MATTHEW J. MURPHY, Ed.D.
Director of Mathematics K-12
This is a letter of understanding between HPS and
a TI-84 Plus, school edition graphing calculator.
, the borrower of
I agree to participate in the Calculator Loan Program under the following conditions:
1. I acknowledge that the TI-84 Plus calculator provided by HPS is and will remain the property of HPS.
2. I understand that it is my responsibility to take care of the TI-84 Plus calculator and ensure that it remains in good
working condition.
3. I agree to return the TI-84 Plus and all its parts in working condition at the end of the current school year (or at
the end of August if borrowed over the summer). If I move to a different school district during the current school
year, I agree to return the TI-84 Plus to the school district prior to moving.
4. I understand that I will be issued only one calculator by the school district each school year and that I am
responsible for taking care of the calculator so that I am able to return it to the school district in the same
condition as I received it.
5. I agree to bring the assigned calculator to class each day and turn it in for recalibration before I take a Regents.
6. I understand that if I don’t return the calculator to the school district at the end of the school year in working
condition, I will be responsible to the school district for the cost of the replacement of the calculator.
Please check one of the options written below and then sign and return this form to your math classroom teacher. A
student will not be issued a calculator until this agreement is signed.
I have read the above, and am aware of my child’s responsibility with respect to the school-district issued graphic
calculator and have decided to accept a graphing calculator.
I have decided not to accept a graphing calculator from the school and understand that I will need to purchase a
calculator in order to fulfill the course requirements.
