Thank you for contacting our office regarding your vehicle’s total loss financed through Lender
Name, loan number Account/Loan #. Our goal is to process your claim as quickly as possible.
Listed below are the documents required in order for us to review your claim request. If you do
not have the required documentation you may obtain it from your lender, insurance company, and
dealership.
Required Documentation
Copy of the Retail Installment Sales Contract
(Loan/Lease Agreement)
Payoff Amount as of date of loss and full loan history
Vehicle Valuation Statement
Copy of Insurance Settlement Check
Copy of Police Report
Copy of your Insurance Policy (Declaration page only)
Copy of Factory Invoice
(Only if the vehicle was new at time of purchase)
Buyer’s Order (Motorcycles only)

(*) If you do not have primary coverage, please contact our claims office immediately and provide the
address and telephone number of where the vehicle is located for an appraisal.

Please contact your dealership to request the cancellation of the following products you may have
purchased: Extended Warranty/Service Agreement, Maintenance Agreement, Credit Life and
Disability, Etch or any other cancelable refundable products.
To help assist you in obtaining the necessary documents included are letters which you can
complete and mail to the issuing Lender/Credit Union, Primary Insurance Company and/or
dealership.
