b)
“Assure that the requirements of subsection (d) (4)(F)” procedures to identify and
assist the non-ambulatory and physically disabled as follows:
Hotels, Motels, and Lodging Houses shall comply with subsection (b) (3);
b(3) Hotels, Motels, and Lodging Houses shall maintain at the registration
desk a list noting the guest room assigned to the physically disabled guest who
have special emergency evacuation requirements. Inn-Keeper shall provide a
place on the registration form so that guest may be identified who may require
special emergency evacuation because of a physical disability.
(B) Owner(s) or operator(s) of high-rise office buildings shall maintain a list of all
permanent building tenants who have disabilities. Building owner(s) or
operator(s) shall be notified in writing by those who have disabilities.
Information provided in the list shall include any special emergency evacuation
