RENTAL REQUEST FOR PROPOSAL
Please complete and return the following rental Request for Proposal (“RFP”) list with the requested
date(s) of your Event. Please complete this document with as many details as you may have at this
time so that we may ensure that The Light Box is prepared according to your specifications. Here are
some hints to assist you:

Any information will help - don't worry if details pertaining to some of our questions (i.e. arrival
times, etc.) are not available at this time.
·
There is a Sketch Plan of the Stage on the last page. Please feel free to scribble on it to show us
(i.e: where the piano goes, where you need special lights, etc.) your plans. Make as many copies
as you want.
·
This list is very general list for use by many different users. Please feel free to add notes in the
margins or attach extra pages.
·
If you have a Technical or Production Rider to your Contract with the Artist, it is important that
you please attach a copy of the Technical or Production Rider to this completed document when
you return it to us.
Completed RFP’s must be e-mailed to: estelle@miamilightproject.com.
Response will be issued with 48 business hours.
